Employee was changing out bottom conveyor rollers. Employee was placing the bad bottom roller into the work truck when EE's middle right finger was lacerated causing injury.

EE was assisting with changing the hoist ropes when their left index finger became pinched between the tugger rope and hoist drum. This resulted in a fracture with a laceration to the tip of the left index finger.

Employee was rolling up a spray hose mounted on the rear of the water truck. The employee over reeled the hose causing the hose nozzle to strike employee's lip causing a laceration.

EE was working on tower three changing out punch hole metal screens. The EE was making final positioning adjustments and smashed left thumb between the existing and new punch hole metal screen causing laceration.

Employee was removing pins from a quick-connect device on a 966 loader when the quick-connect device rolled from the cribbing onto the employee's left foot. The employee experienced fractures to the large toe and the second toe along with a laceration to the large toe. Employee was wearing safety toed boots.

Employee was driving an A40G haul truck with the seat belt on in a parked position when employee was rear-ended by a 45G haul truck traveling approximately 8MPH. Impact caused employee's body to shift forward and left shoulder hit the steering wheel resulting in a cervical strain.

EE was carrying a 45 lb roller when EE lost control of the object causing it to fall, crushing EE's right index finger between the roller and the concrete. The finger was not broken but a prescription was given for swelling.

EE was trimming palm trees when a palm frond sprung back and a thorn punctured EE's hand. First aid was performed but swelling occurred. Triage doctor referred EE to the clinic and antibiotics were prescribed. No further treatment was given.

Injured employee (IE) was in the process of disconnecting the cam lock for a grout hose. As the IE was uncoupling the hose, the remaining grout in the hose, made contact in the facial area. After the IE removed their safety glasses, IE felt irritation in EE's Left eye. IE's eye was flushed onsite and IE was taken to the hospital for evaluation.

EE was on the floating conveyor to move the conveyor chute car. A come-a-long was attached to the car to help move it to the correct place. The EE was brushing some rocks off of the track when the car moved and smashed EE's finger tip.

EE was driving the man-lift and hit a washout causing the man-lift basket to bounce. The EE struck the handrail with their chest/side.

Employee was cutting rubber while on knees and fell back while attempting to shift weight and accidentally cut themselves with the knife in the leg causing injury.

Crew was unclogging the crusher when one of the large rocks came out of the rigging and struck the handrail, breaking it. The crew installed barricade tape and continued working. The IW was working on the platform and backed through the barricade tape and fell to the ground. Coworkers asked if EE was okay, EE responded that EE was fine and went back to work.

Employee was cutting belt with knife and came in contact with hand causing injury.

EE was tightening the plug on the gear box when the wrench slipped causing EE's left forearm to get caught on a piece of metal catwalk material.

Tech was using a wire wheel, a bristle came loose and penetrated the skin in the thigh area on the left leg. Tech went to the hospital to have the piece removed.

Clamp failed holding counter weight to belt sled. Weight fell causing the belt to move while they were on it and employee was bounced back into the top belt.

Injured EE and another EE were replacing a return roller on the carry back conveyor when the roller shifted. The injured EE moved quickly to hold the roller and felt a pain in side/back.

While trouble shooting a mechanical issue, EE went into the collector ring area of the 191M shovel. EE did not follow company policy for Lock out/ Tag out. During the trouble shooting EE got too close or touched the collector ring and received an electrical shock to the head area.

Miner was operating a man lift. While extending the boom the right thumb was pinched between the hand rail and a I-Beam on the Stedman crusher.

dropped a Stedman crusher hammer on his right hand/finger while changing worn hammers.

employee complains of stiff and soreness of neck and back, 2 years of wear and tear.

stepped in a hole when climbing off a machine, causing right ankle to sprain.

Miner was replacing a guard, employee strained his lower right back lifting the guard.

INJURY OCCURRED REMOVING PIN FROM EXCAVATOR BOOM. MAN WAS HIT IN THE RIGHT SIDE OF HEAD WITH THE PIN DRIVER WHEN THE PIN CAME COMPLETELY OUT OF BORE HOLE. EMT WAS CALL AND HE WAS TAKEN TO BE CHECKED OUT.

Mike was exiting his work truck, when his right leg came in contact with the ground he felt a twist and had problems putting weight on his leg.

