Miner was moving a wheelbarrow and hurt EE's back. Medical exam called it a lumbar strain.

Recordable Standard Threshhold Shift of Left ear. Re-testing on 7/14/25 at 8:30am. ***Retest shows as non-chargeable***

EE was helping clear the feeders by poking the portholes. The fine dust and air coming from the portholes got in EE's eyes.

Employee was blowing product off a Pnuematic truck when line plugged up. In the process of investigating the plugged line, employee got LKD in their eyes. We immediately took the employee to the ER where they flushed EE's eyes.

Injured worker was installing a variable frequency drive. Injured worker attempted to place a bar underneath and then fell and struck their knees on the concrete. Injured worker continued to work after incident without restrictions. After physical therapy injured worker had surgery on September 6, 2024, to repair left meniscus tear and has now missed work.

Employee was retrieving a hanger bearing that EE cut out of a screw and that had fallen on a pile of rock. As employee was climbing, EE slipped and felt a discomfort. EE subsequently felt a bulge in groin area. When EE returned to work on 4/17/23, employee was sent to the doctor.

Employee was cleaning out a transfer chute with a chipping hammer. Employee twisted and felt a back pain.

Employee was traveling down stairs at Screen 02-SN-01. Employee misplaced right foot on a step and twisted right knee.

Employee was working on the south silos cleaning out a blow line. Employee over-heated and vomited a couple of times. Vitals were checked by an ambulatory EMT and employee was monitored closely by other individuals at the plant for the rest of EE's shift.

Employee was using a pocket knife to cut a zip tie. When the employee cut through the zip tie, employee cut into left forearm.

Employee was unplugging the LKD blow line for kiln #5. When employee dumped LKD on the ground, employee was standing in the dump pile and the hot lime was burning EE's foot.

Employee was in a squatting position and stood up to a straight position and felt a pain in their left foot at that time.

Employee was on a ladder and lost EE's balance, falling an estimate of 4 feet to the ground.

Miner was exiting desk chair and tripped over lunchbox below the desk causing miner to fall to the floor. Miner hit left knee first, then left arm, then head on the wall. Miner refused medical attention at the time of incident(12/18/2020). Miner later determined on 04/07/2021 that medical care was needed. Miner's first date of lost time was 04/09/2021.

The miner was bitten by some type of nonvenomous insect on the right arm while operating a front end loader.

Bottom rung of ladder broke causing employee to hit EE's left knee on the rung above the rung that broke. Employee did not receive restrictions or miss work at the time of injury, therefore this was not a reportable or lost work time injury. On 4/12/2022, employee was placed off work due to restrictions received from EE's doctor.

Employee was replacing bolts and wrench slipped off causing employee's left thumb to contact the hammer base. DOI was 09/03/20, did not seek treatment on DOI. Emp was scheduled for vacation on 09/04 thru 09/13. Upon empl return of 09/14/20 from vacation, emp informed the employer that treatment was sought on 09/12/20 and the care facility confirmed a fractured left thumb.

Employee was in the act of filling an automatic greaser on a Cat 374F excavator and miss stepped on the catwalk of the machine. When they stepped back they lost balance and fell from the cat walk to the ground injuring right leg.

The miner was mixing up mortar in a bucket. When the mixing was complete the miner lifted the bucket and felt a muscle pull in lower back.

Employee was replacing a motor mount. While trying to align the motor to the new motor base, the employee got a finger hung in a bolt hole in the motor base cutting.

12/11/17 received dr's care due to non-work related injury that occurred 12/9/17 causing limited movement/muscle spasms in lower back after bathing dog. Under dr's care, put on leave by Dr w/restrictions of no lifting until RTW on 12/15/17. Dr. said could cause flare-ups periodically. 7/6/18 aggravated pre-existing condition, off work til further notice

Cleaning an end off a wire, knife slipped & cut finger.

Employee was cleaning the lower part of 8-WS-6 around the water drop area with an air chisel when ee bared fitting became unscrewed from the air chisel striking the employee's right ear with compressed air entering the ear canal.

EE slipped in the parking lot on 2/27/2018 spraining ankle and was released with restrictions but able to do job at full capacity. After a Dr. follow-up visit on 3/28/2018 and more x-rays it was discovered that EE had a slight fracture. We are filing this accident as MSHA reportable at this time after the x-ray diagnosis showed the slight fracture.

Miner attempted to move forks of a fork lift. Miners gloved hand slipped off the fork and little finger of left hand was caught between heel of fork and beam below bar where fork hangs from.

