At 7:05am it was discovered that the wildfire originating off-property had spread onto the west and north boundary lines of the property. The fire did not result in any property damage or injuries to employees. Facility was able to reopen the following day.

At end of shift EE parked excavator in the pit. While completing post-shift inspection, the EE heard a noise and saw smoke from the engine compartment. EE tried to extinguish the fire with an extinguisher but was unsuccessful and evacuated the area. EMS was contacted. Fire continued beyond 30 minutes, at which MSHA Hotline was called. No injuries were associated with this incident.

On 12MAY23 EE slipped while using a pressure washer. EE grabbed a nearby support beam to prevent falling. EE reported incident on 15MAY23. Requested medical attention on 16MAY23. Diagnosed with right rotator cuff strain.

Employee was walking to MCC trailer when employee felt something fly into right eye. Employee went to the first aid cabinet and grabbed some eyewash and flushed the eye. Employee could feel what ever was in there move around the eye. Employee was wearing proper PPE (Safety glasses) and believes it was an insect that entered eye.

The employee was washing under the low washer stepped over a pipe and slipped on wet ground falling on left side.
The employee was working in the locked out clarifier when the employee slipped and struck their back on the rake. The employee initially thought they were okay. We recorded the fall. The next workday the employee said their back hurt and the employee went to the clinic and was told they had strained back.

EE was exsiting excavator and jumped off the last step twisting an ankle on a rock.
EE was cleaning material off of the conveyor. EE used a pipe wrench to turn the pulley. EE lost EE's balance and tripped and fell on the conveyor. EE cut EE's leg and was treated with 1st Aid. EE later complained of leg hurting and was taken to the clinic and left leg treated for a laceration. EE was given antibiotics and light duty.
EE was removing the cutting edge from a loader. The EE did not block out the edge and cut the bolts with a torch. The edge fell from an elevated position and struck the top of EE's foot.

Employee was stopped on side of dirt road going into pit waiting for the driver behind him to ask for instructions when the driver in the truck behind him rear-ended his truck causing him to hit his head on the back glass of the cab and also jar his shoulder and neck.
Opened hot radiator cap and water sot out and landed on his right arm.
Lost balance and fell from screening plant.
Getting out of loader and his left foot slipped on last step and he fell to the right and landed on his right hand hurting his wrist.