Plant @ #4 shaker. Employee was changing the top deck screen on the #4 shaker. Slipped with the screen and slid down the chute feet first.

EMPLOYEE WAS CLIMBING STAIRS CARRYING A PIECE OF METAL, WHEN HE DROP IT, HE WENT TO CATCH IT. HE SLIPPED AND FELL DOWN STAIRS. HE STRAINED HE RIGHT SHOULDER

Helping another employee remove a 1/2 sheet of plywood from under a belt. When he picked it up to pull it out he heard a pop and felt a pain in his left lower leg. Pulled a muscle or possible tear to MCL or Achilles rupture.

Removed shive from pump,placed on ground , shive toppeled over striking left foot of employee. Left foot had swelling, brusing and a small contusion. Employee was taken for x-rays. There were no broked bones. Employee was return to lite duty work.

Inflating small tire. Tire was wrapped by a ratchet strap. Went to release strap. The strap released with great force striking left hand. Small cuts to pointer and ring finger. Stitches were required. The miner returned to work that same day.

Annual Audiogram was performed and revealed a threshold shift of 13.3 in his left ear.*** Second test found no reportable hearing loss***

While changing v-belt on crusher feeder drive come-a-long slipped off anchor point and struck victim on right shoulder.

While walking on a catwalk employee stepped on a rock and twisted his left ankle. Employee was taken to hospital and diagnosed with a sprained ankle; also referred to a bone and joint specialist for further treatment. Employee is unable to work for three days or until specialist clears him.

Loader operator hit a pot hole in travel way to load a truck. The operator said that he missed filling in the pothole during his early morning Inspection.

Injured employee was working with 4 others to replace teeth on cutter head. Employee placed his hand on the tug push knee to look over side to check teeth for damage. Tug shifted and employee's left hand was caught between cutter head teeth and push knee. Employee attempted to pull hand back and left ring finger nail was pulled away and tip of finger broken inside glove.

The employe was struck below the right knee, causing an open tibia fracture, by a piece of metal banding that was released from under a front-end loader as the employee was spotting the front end loader operator.

Employee was tending shore line for crew tug and did not release line as crew tug started reversing. Hand was caught between rope and rail. Employee was wearing gloves.

Fell approx 6 ft while exiting bulldozer. Multiple contusions and abrasion to left leg. No expected permanent temp or partial disability.

Employee sufferred a spider bite to his left forearm while performing routine maintenance on a mini-excavator.

WHILE USING A PRYBAR TO DISLODGE A ROCK FROM A MOVING TAIL PULLEY, THE BELT GRABBED HOLD OF THE PRYBAR PULLING EMPLOYEE'S HAND INTO THE MOVING BELT, SEVERING THE TIP OF HIS FINGER.

Employee was using a hammer and a chisel to remove a steering pin from an off-road dump truck. The chisel splintered and the piece of metal pierced the employee's left thigh.

Employee was walking through the plant area when he stepped in a small depression and twisted his ankle.

EE CUT PALM ON A SHARP PIECE OF STEEL WHEN EE WAS DOING NIGHT TIME MAINTENANCE ON A SHAKER. EE DROPPED TOOL AND SUFFERED A LACERATION WHILE REACHING FOR IT.

EMPLOYEE WAS CHANGING A BEARING ON A SCREEN FEED TAIL PULLEY WHEN A SHARD OF METAL BROKE OFF OF WHAT HE WAS HAMMERING ON AND PUCTURED HIS LEG.

EMPLOYEE WAS CHANGING THE WEAR PLATE ON ONE OF THE CRUSHERS. WHILE TRYING TO REMOVE A ROCK WHICH WAS LODGED IN ONE OF THE BOLT HOLES, HE SMASHED THE TIP OF HIS FINGER BETWEEN TWO PIECES OF STEEL.

EMPLOYEE WAS WORKING ON PIPELINE FLANGES AND GOT HIS FINGERS CAUGHT BETWEEN THE TWO FLANGES CAUSING THEM TO BE CRUSHED.

Ee was welding and received a flash burn due to excessive exposure to ultraviolet light.

EE WAS ATTEMPTING TO FIT A DOOR TO THE BOOSTER WITH THE HELP OF A CRANE. TRYING TO GUIDE IT INTO PLACE, HIS FOOT GOT CAUGHT IN BETWEEN A PLATFORM AND THE DOOR.

