Employee was walking on float of dredge to do maintenance, and stopped and stepped backward, tripped and fell on right shoulder. Employee's shoulder has arthritis and the fall onto the shoulder aggravated it. Employee received a Toradol shot at clinic.

EE was scraping dirt into a pile with loader and hit a dip in soil jarring the loader and straining neck / shoulder. EE injured it on 11/13/17 and reported it on 11/17/2017.

Employee was tightening a bolt on the shaker screen wear rubber with a ratchet and socket. The socket was not seated well on the bolt and it slipped and hit EE in the cheek chipping tooth and caused EE to bite tongue. EE was sent to see the dentist for the tooth.

EE is our dredge operator and a mechanic, and he was repairing an outboard motor on the Jon boat when it "snapped" or broke free from boat's connection and his left hand was in the path of the fly wheel when the motor rose up/throttled out of the water during mechanical repair.

EE was assisting a welder replace plates on scales at the I-164 Sand Pit, and sometime during the course of the day he received a flash burn which was not identified/noticed until later that night. He went to the ER because of a burning feeling. He did not remember ever directly seeing the flash, but did indicate he knew the work was in progress.

Huston reported to his supervisor that he was suffering from pain radiating from shoulder to hand. He was sent to medical provider and diagnosed with tendonitis in his left arm. He is a loader operator and the exact cause is undetermined.

Standing on a platform changing ribbon on a ticket printing machine. When he stepped off the platform to the floor he twisted his foot causing a fracture to his right foot.
Using a sledge hammer and chisel to remove a bushing from a tail pulley. Missed the chisel and struck his left hand resulting in a mashed thumb.
