using only the upper portion of a 20' extension ladder, our employee climbed up to inspect a faulty drive motor on our vsi crusher output conveyor. the ground was wet and the ladder slipped out from beneath EE causing EE to fall approx. 6 feet dislocating EE's left hip and fracturing EE's pelvis. unsafe work practices

While removing the return roller from conveyor #11, the bolt on the bracket snapped causing the roller to fall on the left ring finger of the employee therefore causing a laceration to same.

A mechanic, working on piece of equipment in the shop. Worker was using parts cleaner to degrease a clutch. Worker then violated policy and procedure by using a hand-held butane torch. There was a flash from combustible material that caused burns to the face and head of the worker. Worker was conscious and talking. He was taken to the hospital by another worker was admitted.

While changing a screen on a portable crusher, the screen slipped down and pinched right pinky finger.
An elderly male, (non-employee) apparently suffering from early stages of alzheimer's disease and dementia, trespassed onto posted mine property. He most likely became disorientated and died of hypothermia. The pit was not in operation at the time.
