The injured employee was picking up a 1'x2' ceramic embedded rubber panel and when EE went to set it down to readjust their grip, the panel slipped. EE went to grab for the panel and it pinched EE's left index finger between the panel and the toe board. EE was wearing leather impact and cut resistant gloves at the time of the injury.

Employee was welding screen 1 roolback plate when EE noticed some irritation in right eye. EE removed contact and kept working. EE went home and eye began to swell. EE texted supervisor and reported the issue. EE was taken to ER which referred EE to eye doctor.

Miner was installing channel to structure. Channel was supported on fork lift forks and secured w/a chain fall. Channel slipped off forks pinching the miners right hand.

IE and another EE were changing bearings on conveyer. IE pulled back the pulley so that other EE could hit the bearing with a 6 lb soft face hammer. When hammer struck it bounced off and hit IE's left pinky finger. IE performed basic first aid and made a splint for finger and continued working till end of shift. Went to ER has fractured finger.

While removing a stuck access door on the back of a vibratory screen deck, the door suddenly released, causing the employee to strain his left arm when he tried to catch the falling door. He was diagnosed by a physician with a hyperextension of his arm. He was returned to work on restricted duty.
