On 11.21.11 EE went to start up a Goodwin Submersible Pump when the hose which was attached jumped and struck employee in lip causing laceration. EE received stitches and was released to return to work. No medication was administered.

EE WAS GETTING OUT OF MAXISWEEPER WHEN HE PULLEDHIS LOWER BACK. HE FIRST SAID HE HAD A KIDNEY STONE AND IT WAS DECIDED ON 4/26 BY HEALTH CARE THAT IT WAS A MUSCLE.
EE WAS GOING TO FUEL UP A CAT BACKHOE. THE TRACKWERE WET WITH MUD. SLIPPED AND HIT HIS KNEE ON THE TRACKS. EE DID NOT GIVE US THIS INFORMATION UNTIL 3-19-01 AND THEN ONLY SAID HE HAD FLUID ON HIS KNEE. NOW HE HAS CRACK IN KNEE CAP 4-26-01.
