Employee was removing a tooth from an excavator bucket and was using a hammer to break it loose. A piece of metal struck EE on the chin causing a laceration injury.

The employee opened a gate for a contractor to leave. The contractors trailer became entangled with one side of the gate causing the trailer to jump sideways striking the employee who was holding the other side of the gate.
One employee was tossing down a 100' tape measure to another employee who lost it in the sun and was struck in the face. The employee fractured a small bone in the nose. No work time has been missed. The diagnoses of a need for medical treatment was made on 06/26/17. Repair procedure has not yet been scheduled.

While clearing a jam, the employee pinched his left little finger between the pipe he was using and the bottom of the chute.
While turning a valve the employee's left hand slipped and he mashed his thumb against a packing rod bolt

Employee was pushing against the sides of a roller-mounted conveyor to move the conveyor and he strained his upper thoracic area.
While the employee was cleaning the plant structure with a hot water cleaning system (steam genie), the wand of the system was broken and the hot water burned the employee on the left forearm.
Employee was driving haul truck when his truck hit a rut shifting his weight and straining his right shoulder on the cab's rightside handhold.

EE WAS STEPPING INTO A FEEDER BOX WHEN HE SLIPPED FROM THE EDGE OF THE FEEDER BOX SIDE PANEL. WHEN HIS 3-POINT CONTACT WAS LOST HE EXTENDED HIS RIGHT ARM FOR BALANCE AS HE STEPPED DOWN TO THE BOTTOM OF THE FEEDER 2' BELOW. HE STRAINED HIS RIGHT UPPER ARM.

EMPLOYEE WAS LOOSENING A TURNBUCKLE HOLDING A TRACTOR ON A LOWBOY TRAILER. THE EMPLOYEE ROLLED THE TURNBUCKLE AND THE HANDLE OF THE TURNBUCKLE ROLLED AND STRUCK HIM IN THE MOUTH. THE EMPLOYEE CHIPPED TWO FRONT TEETH REQUIRING DENTAL WORK TO BE DONE.
