EE struck a ceramic impact panel with a hammer in an effort to dislodge it when a piece of the ceramic went under EE's glasses and entered EE's left eye.

While lifting a roller associated with a conveyor the employee injured EE's back.

While loader operator was digging into muck pile to load haul trucks, the loader struck hard ground in the form of a high wall toe, jolting the loader and rocking the operator. The operator sustained a pulled muscle in his neck, shoulder, and right arm area.

EE WAS TRYING TO INSERT A CHAIN UNDER A SET OF METAL BARS THAT WERE BEING UNLOADED FROM A FREIGHT TRUCK. THE LIFTING MECHANISM SLIPPED AND THE LOAD PINCHED THE RIGHT INDEX FINGER OF THE EE MED ICAL TREATMENT INCLUDED 9 STITCHES AND 7 DAYS OFOF LIMITED LIFTING.
WHILE LOOSENING BOLTS EE STRAINED HIS GROIN AREAHIS DISCOMFORT WAS REPORTED IMMEDIATELY BUT HE DID NOT REQUIRE MEDICAL ATTENTION UNTIL 7/16/01. HE WAS LIMITED TO LIFTING 10LB FOR 1 WEEK.
WHEN EE TRIED TO RELEASE A JAMMED HOIST PULLEY, THE PULLEY FLIPPED BACKWARDS WHEN THE TENSION WAS RELEASED. THE EE S HAND WAS PINCHED BETWEEN THE PULLEY BLOCK AND THE HOIST FRAME. THE FRAME ED GE CUT THE EE THUMB, REQUIRING 6 STITCHES.
EE SWUNG A SLEDGE HAMMER AT A KNOCKER WRENCH WHILE TIGHTENING JAW BOLTS. THE HAMMER MISSED THE WRENCH HANDLE & HE PINCHED HIS HAND BETWEEN THE THUMB & FINGER. 4 SITCHES WERE REQUIRED.

EE TWISTED HIS RIGHT ANKLE GETTING DOWN FROM CRANE HOIST DRUM TO CAB FLOOR 3-4 FEET BELOW.
WHILE TRYING TO STRAIGHTEN AN UPSIDE-DOWN V-BELTON THE CRUSHER SHEAVE AND V-BELT ASSEMBLY, EMPLOYEE'S LOCKING PLIERS SLIPPED AND HIS FINGER WAS CAUGHT BETWEEN THE BELT AND THE SHEAVE, CUTTING HIS RIGHT LITTLE FINGER.
WHILE EE WAS TROUBLESHOOTING NOISE FROM CONVEYOR TROUGHING IDLER, HE CONTACTED THE CENTER IDLER CATCHING HIS HAND. EE S HAND AND ARM WERE PULLED OVER THE ROLLER AND UNDER THE BELT RELEASING HI S HAND ON THE OTHER SIDE OF THE SUPPORT BRACKET.THE EE RECEIVED 3 STITCHES FROM A CUT ON HIS ARM.

EE WAS LEAVING THE PIT WITH A LOAD OF ROCK AS HE PASSED THE AREA WHERE THE DROPBALL WAS OPERATING A ROCK FRAGMENT FLEW THRU THE OPEN WINDOW OF THE HAND TRUCK STRIKING THE OPERATOR IN THE FOREH EAD THE OPERATOR RECEIVED A LACERATION TO THE FOREHEAD WHICH REQUIRED SUTURES.
EMPLOYEE WAS SITTING ON TOP OF A CAGE GUARD. TIGHTENING THE CHEEK PLATE BOLTS, WHEN HE SLIPPED AND FELL. HE LANDED ON HIS FEET, CAUSING HIM TO SPRAIN HIS RIGHT KNEE.