Miner walking from one location to another, uneven ground with loose rock. Miner slipped on loose rock causing miner to fall to the ground and land on right shoulder. Miner sought medical care on 11/18/16 and was released without restrictions. It was later determined that miner needed further medical care with restrictions beginning on 01/24/2017. Return to work full duty 01/30/17.

Miner cleaning and door slammed on EE's right middle finger.

Accident happened in equipment yard when jumper cables slipped from miner's hand making contact with miner's face area creating a gash near lip.

Employee was removing equipment extension to facilitate welding repairs when he propped his left leg against truck and felt pain in hamstring area.

Miner placed wheel chocks behind rear wheels. Miner then picked up 5 gallon bucket and started to walk away when the rope attached to wheel chocks was tangled around foot causing miner to lose balance and fall on right shoulder.

Employee had irritation of the eye on 04/25/2015 and sought medical treatment on 04/25/2015 leading to a foreign metal sliver removed from surface of eye. Employee is unaware of when or how the injury occurred.

The employee was climbing down steps, placed foot into a puddle of water and slipped which caused his left knee to hurt. Employee reported to immediate supervisor, however medical attention was not warranted per employee's request. At a later date of 04/22/2015, employee requested medical attention due to pain in his left knee.

Employee removed glasses to clean hands and face with vinegar. While washing his hands, vinegar splashed into his right eye.

Miners foot was pinched between equipment causing the miner to fall and hit chair bracket with his elbow creating toe to become swollen and soreness of the elbow.

Miner was climbing into the cab of the truck and hit his right knee under the second rung of the mounted truck ladder. Reported to immediate supervisor, however medical attention was not warranted per Miner request. Miner went to the doctor on his own behalf on 09/29/2014,the employer became aware on 10/07/2014 when notified by Miner that medical attention had been sought.

The miner was cleaning out a chute with a pry bar when the bar broke through and he caught the bar pulling muscle in lower back

The miner stood up after squatting to look at a pipe and felt pain in right knee.

The miner was exiting a truck when he stepped on a small rock and sprained his right ankle.

The miner was rolling up his welding leads when lime got in his right eye irritating it.

The miner was raking clods on kiln floor he slipped and twisted his right knee.

Employee was replacing buckets on a material elevator, lime contacted his skin around at various locations including his eyes. Employee received 1st and 2nd degree burns. Employee missed 2 scheduled shifts while under physicians care to prevent infection.

Employee was clearing a blockage in transfer chute of tunnel screw that was carrying hydrate into a collecting screw. Upon clearing the blockage, hydrate was forced from the access port onto exposed body parts causing lime burns to the head and facial area.

Customer arranged truck driver was latching dome lid on a bulk tanker trailer during the loading procedure and fell from the ladder accessing the trailer, fracturing a bone in his foot. Contacted the carrier and explained the reporting requirements. This report is to ensure that MSHA is notified of the incident. Social Security and Date of Birth of the individual is unknown

The miner was walking to the kiln area when he slipped and fell on some icy pavement hurting his left elbow and shoulder

Employee dropped a steel I-beam on right foot breaking bone in great toe. Employee was placed on restricted activity. At follow up evaluation on 2/24, restrictions were extended and the company can no longer accommodate the restrictions placed upon the employee by the treating physician. Lost time as of 2/24/14.

Details surrounding the incident are uncertain. Contract driver claims to have fallen approx 15' off metal stairs at loading area. Driver left the premises and delivered at destination approx 250 miles from plant. After unloading, driver reported to the trucking company next day. Texas Lime was made aware that this incident led to MSHA reportable criteria on 4/1/14. DOB Unknown

Employee was helping another employee washout a hydrate tank with a water hose when some hydrate lime got in his right eye. He did not think that he needed to go to the doctor until 9-14-13.

While fabricating & installing a chute, employee bumped his knee on the adjustment bolt. He did not feel this to be a significant incident at the time. However, in the evening of 8-13-13 he sought medical treatment at an urgent care center and was diagnosed with cellulitis.

On 5-29-13 EE returned a Disability form to the Co in which his Dr stated that his hand condition was work related. Co had no reason to suspect his condition was work related until the Dr's form was returned. He has work restrictions that the Co cannot accommodate and is missing work. Date of inj unknown. Investigation is ongoing as the Co's W/C carrier has denied this claim.

A bag of hydrated lime was traveling up the conveyor sideways. The employee reached to turn the bag when it struck the bag flattener and blew up causing lime to go in his eyes and throat.

