Mechanic used a chain as a puller to replace center bushings and chain broke and hit mechanic in the mouth. The lip was split open and employee was taken to hospital where it was sewn back up. Mechanic tried to come back to work the next scheduled work day but was advised to take a few days off with pay to let it start healing.

An employee was using an impact wrench to tighten a carriage bolt into a screen. He saw the bolt turning so he used his right thumb to push the bolt back into the hole. (He was on the oppsite side of the screen.) and the edge of the bolt head caught his right index finger causing a laceration.

Employee suffered a hairline fracture to hisleft hand when it was smashed between the crane pad and crib block.
While lifting on a piece of metal, employee pulled a muscle in his back.

Employee was raising screen plate and 3" pipe caught on chain hoist, causing pipe to break loose, smashing left hand & breaking it.
EE CLIMBED DOWN FROM SUCTION LINE OF SUMP PUMP (4'H) AND STEPPED ON A VALVE AND JUMPED ONTO FLOOR. HE LANDED ONHIS FEET AND LOST HIS BALANCE AND FELL TO THE CONCRETE FLOOR AND HIS HEAD STRUCK CONCRETE CAUSING A CUT OVER LEFT EYE.
The employee call the supervisor on this job on January 9, 2005, to report this accident, he did not mention anything to him on the date he said it happened and continued to work that shift as well as every day that week. The employee said that his knees were sore, he was pulling the hose for grout down the hill and hurt his knee.

WHILE BEGINNING TO SWITCH POSITIONS IN THE MACHINE, EE WAS ATTEMPTING TO ADJUST THE SEAT BY USING THE DOOR FRAME TO PULL SEAT INTO POSITION. DURING THIS THE CONTRACT EE CLOSED THE DOOR, INJURING THE EMPLOYEE'S FINGERS.
