An inintentional roof fall was discovered 2300 ft outby the face of 004 MMU at spad #2091. The fall measured approximately 20 ft wide x 20 ft long x 4 1/2 - 5 ft high.
An unintentional roof fall was discovered at 10:00am on 9/14/06, during a weekly examination. The roof fall material consisted of laminated slate and sand stone. The fall measured approx 20' wide X 16' long X 4 1/2 - 5" high.
An unintentional roof fall occurred 6200' outby the face at Break #99, #7 Entry, 1 Break inby Spad #1349. The fall measured approx 12' long X 20' wide X 4 1/2' high.
An unintentional roof fall was discovered on 8/31/06 during a weekly examination. The incident location was at the #6 entry approximately 6,500 ft outby the face at Break #95, Spad #1331. The fall measured approximately 120 ft long x 20 ft wide x 4 1/2 ft high.

Employee was using a chain ratchet to pull a belt together when the clamp broke and struck the victim on the left side of his face. The incident resulted in a fractured left mandible.
EE strained his right knee while crossing the section tail piece.
The employee was bolting the #7 break when his drill steel became lodged in a hole, then came loose unexpectedly, striking the ring finger of his left hand. The incident resulted in a lacertaion that required stitches.
DUE TO AN UNDETECTED POCKET OF GAS (MEASURING APPROX. 4' WIDE BY 6' LONG) 2 EMPLOYEES WITNESSED AN IGNITION OF METHANE GAS ON THE LEFT SIDE OF THE MINER. HOWEVER, NO PHYSICAL EVIDENCE OF AN IGNITION WAS FOUND.
The injured employee was operating a four-wheeler which struck a Mac 8 that was stopped in front of him. This caused the steering wheel to spin around and strike the top of the employee's right hand, resulting in a fracture.

The injured employee was crawling toward the roof bolt machine when his left foot slipped off the coal and became caught underneath the wheel of the machine.
THE EE WAS TRAVELING IN A MANTRIP WHEN IT DIPPED DOWN. THIS MOTION CAUSED THE EE'S KNEE TO STRIKE THE ROOF, RESULTING IN A LACERATION THAT REQUIRED STITCHES.
The employee twisted his knee exiting the car ram.
EE WAS BOLTING THE HEADING WHEN A ROCK FELL OUT OF A SLICK AREA, SLID DOWN BETWEEN THE CANOPY AND DRILL POT, HITTING LEFT LEG AND KNEE

A ROCK FELL FROM BETWEEN THE RIB AND BOLT AND ONTO THE EMPLOYEE'S FOOT AND ANKLE.
INJURED EE TOLD THE WITNESS THAT HE WAS GOING TO MOVE THE CABLE CROSSER. HE DID NOT HEAR HIM AND ACCIDENTALLY RAN OVER HIS RIGHT HAND WITH A ROOF BOLTER WHEEL.
A rock fall occurred at 1:30 p.m. in the #2 heading 10' inby spad #1366. there were no injuries. the travelways and airways were not affected. the fall measured approx. 5 ft high, 18 ft wide, and 65 ft long.
ADVERSE ROOF. * EMPLOYEE HIT WITH FALLING ROCK WHILE ROOF BOLTING *
DURING THE 1ST SHIFT ON 6/17/04, THE ROOF STARTED WORKING AT SPAD #1297 IN THE #3 HEADING. EMPLOYEES WERE EVACUATED FROM THE AFFECTED AREA AND THE AREA WAS DANGERED OFF. THERE WERE NO INJURIES AND NO EQUIPMENT WAS INVOLVED. THE FALL WAS 8' HIGH, 30' LONG, AND 30' WIDE AT THE INTERSECTION. THE FALL HAS BEEN CLEANED UP AND BOLTED.

A HIGH VOLTAGE FUSE WAS BLOWN & WHILE ATTEMPTING TO CHANGE THE HIGH VOLTAGE FUSE EMPLOYEE CAME IN CONTACT WITH ENERGIZED CONDUCTOR, BURNING HIS LEFT HAND & RIGHT FOOT.
TIGHTENING BOLT ON REAR END OF SCOOP, WRENCH SLIPPED AND HE FELL BACKWARDS. EE STRUCK OIL CAN & GROUND WITH HIS BACK.
