EE was assisting the plant welder, with fabricating brackets for the bin gate clam shell. While using a drill press and grinder small metal particles collected on the sleeves of the EE shirt. EE wiped EE's face with EE's sleeve contaminating EE's eye with the small metal particles. EE did not seek medical treatment until 5/12/2026.

Employee was working in a 5-man team to screw bowl of crusher in place. While exerting force with EE's arms EE's right shoulder was strained.
EE was using a battery powered drill when the drill bit got caught on the steel causing the drill to quickly spin around. This caused the bottom side of the drill to flip and impact EE at the top of their safety glasses, resulting in a laceration above their right eye.

An employee sustained a small fracture and laceration to the tip of EE's right pinky finger. EE was lifting a return roller onto the top of a conveyor belt to stage it until needed. EE pinched the finger between the roller and the belt.

While working on a de-watering pump the employee grabbed the v-belt of the pump and pulled on it to see if the pump was free of the obstruction they were clearing. The belt rotated and pinched the employee's left pinky between the belt and the pulley.

The employee stepped on a metal bracket to climb on to a conveyor frame EE was disassembling. The weld holding the bracket on failed causing the employee to fall approximately 3.5 feet. While falling EE attempted to grab the conveyor frame and lacerated left palm. The laceration required stitches.
Employee was using a utility knife to cut a zip-tie from a cable. When the zip-tie was cut the momentum of the knife caused it to strike EE's opposite arm causing a laceration to EE's forearm.

The employee used the back edge of pocket knife blade to remove the cap from a bottle of chalk for a chalk line. When the cap released, the front of the blade cut right thumb. The laceration required 3 stitches.
The injured employee was working as part of a team installing an I-beam on a construction project. While attempting to move the beam in place with a pry bar, the beam shifted downward and out striking the employees left thumb. It resulted in a fracture and laceration.

While the employee was moving a 55 gallon drum of oil, the contents shifted and caused the drum to stand up unexpectedly and slammed against an adjacent barrel. The employees left pinky finger was pinched between the lids of the two barrels. The finger flesh was removed from the nail to the tip.

An employee was standing on a belt conveyor that was locked and tagged out. Another employee removed the tag and lock and started the belt. The employee jumped from the belt and onto the catwalk. EE landed on hands and knees and sprained right wrist. Employee's first doctors visit was on 7/26/2016.

While the employee was tightening the locking nut on a crusher, a sliver of metal splintered off of the pin hammer that was being struck with a sledge hammer by the employee. The sliver lodged in the employee's left upper arm.
Employee was striking a pin on an excavator adaptor with a hammer when he re-injured his wrist. The injured wrist has been reported as an original injury dated 5/15/02. The employee has received 2 weeks of restricted duty as of 3/28/05, the date of the first evaluation for injury.