Employee was doing a coal yard inspection. As he was walking back to the Loader, he twisted his right ankle.

Employee contacted Personal/Payroll Admin on 2/1/12 requesting information concerning short term disability due to his back hurting. The employee stated that the injury was not work related at the time. The work restrictions placed on him by the treating physician of his choice, we are unable to meet. We were informed on 2/17/12 that a workers comp claim had been filed.

EE was lighting the natural gas pilot with a 16 ft extendable aluminum pole from outside the man-door to the kiln, which was common practice. Injured employee was standing behind him when the pilot was lit. The ignition of the pilot & subsequent backdraft was larger that normal. EE turned away from the kiln and contacted the other employee, knocking them both down.

Employee was pulling the Guzzler hose out of the Cooler Pit and up the stairs. He has restrictions that prevent him from working and also has Physical Therapy for the next 2 weeks.

Employee was cleaning up in the Hydrate Bag area when a bag of lime broke and caused some lime to get in his right eye.This became a lost time on 4/28/11 with restrictions that prevented him from doing his job. He got a full release on 4/29 and worked thru 5/2 until the Dr put the restrictions back. He is again lost time as of 5/3/11

Employee stated that he was climbing up on the Dozer from the track to the cab when he slipped and hit his right hip on a light cover. He did not report this specific incident until 4-28-2011 when he asked to see a doctor. He has restrictions that prevent him from doing his job. Lost Time began on 4-29-11.

Employee was breaking clods with a sledge hammer when he injured his hand. He also suffered a lime burn to his wrist causing cellulitis

The Employee was hand bagging on 9-B-3 when a bag came off the belt and broke. Lime went into his eyes.

The employee was picking up a hose which was in water and slipped on something. Rain had flooded the area the night before.

Employee was walking through the Hydrate Baghouse at the end of his shift and was stung by a bee near his right eye.

Employee was trying to unplug the bottom of an elevator and a pile of product built up around him. When he tried to exit the area, he decided to jump over the pile and came down wrong on his foot.

Employee was moving rock knockers to be placed on a forklift. One of them slipped and mashed & cut his left index finger requiring stitches.

The employee was helping adjust the clutch on the Guzzler with a pair of channel locks when it slipped and a sharp piece of metal cut his finger and nail. Fingernail had to be removed.

Employee was attempting to remove buildup in preheater. He was using a piece of pipe about 3 ft and smashed his finger in the process.

The employee was straightening a hydrate bag that had jammed when it opened and blew lime into the employee's right eye. Has received medicated eye drops and missed one day of work.

Employee was servicing the 992C Loader. While dismounting the loader, his left foot slipped and went between the step and the machine.

Employee went into the tunnel to clear clods off of grates. When he stuck his hand in, he cut it on a piece of metal. He received stitches on his thumb and index finger.

Employee was cleaning out the preheater using a pry bar and complained the following day of testicular pain. He has restrictions that prevent him from working.

The employee was moving a roll of shrink wrap from a pallet to the forklift. He has had soreness but did not ask to see a doctor until 9/22/09. Did not become Lost Time until 9/23/09.

The employee was going up to the #2 Dryer Tower to check the screen samples. He was at ground level when he said he slipped and fell backwards onto his back.

The employee was putting pins in the #6 Preheater. While knocking the pins in he lost his balance and slipped. His foot went between the top rung of the ladder and the catwalk.

The employee woke up sore on 5-1-09. He had been pulling brush from the front of the Dozer. Asked to see a DR on 5-4-09. He does not know when injury happened. He does not have any restrictions that prevent him from doing his regular job, however he has multiple visits to a Chiropractor scheduled.

The belt conveyor that transports quicklime to the pebble silos caught on fire. It may have been caused by the lime coming from the cooler feeders was too hot. The SLAM process was applied, the fire could've been put out faster, due to the location of the belt it was decided to not put our miners in harm's way. The fire dept. put the fire out in less than 10 minutes.

The employee was adjusting a bag of hydrate in the palletizer and pinned his right ankle against a beam and cut his leg requiring 7 stitches. NO LOST TIME, however he had work restrictions for 2 workdays.

The employee was breaking up a dry ball in the hydrators when it came off the shaft with such force that the area was filled with hot lime. He did not think he needed to see a doctor at that time but later went to the Emergency Room on Sat. 9/13/08. He was diagnosed with 1st & 2nd degree burns. NO LOST TIME, however he does have work restrictions.

