THE EMPLOYEE GOT OUT OF THE MINER DECK TO HANG THE LINE CURTAIN ACROSS THE ENTRY PRIOR TO TAKINGA CUT OUT OF THE NO. 3 HEADING ON THE 002-0 SECTION WHEN HE FELT A PAIN IN HIS LOWER BACK. HE CO NTINUED TO WORK THROUGH HIS REGULAR SHIFTS THRU 11/13/01 AND DUE TO INCREASING PAIN WENT TO THE DOCTOR ON THAT DATE. HE WAS PLACED ON RESTRICTEDWORK DUTY BEGINNING 11/14/01.
THE SECTION HAD JUST BEEN MOVED AND THIS WAS THE FIRST TIME THE EMPLOYEE, A MAINTENANCE FOREMAN, HAD OPERATED THE SHUTTLE CAR IN THIS ENTRY. AS THE EMPLOYEE WAS TRAVELING TO THE CONTINUOUS MIN ER, THE SHUTTLE CAR HIT A LARGE POT-HOLE IN THE MINE FLOOR & HE WAS BOUNCED IN THE DECK & LANDED ON THE SIDE OF THE SHUTTLE CAR DECK. 0N 10/9/01WAS DIAGNOSED AS HAVING A HERNIATED DISC IN BAC
A ROOF FALL WAS DISCOVERED DURING A CHECK OF THE SEALS IN THE AREA. THE FALL OCCURRED JUST INBY THE OLD INTAKE OVERCAST TO THE 1ST RIGHT SECTION(NO. 6 ENTRY) AND JUST INBY SURVEY STATION NO. 1 83. THE FALL IS APPROX. 50' LONG X 18' WIDE X 5'THICK AND IS APPROX. 6900' OUTBY THE WORKING SECTION AND 3200' INBY THE SURFACE. FALL IN HORSEBACK SHAPE OF LAMINATED SLATE & BOLTED W/ 36" MEC

EMPLOYEE WAS OPERATING A SHUTTLE CAR AND HE WAS TRAVELING TOWARD THE DUMP, THE SHUTTLE CAR RAN OVER A CRUB BLOCK ON THE OFFSIDE OF THE OPERATOR. THE SHUTTLE CAR BOUNCED AND THE EMPLOYEE CAME D OWN ON EITHER THE BRAKE PEDAL OR THE SIDE DECK OF THE SHUTTLE CAR. THE CRIB BLOCK WAS LEFT BESICE THE ROADWAY WHEN THE HEAD OF THE MINER WAS CHANGED OUT.
EMPLOYEE PREPARED A ROOF BOLT TO PLACE IN A HOLE HE WAS DRILLING IN THE ROOF. DUE TO THE AREA BEING WET, THE EMPLOYEE PLACED THE ROOF BOLT ON THE FOOT OF THE ATRS TO KEEP IT OUT OF THE WATER. AS HE WAS DRILLING, THE BOLTER HEAD STOPPED UP AND HE LOWERED THE HEAD TO CLEAN OUT THE BLOCKAGE. AS HE LOWERED THE HEAD THE ROOF BOLT HE PLACED ON THE FOOT OF THE ATRS ROLLED TO AN AREA WH

A ROOF FALL OCCURRED IN THE XCUT BETWEEN THE NO.'S 8 AND 9 ENTRIES ACROSS FROM SURVEY STATION NO. 531 IN THE NO. 8 ENTRY. THE FALL MEASURED 25-30' LONG BY 20' WIDE BY 42" HIGH AND FELL ABOVE T HE ANCHORAGE ZNE OF THE 36" ROOF BOLTS INSTALLED IN THE AREA. THE MINE ROOF CONSISTED PRIMARILY OF SOFT SHALE THAT WAS LAMINATED STREAKS OF 1-4" IN THICKNESS AND WAS IN A WEATHERED CONDITION.

EE ALLEGES TO HAVE CONTRACTED COAL WORKER'S PNEUMOCONIOSIS WHILE EMPLOYE AT THIS COMPANY. CLAIM WAS SETTLED AND APPROVED BY COMMISSIONER ON 5-15-98.

AS A LOST DAY BASED ON PART50.20-7, SECTION D, ITEM C OF CODE
EE BRUISED HIS RIGHT KNEE WHEN HS KNEE CAME IN CONTACT WITH THE MINE FLOOR WHILE ATTEMPTING TO CRAWL IN LOW COAL. EE WAS WORKING WITH THE CONTINUOUS MINER CABLE. THE INJURY WAS CAUSED BY A ROC K BEING LOCATED BEHIND EE'S KNEE PAD.
THE EE HAD BEEN EXPERIENCING ONGOING BACK PAIN & WENT TO THE DR IN OCTOBER. THE ATTENDING PHYSICIAN REPORTED BY CORRESPONDENCE DATED 11/20/96, THAT THE LOWER BACK PAIN WAS BEING CAUSED BY A C HRONIC CONDITION. THE EE IS CURRENTLY ON RESTRICTED WORK DUTY.
ATTENDING PHYSICIAN REPORTED THAT EMP HAS CLACIFIC TENDONITIS AND ONGOING IMPINGEMENT IN THE SHOULDER BURSA WHICH HAS WORSENED OVER TIME. DR CATEGORIZED CONDITION AS OCC ILLNESS. EMP HAD SURG ERY ON 9-30-96 AT WHICH TIME BEGAN MISSING WORK.
THE ROOF WAS IN HORSEBACK FORM WITH SLICKENSIDED SURFACES. THE FALL OCCURRED IN A BREAK BETWEEN THE INTERSECTIONS, 50 FT. X 20 FT. X 4-6 FT. THICK. THE FALL WAS IN NO. 3 ENTRY ONE BREAK INBY S URVEY STATION NO. 443, 630 FT. FROM ACTIVE AREAS.

