Employee stated that ee was standing on ladder changing the hydraulic filters on a hauler. As ee was pulling on the ratchet, the ladder shifted sideways causing ee to strain back/hip area. Went to doctor on 3/22/17 to have checked. First day of lost work started on 3/23/17.

Employee was pulling hose from fuel truck to fuel dozer. Foot got stuck in the mud causing employee to lose balance. Employee fell backwards into the rear bumper on the fuel truck. Employee hit back against the bumper causing back pain and sprained left wrist when employee tried to break the fall.

Employee stated that EE used a torch to heat up a hand rail on dozer so as to bend it back into place. As EE was getting down, EE accidentally put hand on the hot metal hand rail causing burn to right hand. Got checked on 7/21/16 and had some skin debrided and bandaged. Was put on restricted duty on 7/26/16 and returned on regular duty on 8/2/16.

Employee stated that EE was operating a dozer creating drill bench area. The rock EE was working on caused the dozer to jar EE around during the shift. This caused pain to back, neck, and hip area. Continued working. To ER on 4/1 to check. Continued working till 4/11/16 when doctor took EE off work thus lost time starting 4/11.

Employee stated that EE was operating loader in coal pit. The jarring of the loader caused pain in the employee's lower back area. Finished that work shift. Started work the next day but ended up going to doctor that day to have checked. Injury is reportable due to missing the shift on 2/5/2016.

Employee stated that he was tightening the bolts on the fan belt pulley on a hauler. As he was pulling on the ratchet, he stated that he felt pain in his left side area. The pain eased up later but never went away and was worse on Tuesday morning. Missed work and went to doctor. Returned on 09/16/15 on restrictions.

Employee stated that he was operating a dozer. He stopped the dozer so as to check the hard bar oil. When he opened the panel door to check the oil level, wind blew a foreign body into his right eye. He could not get it flushed out. Went to ER to get removed. Given eye medication and missed one shift, thus reportable.

Employee stated that he was using a come along to repair the hand rail on a dozer. As he was pulling on the come along, it slipped causing him to strike the hand rail with his left elbow. Went to doctor on 8/11/15 to have checked due to swelling and pain. No lost time, some restrictions, and removed fluid from his elbow.

Employee stated that he handed another mechanic a step ladder to put into his truck. As the mechanic was putting the ladder into the truck, it hit the tailgate causing the end of the ladder to strike the injured employee's nose resulting in a laceration to the nose. Reportable due to requiring two sutures.

Employee stated that he was prying on rock with the dozer blade. The blade slipped off of the rock causing the dozer to jar the employee resulting in low back pain. He decided to go to doctor on 3/2/15 due to persistent low back pain and missed the 3/3/15 work shift. He is currently on restricted duty.

Employee stated that he was standing beside parts truck while another employee was dislodging a stuck piece of steel plate in the parts truck bed with a sledge hammer. When the employee was swinging the hammer against the steel plate, he missed and the hammer slipped out of his hands striking the injured employee in the mouth area causing a laceration requiring sutures.

He did not tell the company to file as a workers comp injury. Was off again in Feb. 2011 for same injury but called on 2/22/11 & said to turn in as comp injury & would go to comp doctor.

Employee stated that he was getting onto a dozer. He had one foot on the step of the push beam and the other foot on top of the push beam. His right foot slipped off of the step causing him to fall down and strike his right rib cage against the top of the push beam resulting in a fractured right rib.

Employee stated that he was operating a dozer pushing out shot rock/material. He backed the dozer up over a rock ledge. When the rear of the dozer sat down, it jostled the employee causing his left side strike the arm rest on the seat, resulting in a fractured left rib.

Employee stated he was getting onto dozer when left foot slipped off step. He had hold of grab iron which prevented fall but it strained right shoulder. Hurting but seemed ok. Went to doctor later. Found small tear in shoulder. Opted to keep working. Finally decided on minor surgery to repair tear. Did so and lost time starting on 11/17/08, thus reportable.

Employee stated that he was operating loader in coal pit. He noticed his right hand itching with red spot developing. Swelling became more pronounced at end of shift. To ER next day when right hand and arm was swollen. Probable spider bite but was not felt by employee.

Employee stated that he was using an impact wrench to tighten the bolts on the cutting edges which he had replaced on a D11R dozer. He said he felt a burning sensation in right shoulder area and some numbness in his right hand toward the end of the work day. He did not report this as a work related injury until 8-11-06 when his doctor said it was work related.

SYNOPSIS: TWO CONTACT MECHANICS NOTICED TWO INDIVIDUALS ON ATVS NEAR WHERE A BULK TRUCK WAS PARKED. UPON INVESTIGATION THEY FOUND THE TRUCK ON FIRE. IT HAD TO BE EXTINGUISHED BY THE FIRE DEPT. IT APPEARED THE TRUCK WAS VANDALIZED & THE FIRE INTENTIONALLY SET. THE FRONT OF THE TRUCK INCLUDING THE CAB HAD COMPLETELY BURNED.