DEVELOPED A HERNIA WHILE TRYING TO PRY OPEN & CLEAN OUT THE LID ON THE BOOSTER

EE WAS GREASING A GRADER (V-JOINT) IN A CONFINED SPACE USING A COMPRESSED-AIR GREASE GUN. EE WAS HIT IN THE EYE BY GREASE WHICH CAME THROUGH THE NIPPLE AND SHOT BACK ONTO HIM. HE WAS WEARING SAFETY GLASSES BUT THE GREASE STILL MANAGED TO GET IN HIS EYE.

EMPLOYEE WAS CHANGING SCREENS ON THE TOP DECK OF THE #4 SHAKER. HE WAS USING A PRY BAR TO ADJUST THE SCREENS WHEN IT SLIPPED SMASHING HIS FINGER BETWEEN THE PRY BAR AND THE SCREEN.

EE WAS REMOVING THE FRONT BORE OF A 30" PIPE WITH AN IMPACT GUN WHEN THE PIPE MOVED AROUND TOWARDS HIM CRUSHING HIS HAND AGAINST THE WALL.

EE WAS WASHING OFF AROUND TAILPULLEY OF A PLANT STACKER WHEN A ROCK BOUNCED OFF OF STACKER STRIKING HIS FOOT.

AFTER HANDLING OILY RAGS EE ATTEMPTED TO CLIMB A LADDER WHEN HE SLIPPED AND FELL HITTING HIS KNEE ON THE LOWER RAILING.

EE WAS FIXING LIGHTS ON BULLDOZER WHEN HE SLIPPED AND FELL LANDING ON THE LEFT MID-SECTION OF HIS BODY.

EE WAS TIGHTENING BOLTS ON WEARPLATES WHEN HIS FINGER WAS CAUGHT BETWEEN WEARPLATE AND PRIMARY DOOR CAUSING LACERATION TO HIS NAIL BED ON HIS LEFT THIRD DIGIT.

EE CRASHED THE A40D VOLVO DUMP TRUCK, THAT HE WAS DRIVER INTO THE BACK (TAILGATE) SECTION OF A A40D VOLVO DUMP TRUCK.

EMPLOYEE WAS DUMPING TRAY FULL OF SAMPLE ROCKS. WENT FROM THE HAND HEIGHT TO THE SHOULDER HEIGHT & ATTEMPTED TO DUMP ROCKS WHEN EMPLOYEE HEARD & FELT A POP IN HIS RT ELBOW.

EE STATES HE WAS WORKING ON BROKEN PRODUCT PUMP SLIPPED ON HYDRAULIC OIL( IT WAS ALSO RAINING AND IT WAS WET) AND HIT HIS RIGHT ELBOW ON THE BACK OF THE TRUCK.

EE WAS GUIDING A METAL PIPE CAP INTO A PLASTIC PIPE WHEN HIS HAND GOT CAUGHT BETWEEN THE METAL CAP AND THE PLASTIC PIPE.

EE WAS GRINDING A PIECE OF METAL WHEN THE GRINDER KICKED BACK & CUT THE MEDIAL LT HAND.

EE WAS WALKING UP A HILL CARRYING ROUGHLY 30 LBS OF TOOLS WHEN HE STEPPED ON UNEVEN PAVEMENT & FELT HIS RT KNEE POP ON BACK SIDE OF KNEE.

EE WAS MOVING ROCKS WEIGHING BETWEEN 60 - 100 LBS. THE ROCKS WERE BEING PUSHED OUT OF THE WAY, WHEN A ROCK ROLLED TO THE ROCK SIDE CATCHING EE'S FINGER BETWEEN THE ROCK & A STEEL WALL.

EE WAS GETTING BACK IN HIS CAB TO SIT DOWN WHEN THE WIND BLEW HARD & CLOSED ON HIS HAND.

EE WAS OPERATING A BULL DOZER. HE WAS PUSHING DIRT FORWARD WHEN THE BLADE HIT A ROCK & CAME TO A COMPLETE STOP, BUT THE SEAT BELT HAD PLAY IN IT. THE SEAT BELT DID NOT STOP HIM & HE FLEW FORWA RD & UP & HIT HIS HEAD (LACERATIONS) ON THE ROOF OF THE CAB.

