EE was standing on a shuttle car cable with conversion valve out. As EE put the valve back in the cable took up while EE was standing on it, and it flipped EE over on right shoulder. X-rays showed no breaks and EE is home waiting an MRI.

Pinner steel got stuck in top, it fell out hitting employee on right middle finger.

Was laying rail and the chain broke from rail to scoop, and the rail fell about 4 inches on top of their right foot. X-rays were negative.

Pushing a small rock while bolting and bend back on left thumb. X-rays were negative and EE came back next shift. They gave EE a splint to wear on thumb.

EE got hurt on 8-26-19 and continued to work until September 4th. 9-5-19 was the day EE went off waiting an MRI.

Had a bent bolt close to top, tried to straighten it with rotation, the bolt came around and hit employee on left wrist.

Was dropping down cable and belt hanger got caught in right pointer finger. No stitches were required but they removed the hanger at hospital.

At 1030 am range a pre-shifter found a rock fall on break 23 on 3A belt we called the 800 number and MSHA and State has came out an look at it.

Got off 12 ton motor on bottom of slope to hook up supply car when the employee got foot caught between 12 ton and mine floor.

Employee was struck by a rock on hand causing injury

Was outside top of slope picking up a battery and felt pain in lower back. They gave the employee a back brace to wear.

Loading roof bolter and as they threw a bundle of plates towards bolter they hit right pointer finger on rope bolt it was a small cut that had one butterfly bandage put on it.

Injured was cutting entry when a piece of draw rock (2" to paper thin) fell from in between rib and bolt striking them on lower back area. The X-Ray and CT Scan taken at ER were negative and employee was suppose to return to work. Another doctor took them off pending MRI.

A non-injury roof fall occurred at #5 belt entry intersection and crosscut over to #6 entry travel way or secondary escapeway. One crosscut out by survey station 12206 in the #9 entry.

Injured employee was installing roof bolts. The bolt was bent so that they could be installed. As EE tried to straighten the bolt EE felt a pop in EE's right shoulder.

EE was working on 7 section and was using utility knife and cut right finger. EE saw nurse, did not need stitches but nurse put a butterfly strip on cut.

Injured was cleaning dust box on roof bolter. When EE shut lid a small piece of rock hit EE on the top of EE's left foot.

Walking past bolter in back entry on #5 entry on 5 section and tripped on gob and hit t-bar flab hurting nose. Came back next shift but was treated with ice to nose area.

Employee was setting timbers. EE picked up a timber and felt pain in lower back.

Employee was moving from seat to seat on the shuttle car, to change direction, when employee felt pain in lower back.

A roof fall occurred at #8 entry. The estimated size of the fall is 18 ft. wide x 50 ft. long x 6 ft. high. No injuries were reported as a result of the roof fall.

Employee was putting up curtain board when drill steel got stuck. EE was pulling on steel and board slapped top causing a piece of rock to strike ee on finger pushing it against pot of roof bolter resulting in a laceration.

Employee got their glove caught between steel and wrench while bolting. This caused their hand to be twisted.

Employee was cleaning rock duster when EE cut finger on the auger.

Employee was using starter steel to start inside bolt. When drill steel contacted the roof a small piece of cap coal fell making contact with EE's left thumb.

Employee was getting off scoop when a small piece of draw rock fell striking EE's finger.

Employee was putting 12 Ton on charge. Safety chain went under motor and came back striking the employee on left index finger.

Employee was putting up outside roof bolt using ee body to push it into hole. EE stated knee popped and started burning. EE continued working until 7/20/18 giving an excuse to be off until 7/24/18. EE did not return to work the next week and when contacted ee had doctor send excuse stating ee would be off until 8/28/18 due to knee.

Employee was hanging scoop cable when they struck their right middle finger against a roof bolt plate. This resulted in a small laceration that required 3 stitches to close.

Employee was loading supplies onto roof bolter when EE stated EE hurt left shoulder. Started as an ache and continued to get worse. Felt a pop. EE worked the entire shift and left for the ER at 3:00 AM on 03-16-2018. EE is off work pending a MRI.

Employee was carrying a half bag of dust as they were rock dusting around section power center. As they stepped over chain at power center they felt a pop in left knee.

Employee was tramming roof bolter from deck. Employee's hand slipped off tram lever. Employee's arm was caught between deck and rib.

Was at 5 section supply hole getting out of scoop, and a piece of draw rock hit EE in hard hat. Head ache was only complaint. Doctor took EE off for 3 days, and EE has came back full duty.

