EE claims they injured their left shoulder while climbing out of a chute. This incident did not become reportable until 12/5/2023 due to the employee going to the doctor on 12/4/2023.
Employee was attempting to shovel material underneath a conveyor when EE felt a pop in their left shoulder.

Driver was attempting to cut the end of a hose when the safety knife slipped, and EE cut self on the top of EE's right thigh a few inches above EE's knee. EE required 3-4 stitches.

Employee was dismounting from the Cat 988F loader located inside the plant shop. There were some loose rocks on the bottom step, employee's foot slipped on loose rock and he lost his balance, he tried to catch himself with his left hand and fractured the small bone between his thumb and wrist.

2 employees were performing routine maintenance on the Komatsu PC300 Rock hammer. They were replacing a section of the hydraulic hose from the boom to the hammer. Injured was loosening the nut on the hammer end & co-worker was using his pocket knife to cut tie straps which were holding a smaller hose to the one they were replacing. He cut a strap & knife hit EE in lower right leg.

EE WAS LOADING A FULL OXYGEN BOTTLE ONTO THE WELDING TRUCK. HE LIFTED THE BOTTLE BUT WAS NOT ELEVATED ENOUGHT TO PLACE IT ONTO THE STAND. HE GOTON HIS TOES AND ARCHED HIS BACK TO GAIN HEIGHT. HE FELT A PAIN IN HIS LEFT SHOULDER BLADE.
EE WAS INSIDE OF A SCREEN. HE WAS LAYING ON HIS BACK USING HIS FEET TO PUSH A PIPE WRENCH TO TIGHTEN SCREEN BOLTS
EE WAS LEVELING THE FLOOR IN THE PIT. AS THE EE WAS DRIVING FORWARD, THE LOADER BUCKET CONTACTEDA TOE STOPPING THE LOADER IMMEDIATELY THE EE'S KNEE CONTACTED THE DASSH

TWO EMPLOEYE WERE TIGHTENING THE LOCK NUTS ON THE FRAME BOLTS OF A JAW CRUSHER. ONE STEADIED THE WRENCH WHILE HTE OTHER STRUCK IT WITH A HAMMER. DURING THIS ACTIVITY. THE WRENCH SLIPPED AND STRUCK THE EMPLOYEE IN THE FACE.
THE EE HAD BEEN OPERATING HIS HAUL TRUCK FROM THE PIT TOT HE PRIMARY ALL DAY. THE TEMP WAS IN THE HIGH 90'S. THE AIR COND IN HIS TRUCK HAD QUIT WORKING DURING HIS SHIFT. HE HAD A COOLER FULL O F ICE AND DRINKS IN HIS CAB. AFTER DUMPING HIS LOAD HE PULLED TO THE SIDE OF THE HAUL ROAD AND PARKED. MOMENTS LATER, HE PASSED OUT.

EMPLOYEE WAS ATTEMPTING TO REMOVE A PIECE OF SCRAP METAL FROM THE PRIMARY CRUSHER FEEDER. AS HE WAS STANDING IN THE FEEDER, PULLING THE METAL WITH BOTH HANDS, A ROCK ROLLE DONTO HIS LEFT HAND SCRAPING IT AGAINST THE METAL.