EE WAS TURNING A STEEL ROTOR ON THE PORTABLE CRUSHER, WHEN HIS LEFT FOOT GOT WEDGED & CRUSHED BY THE ROTOR. THE ROTOR WAS ROUGHLY 2,000 LBS.

EE WAS UNSCREWING AN OIL FILTER. THE FILTERS WERE VERY TIGHT. WHEN EE INSCREWED THE FILTERS HE EXERTED HIMSELF & FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS OPERATING A JOHN DEERE 650. HE WAS MOVING BACKWARDS ON A SLOPE WHEN ALL OF A SUDDEN, HE HIT A BOULDER (ROCK) WHICH CAUSED MACHINE TO JUMP INTO THE AIR & LAND HARD, SENDING PAIN THROUGHO UT HIS LOWER BACK.

WHILE EXCHANGING BUCKET ON BACKHOE EE WAS WATCHING FOR RETAINING PIN TO BE PUSHED TOWARD HIM. ANOTHER EE WAS SPOTTER AND 3RD WAS OPERATING COMBINATION TO PUSH RETAINING PIN OUT. WHEN THE ORDER WAS GIVEN THE PIN WAS PUSHED BUT IT CAME OUT FASTER THAN NORMAL AND WAS ABOUT TO FALL WHEN INJURED TRIED TO SUPPORT AND HOLD THE PIN WHICH CRUSHED HIS FINGER ON THE BUCKET.

EMPLOYEE WAS STANDING ON GROUND GIVING INSTRUCTIONS TO ANOTHER OPERATION EMPLOYEE TURNED TO WALK AWAY & STUMBLED OVER SMALL BOULDER. TIRED TO CATCH HIMSELF WITH LEFT HAND & SPRAINED HIS WRIST.

OPERATOR WAS PUSHING DIRT WITH A BULLDOZER & SAID HE JARRED HIS BACK WHILE WORKING ON THE ROUGH TERRAIN

SEVERAL EMPLOYEES WERE SEPARATING VARIOUS SIZE ROCKS TO BE USED AS RIPRAP BY A CUSTOMER. HE MOVED ONE ROCK CAUSING ANOTHER TO FALL AND HIT HIS FINGER.

WELDER STRUCK ARC FLASHED EMP

MINE SUPERVISOR HAD COMPLAINED OF CHEST PAIN.INJURED WAS SITTING DOWN THEN STOOD UP AND PASSED OUT.HE REGAINED CONSCIOUSNESS ALMOST IMMEDIATELY.HE WAS TREATED BY PARAMEDICS AND TRANSPORTED TO HOSPITAL.WAS RELEASED 3 HRS LATER.

EMPLOYEE WAS OPERATING A FRONT END LOADER,HE CLAIMS ENGINE STALLED WHICH INCAPACITATED STEERING AND BRAKES WHICH CAUSED HIM TO DRIVE MACHINE INTO DRAINAGE DITCH IT APPEARS EMPLOYEE WAS TRAVELI NG AT AN EXCESSIVE SPEED WHEN INCIDENT OCCURRED.

EMPLOYEE WAS ADJUSTING ALIGNMENT OF CONVEYOR BELTS ON DRIVE ROLLER WHEN THE GLOVE ON HIS RIGHT HAND WAS CAUGHT ON THE CONVEYOR BELT. EMPLOYEE HAD SLIPPED CAUSING HIS HAND TO CONTACT THE MOVING CONVEYOR BELT. THE EMPLOYEE LACERATED HIS FINGER WHEN HE PULLED HIS HAND AWAY.

EMPLOYEE WAS CHANGING MATERIAL SCEEN WHEN SCREEN STOPPED AND JERKED EMPLOYEE CAUSING BACK STRAIN.

THE EMPLOYEE WAS OPERATING A FRONT END LOADER AND WAS TRAVELING DOWN THE RAMP INTO THE PIT AREA WHEN THE FRONT END LOADER BUCKET DROPPED AND DUG INTO THE ROADWAY INSTANTLY STOPPING THE MACHINE .THE EMPLOYEE SUFFERED BLUNT TRAUMA TO THE UPPERTORSO.OUR INVESTIGATION REVEALED A SPRING IN THE MAIN HYDRAULIC CONTOL VALVE BROKE,CAUSING THE BUCKET TO DROP.