Employee lifting lid to scoop battery box and was putting water in the batteries. The lid slipped out of employee's hand catching the tip of employee's right ring finger. This resulted in a small laceration to the tip of employee's finger.

Employee was walking to roof bolter to move it up when a thin piece of fallen material struck employee on left arm causing a laceration. Employee returned to work on next scheduled work day which was 09/05/2017. Mine was idle from 09/02 thru 09/04/2017 due to Labor Day Holiday.

A roof fall occurred at break 29 on #4 belt. The estimated size of the fall is 20 ft. wide x 20 ft. long and 5 ft. high.

Employee was putting up a roof bolt when EE mashed EE's right pinkie (little) finger between a roof bolt and pot of the roof bolter. On 08/11/2017 EE worked 5.5 hours then left to go to the doctor for a follow-up on EE's finger, at which time the doctor took EE off work due to a minimal distal fracture on the tip of EE's right little finger. EE began missing work as of 08/12/2017.

Employee was setting up roof bolter to bolt last row of bolts, jerked down a small coal brow with T-Bar. It landed on top of the canopy then slid off striking employee's right thumb.

Bit on end of steel was stuck and employee was hitting steel on pot of roof bolter trying to knock it loose. Employee struck their right pointer finger on the pusher plate causing a laceration which required stitches to close.

Employee was picking bundle of roof bolt plates up when ee felt something pull in right arm.

Employee was loading supplies into scoop bucket when a piece of rock fell out from between bolts striking ee on right forearm. This resulted in a laceration which required stitches.

Employee stated was pushing finisher into hole that had just been drilled. It hung up and employee felt something pop in lower back. Employee did not miss any work until 04/10/2017.

Employee was loading pizza pans in scoop bucket and as EE was laying them down EE felt left shoulder pop.

Employee was trying to straighten a bolt, the bolt turned causing employee to strike a shoulder on the canopy post.

Employee was spot bolting in return. Had finger over top of steel, pushing up to top. Caught finger between finisher steel.

Employee was pushing bolt up into top when a knot popped up above groin area. Employee did not miss work until 03/03/2017.

Employee was removing a rock from top of miner and lacerated middle finger on right hand. Required stitches.

Employee was driving a stinger and ran over a crib block. The steering wheel spun and the steering knob struck the right hand. This resulted in a fracture to the 5th(pinkie)finger.

Employee was picking up trash when a small piece of draw rock fell striking employee's right forearm resulting in a small laceration.

Employee was pushing a rock off fender of shuttle car when the rock rolled up and struck the top of left ring finger resulting in a laceration.

Employee started drill steel when a small piece came off the steel cutting employee on the left arm just above the kevlar sleeve. Required 3 stitches.

Employee was helping make a belt splice. As EE was running EE's hand along the splice, making sure splice was braided, another employee struck EE's hand with a hammer. Resulting in injury to EE's left middle finger.

Employee was preparing to hang a ventilation curtain extension when EE stepped in a hole twisting right knee.

Employee had just dropped underground and was getting on 6 ton to go to section when they twisted their left knee.

Employee was hanging belt chains when a piece of draw rock fell striking lower back.

Employee was pulling a rock duster hose when employee slipped and fell striking hand on a rock on bottom of mine floor. This resulted in a small laceration to left hand requiring 2 stitches.

Employee was walking across section toward miner when employee walked into a roof bolt and fell striking left shoulder. Off work pending test.

Employee was installing a roof bolt when the plate wire caught glove causing injury to right arm. Employee did not miss work until 07/14/2016 when doctor removed employee from work for evaluation.

Employee was walking across section when stepped in a hole (indenture) in mine floor. Doctor took employee off work on 06/20/16 pending the results of any MRI.

Employee was straightening out a 6' roof bolt when EE felt pain in right wrist. Mine personnel were not aware that EE was having any problem until EE brought in an off work doctor's excuse on July 11, 2016, the day the mines returned to work after regular mine vacation.

Employee was drilling a hole for roof bolt when the drill steel fell out striking EE on the right thumb causing a laceration.

Employee walked into an anchor bolt causing a laceration to lip.

On 7 section in # 7 entry the Continuous miner hit a core hole in the top right of entry. Water came in the mines. No one was hurt, and MSHA 800 number was called. It was investigated by MSHA, and after grounding the hole. We are back to normal production.

Employee split half header to build crib. Hit half header on belt rail smashing left middle finger.

Employee was in the warehouse before EE's shift started. EE was cutting a zip tie, and the knife slipped cutting EE's right thumb. The laceration required 6 stitches. EE returned next shift to regular job duty.