A SMALL PORTION OF DRAW ROCK HAD FALLEN ANDTHAT BREAK POSTED OFF. THIS DATE IT WAS DISCOVERED THAT THE ROOF HAD FALLEN OUT MORE. THE DAY SHIFT MINE FOREMAN EXAMINED THE AREA ON 3-20-95 AND DET ERMINED THAT IT HAD FALLEN ENOUGH TO MAKE IT REPORTABLE BECAUSE IT FELL 5 FT THICK X 30 FT LONG X 4-5 FT THICK IN THE FORM OF HORSE BACK.
THE EMPLOYEE WAS IN THE PROCESS OF CUTTING A SECTION OF CONVEYOR BELT WITH A POCKET. THE KNIFE SLIPPED OUT OF THE CONVEYOR BELT, STRIKING THE EMPLOYEE'S LEFT THUMB CAUSING AN INCISION. STITCHE S WERE REQUIRED TO CLOSE THE WOUND. THE EMPLOYEE EXPERIENCED NO LOST WORK DAYS NOR ANY RESTRICTED WORK DAYS.

WHILE IN THE PROCESS OF INSTALLING A ROOF BOLT, THE EMPLOYEE CAUGHT HIS RIGHT THUMB BETWEEN THE MINE ROOF AND A SECTION OF DRILL STEEL. THE ACCIDENT CAUSED A LACERATION TO THE RIGHT THUMB WHIC H REQUIRED SUTURES.
AS A RESULT OF INCLEMENT A SLIDE OF MUD AND ROCK OCCURRED ON THE SURFACE IMMEDIATELY ABOVE THE NO. 4 INTAKE ENTRY. NO ROOF SUPPORTS WERE AFFECTED AND THE SLIDE WAS CLEANED UP WITHOUT INCIDENT. NOTE WERE DIRECTED BY LOCAL MSHA OFFICIALS TO REPORT THIS OCCURRENCE AS A ROOF FALL EVEN THOUGH WE CONTEND THE SLIDE DOES NOT QUALIFY AS REPORTABLE UNDER THE DEFINITION OF A ROOF FALL.
A ROOF FALL APPROX. 70' LONG X 20' WIDE X 7' THICK OCCURED ONE BREAK INBY SURVEY STATION #75 OF ABANDONED W-MAINS. THIS AREA IS APPROXIMATELY 2500 FT. OUTBY THE FACE OF 003-0 SECTION. THIS A REA WAS KNOWN TO BE ADVERSE AND HAD BEEN CRIBBED OFF AND DANGERED OFF. THIS AREA HAD BEEN BOLTED WITH 84" LONG X 7/8" DIAMETER BOLTS. IT WILL NOT BE CLEANED UP.
AN AREA OF LAMINATED SANDSTONE/SHALE MINE ROOF THAT HAD BEEN PREVIOUSLY DANGERED OFF AND CRIBBED OF APPROXIMATELY 2,500 FEET OUTBY 001-0 SECTION FELL. THE AREA WAS 4' HIGH X 20' WIDE X UNKNOW LENGTH. IT IS LOCATED ONE BREAD INBY SURVEY STATION #78 IN THE #8 ENTRY. THIS AREA HAD BEEN BOLTED WITH 48" AND 84" 7/8" DOUBLE-LOK ROOF BOLTS.

A ROOF FALL WAS FOUND DURING AN EXAMINATION OF THE RETURN ENTRIES IN THE NO.4 ENTRY, ONE CROSSCUT INBY SPAD NO.769. THE ROOF FALL IS APPROX. 20 FEET WIDE X 80 FEET LONG X 0 TO 7 FEET THICK. TH E ROOF FALL IS LOCATED APPROX. 4500 FEET OUTBY THE NEAREST ACTIVE FACE.
THE EMPLOYEE STATED THAT AS HE TURNED TO THROW A TIMBER, HE FELT A RUBBING/BURNING SENSATION IN HIS LEFT KNEE.
THE EMPLOYEE'S FOOT WAS INADVERTENTLY RAN OVER WHILE A SECOND EMPLOYEE WAS TRAMMING THE ROOF BOLTER INTO POSITION. DUE TO A SOFT TISSUE BRUISE TO THE EMPLOYEE'S LEFT FOOT HE WAS PLACED ON REST RICTED WORK ACTIVITY FROM 9-2-93 TO 9-8-93.
A ROOF FALL APPROX. 20' X 20' X 9' THICK OCCURRED 1 BREAK INBY SURVEY STA. #69 (1 BREAK OUTBY LAST OPEN CROSS CUT) OF ABANDANED "W" SECTION. THE AREA IS APPROX. 8,000' INBY PORTALS. AREA KNOWN TO BE ADVERSE CONDITIONS. WILL NOT BE CLEANED UP.
EMPLOYEE STATED THAT HE WAS IN THE PROCESS OF USING A JACK AND BAR TO RAISE A PIECE OF TRACK EQUIPMENT WHEN HE EXPERIENCED A PAIN IN HIS BACK. THE EMPLOYEE BEGAN MISSING ORK ON THE 05/18/93 S HIFT.
THE EMPLOYEE WAS IN THE PROCESS OF CHANGING A HYDRAULIC PUMP MOTOR ON A 21 SC SHUTTLE CAR. WHILE ATTEMTPING TO POSITION THE PUMP MOTOR PROPERLY IT MOVED CATCHING THE EMPLOYEES LEFT MIDDLE FING ER BETWEEN THE MOTOR AND SHUTTLE CAR FRAME.
