EE was using left hand to wipe dirt off excavator fuel filter part number when EE cut left index finger on the fan.

The employee was instructed to wash down under the transfer tower. EE was attempting to load a 70' wash down hose into the back of an F150 when EE felt EE's knee pop. EE continued to work and then while unloading the hose EE's foot got stuck in the mud and tried to free it hearing EE's knee pop a second time.
Normal Mine working conditions. Compressor was started and the air Valve was opened. Employee noticed an air Leak and was going to close the air valve to relieve the air pressure before tightening the hose fitting. As the employee was about to turn off the air valve, the hose fitting disconnected from the air compressor and struck employee in the arm.

EE was seen at Corporate Occupational Health Solutions for a contusion and open wound to left foot. He received modified work restrictions. No lost time from work.

EE WAS RUNNING A VOLVO LOADER AT THE SURGE PILE. A LARGE ROCK SHIFTED IN THE BUCKET, CAUSING THE LOADER TO BE JERKED OR JARRED AROUND. THE EE FELT PAIN IN HIS LOWER BACK JUST AFTER THIS HAPPENED.