Pulling up a buggy cable behind 7 section power box. Hit hard hat on top hurting neck, and lower back.

He was running a roof bolter. As he put his steel to top. A small piece of draw rock from the steel hit him on forehead causing a laceration that required 7 stitches.

Employee was helping load bolts on roof bolter on 1 section. As he loaded the bolts he said he felt a pain in lower back. He did not go to see doctor, and continue to work every day till Oct 6. He never said anything the whole time or on Oct 6 about back hurting. He called us Oct 7th, and said his doctor had him off on the strain from the 9-14-2015 incident.

Employee was drilling a 10 ft bolt. The drill steel came out of top hitting him on right pinkie finger that caused a laceration that required 5 sutures. He returned next shift.

He was on a scoop taking down old 5b head. He ran over a block hurt upper back. He finish shift. He continued to work everyday regular duties until 11-9-15. His first day missed was on 11-10-15.

Employee was running a continuous miner on 5 section. He was turning 8 right when a piece of falling material hit him on right hand causing a laceration. He had sutures to close the wound.

Using a hydraulic ram pressing a bearing in a Joy Miner frame, the ram slipped hitting employee in the right knee

Employee stated he was helping lift mine cable to hang when he felt pain in lower back area. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. We do not agree that this was a reportable injury. We do not accommodate "restricted work activity" at Excel Mining. Time sheets show he returned to his regular job the next day.

MSHA has requested the filing of this Form 7000-1 in order to terminate a citation. EE informed mine personnel that he had back spasm on 4-24-15. EE informed mine personnel of previous back issues, and MRI shown a bulging disk. Excel Disagrees with citation believes no work-related accident occurred, and is completing form 7000-1 under protest.

Employee was hooking up a chain from shuttle car to shuttle car. He did not know that a scoop had come around to push shuttle car back. he got his left leg caught between the two shuttle cars that resulted in a fracture to left ankle.

According to MSHA Inspector, Employee operating shuttle car hit hole, hit head on canopy. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. This is filed under protest. Excel Mining disagrees that a job related injury occurred.

Employee was pulling a rock off top of miner. Cut pinkie finger. Employee sustained a laceration that was closed with Dermabond. We were not aware that using Dermabond to close a laceration made it a reportable injury.

Employee was drilling a hole when his partner thought he saw a flash from the hole being drilled.

At around the 9:30 am range on #3 section in 7 right entry. A one second flash of methane occurred on the left bolter as he was installing a roof bolt. We called MSHA, and State on the 1 800 number, and after we were informed of a K order we evacuated the mines.

Employee was walking back to his scoop, and hit a torque tension bolt that caused a laceration to top of head. that required 4 staples to close wound. He returned next shift to regular job duty.

We found a fall in Panel 1D # 6 entry 2nd cross cut outby. Measuring 6 feet high by 18 foot wide by 40 foot long. No one was hurt, and they cribbed, and dangered off, and have another entry to travel.

EE was carrying a roll of fly pad material. When he threw it to the ground pain in his lower back area. 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. Do not agree that this was a reportable injury. Do not accommodate "restricted work activity" at Co. Time sheets show he returned to his regular job the next day.

Employee was carrying a timber to be set on the 3 section pillar section. He felt pain in lower back area.

EE was waiting on miner when a piece of falling material fell striking him on his middle and ring fingers. 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. Did not know until this time that the ee had a fracture to the tip of his finger, due to the fact that he returned to work the next day to his regular job.

He was putting up a roof bolt. The bolt bent. He accidentally hit the rotation. The bent bolt hit him on his left wrist resulting in a fracture.

Employee was operating battery scoop. According to MSHA inspector AR#23903, back injury occurred. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. This is filed under protest. Excel Mining disagrees that a job related injury occurred.

Employee was installing a bottom roller at the bottom of the slope belt. A belt rail slipped out of the belt hanger striking him on left hand causing a hair line facture to the pinkie finger on his left hand. He returned the next to regular job duties.

He was in the process of placing bolt into pot when he bumped the rotation. A burr on the bolt caught his glove resulting in two facture fingers to left hand.

Employee was in the process of splitting a half header by striking it against the scoop bucket, when a piece of the half header struck him in the lower lip causing a laceration that required sutures to close.

While in the process of shoveling belt employee strained his lower back.

Employee was checking voltage on each phase of the section power center, for unknown reason the multimeter he was using short circuited producing an electrical flash that resulted in burns to his face and hands.

Employee was in the process of installing permanent roof support as he leaned forward to straighten bolt he lost his balance causing him to fall forward, mashing the tip of his right index finger between the roof bolt plate and the boom of the bolter. He was taken to local ER and referred to a hand specialist. Surgery was needed to repair the damage to his finger.

