EE was using left hand to wipe dirt off excavator fuel filter part number when EE cut left index finger on the fan.

The employee was instructed to wash down under the transfer tower. EE was attempting to load a 70' wash down hose into the back of an F150 when EE felt EE's knee pop. EE continued to work and then while unloading the hose EE's foot got stuck in the mud and tried to free it hearing EE's knee pop a second time.

Normal Mine working conditions. Compressor was started and the air Valve was opened. Employee noticed an air Leak and was going to close the air valve to relieve the air pressure before tightening the hose fitting. As the employee was about to turn off the air valve, the hose fitting disconnected from the air compressor and struck employee in the arm.

Employees left foot was fractured when it was caught between feed plate and drum of crusher while trying to change out blow bars.

Employee held shop door open for supervisor to exit (supervisor was carrying oil). Employee then exited and caught right index finger between door and frame on the hinge side (the door swings in).

Employee was mounting the first step on a haul truck when his left hand slipped off the handrail and he pivoted to the right and fell to the ground.

Employee was exiting the cab of a loader when his left foot made contact with the ground and slipped on the ice tearing the tendon in his leg.

Employee strained lower back while performing unit weight samples in the lab.

EE was assisting as a spotter for a forklift operator lifting a 41 foot I beam. Weight shifted falling on the EE's left foot causing bruising and swelling. EE was seen at Corporate Occupational Health Solutions for a contusion and open wound to left foot. He received modified work restrictions. No lost time from work.

WHILE TIGHTING SCREEN BOLT PULLING RACHET TOWARD HIM, BOLT SLIPPED AND HANDLE END OF RACHET HIT HIS FOREHEAD

Employee was moving spare parts at the plant. He was engaging a chain through materials to be moved. One part shifted position and his hand was caught between the two parts.

Windshield wiper traveled off glass area during heavy rain storm & he attempted to move it back.

ALLEGED SHOULDER INJURY WHILE WORKING IN CONGESTED AREA UNDER INCLINED SCREEN.

8 days after helping other ee's work on #4 screen support tubes he complained of soreness in left shoulder.

EE WAS RUNNING A VOLVO LOADER AT THE SURGE PILE. A LARGE ROCK SHIFTED IN THE BUCKET, CAUSING THE LOADER TO BE JERKED OR JARRED AROUND. THE EE FELT PAIN IN HIS LOWER BACK JUST AFTER THIS HAPPENED.

EE WAS LOCKING UP THE PARTS TRAILER AT THE END OF THE SHIFT. AS HE STEPPED ON THE WOODEN STEPS, ONE STEP BROKE CAUSING HIM TO TWIST HIS LEFT ANKLE.

EE WAS SHOVELING STONE OFF A STALLED CONVEYOR BELT.

INSTALING COUNTER WEIGHTS ON A CONE CRUSHER WHEN FOOT SLIPPED CAUSING EMPLOYEE TO HIT HIS RIGHT SHOULDER ON STEEL FRAME.

HELPING CHANGE FLAT TIRE, EMPLOYEE STRUCK NOSE WITH BAR. (SUTURES X3 NOSE).

EE STATES WHILE MOVING A CONCRETE SUPPORT BLOCK FROM UNDER A CONE CRUSHER FRAME HE PULLED A CHEST MUSCLE.

WHILE WALKING DOWN SLOPE PULLED TENDON IN LEFT LEG.