EE was bending bolt so he could install it into hole. Bolt slipped causing him to experience pain in his shoulder. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. We do not agree that this was a reportable injury. We do not accommodate "restricted work activity" at Excel Mining. Time sheets show he returned to his regular job next day.

Employee was traveling the track on a battery rubber tired 4 wheeler, when the front tire of the 4 wheeler struck an unseen object on the track. This caused the steering wheel to spin, striking employee's right hand, resulting in a fracture.

While working on airlock doors EE moved to let mantrip open doors. A piece of falling material fell striking him on back of neck causing him to fall to ground. This is being filed to terminate citation written during a Part 50 Audit. We do not agree this was reportable. We do not accommodate "restricted work activity". Time sheets show EE was back at regular job next day.

Employee was tramming a continuous miner across the entry toward the air current when the tail boom struck the mine roof causing some dust particles to fly into his right eye. Employee was taken to a local eye doctor where he was given antibiotic drops.

2 ee working with a DC powered welder and oxygen/acetylene touches to repair a cutting bit lug on a continuous miner. Rubber hoses of the cutting torches accidental came in contact with the hot case of the bonder, melting the hoses and igniting the gases, causing a fire. Fire lasted 10 to 12 min was extinguished with water. No inj were reported due to this fire.

A roof fall was discovered by the mine examiner while traveling the return air course in #2 entry inby spad 450. The estimated size of the roof fall is 30'long, 20'wide , and 8'high. No injuries were reported as result of the roof fall.

Employee was moving Vent curtain and stepped in a hole twisting knee.

Employee was operating a battery powered 4 wheeler. While traveling the track entry the 4 wheeler slid off the track causing employee's neck and lower back to be jarred. Employee was seen by local physician who removed him from work pending MRI results.

While in the process of using a jack and chain to adjust belt structure, jack slipped causing employee's right middle finger to be mashed between the chain and belt structure.

Employee was in the process of tramming a continuous miner, as he twisted on his feet in place to watch the tail of the miner he felt pain in his right knee. Employee was seen by a local physician and was removed from work pending MRI results.

While in the process of using a utility knife to cut rope hangers from roof bolt cable, knife slipped causing a laceration to employee's right hand that required sutures to close.

Employee was bending a bolt and pulling on pot when he experienced pain in the middle of his back. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. We do not agree that this was a reportable injury. We do not accommodate "restricted work activity" at Excel Mining. Time sheets show he returned to his regular job the next day.

Injured employee was assisting the scoop operator that was scattering joints of track rail using a chain with a bolt thru the end of the rail to pull with. As he was removing the bolt, the joint rolled over and pinched his left middle finger against the mine floor. He was taken to the local ER where he received sutures to repair the finger.

While in the process of installing permanent roof support, the plate was not uniform with the bolt pattern. When employee placed his left hand to straighten the plate he unintentionally applied up pressure and mashed the tip of his left index finger between the plate and mine roof.

While in the process of trimming a belt splice using a utility knife, the knife slipped causing a laceration to his left hand that required sutures.

Employee was sitting on bench waiting for hoist car. According to MSHA inspector AR#23903, EE rose up and twisted knee. We have no report of injury by which to gather information to complete this form. Employee is not available to gather information. This 7000-1 is being filed in order to terminate a citation written during a Part 50 Audit. This is filed under protest.

Employee had been kneeling to work on continuous miner, as he started to rise his right knee popped. He was seen by a local physician at a local hospital and released to return to work. Employee continue to have pain, on 2/25/13 he was removed from work by a specialist.

Employee and a co-worker was dropping the miner cable and water line across the no. 7 entry when the cable dropped the metal hook of a rope hanger hit his right hand between the thumb and index fingers creating a puncture wound. The wound was cleaned and 1 stitch was installed to hold the wound closed.

Employee was in the process of installing the outside roof bolt in the last row, he had just put the two pieces of drill steel together and was in the process of starting to drill while applying up pressure when the collar broke resulting in the bottom piece striking him on the left side of his face.

Due to the coal seam height, the employee was bending a 6' roof bolt preparing it for installation when he felt a sharp pain in his lower back.

Employee was operating a battery power scoop lapping up conveyor belt, he exit the scoop to help lift one of the belt loops when he felt a sharp pain in his lower back. Employee was seen by a local ER physician and was removed from work.

Employee was lifting large rock out of sump. Strained groin

Employee was unloading jackleg from shaft for repair, drill folded back pinching middle finger right hand. 8 stitches