Employee was pushing the waste lime pit. When employee got to service area and exited the loader, he slipped on the lime that had built up on the bottom step. His right foot hit the ground jarring his lower back. NO LOST TIME but he does have work restrictions.

While using the power washer EE noticed that the water tank was running over. He pulled the unit out from the wall and attempted to bend the float shaft down. His hand slipped cutting open the top of his left hand on the stainless steel tank. NO LOST TIME however he did receive stitches and medical treatment.

Employee was rebuilding screen drive. He was removing the bearing and housing from the drive when it popped off striking his left wrist. NO LOST TIME, however he does have some work restrictions.

The employee was using a hand drill. The drill caught and twisted around, caught hand and twisted wrist. NO LOST TIME, however he does have a splint and some work restrictions.

The employee was standing on the south side of the discharge on 6-SC-9. The bank gave way and the employee slid into the kiln dust fines and water. He has lime burns on his legs. He has not missed any time - however he is on restricted duty.

The employee was sweeping by the bagging machine, when a bag of lime blew up and hit him on the side of the head, causing lime to go around his safety glasses into his right eye.

Employee was cleaning windows on 992 Quarry Loader. He was moving from the windshield area to the right side of the cab, when he lost his footing, grabbed the right handrail and slid to the ground. No LOST TIME yet, however he has work restrictions.

The employee was cleaning with a shovel and had walked out of work area behind the bobcat. He was bumped by the back door on the bobcat. He has not lost any time but has some work restrictions and will receive some chiropractic treatment.

Quarry Mgr, stepped up on a 3 ft concrete support to inspect 1-BC-1 belt. He lost his footing, slipped and fell to the ground on his left side. He had surgery on 9-27-06 to repair left femur.

Employee was cleaning out cooler-feeder with an air lance. Lime broke loose and blew in his face.

Employee was changing clutch on auxiliary engine of Guzzler Truck when he caught his knee on a sharp edge of the toolbox. No Lost Time or restricted work duty - Medical Only due to receiving stitches.

While opening the hood on the Cat Haul Truck, the employee pulled or twisted his back.

When the bag fell off the Bagger it had too much air in it and hydrate lime spewed out of the bag when it hit the conveyor. Lime went under the employee's safety glasses into his left eye. Employee went to the E.R. on Saturday afternoon.

Employee was at the back of the preheater taking measurements. Lime dust blew into his right eye. Did not see Dr until 8/16/2005. NO LOST TIME, HOWEVER HE GOT PRESCRIPTION EYE MEDICATION.

Employee was cleaning out transfer chute of #5 kiln when he was struck by a 10' piece of conduit he was using. NO LOST TIME OR WORK RESTRICTIONS, however he received 5 stitches at the hospital emergency room.

Employee was cleaning with shovel & bobcat and thinks he got bit by a spider. He asked to see a Dr. on 6/24/2005 when he reported the incident. NO LOST TIME, NO WORK RESTRICTIONS AND NO MEDICAL TREATMENT OTHER THAN A RX FOR ANTIBIOTICS.

Employee was adjusting kiln trunnion, missed the knocker wrench and hit himself on the leg. No Lost Time, however he has some work restrictions and is receiving physical therapy.

EE had been driving an air-conditioned haul truck approx. 30 min. He said he felt hot & went to the Dryer Control Room where he passed out. He was taken by ambulance to ER where he was treated for dehydration & released to return to work. 2 days later after work he again went to an ER for dehydration and was told to take 2 days off work.

Employee finished working on screen 1-SN-2. As he was leaving, he stepped on a large rock & twisted his ankle. He has not lost any time but has work restrictions and is going to physical therapy.

Employee was welding on return rollers and cutting with a torch, he sustained flash burns to eyes.

Employee was cleaning fines out of a screen hopper with a pipe. Some fines came down and the pipe struck him on the side of the head. Employee continued working until 11-10-04 when he requested to see a Dr. An MRI was performed on 11-11-04 to rule out a concussion. The MRI was considered normal and the employee was released to return to work full duty on 11-12-04.

Employee was cleaning out discharge chute of 3-BC-1 when the head pulley caught the metal bar, slinging it into the right jaw of the employee.

Employee was cleaning out the hydrate tunnels when a pile of lime fell down and flew up into his face.. He had lime burns to both eyes and corneal abrasion of the right eye. He received antobiotic drops and missed one full shift of work.

Employee said his back hurt after stacking 50# bags in the Hydrate Bagging Dept. Asked to see a doctor about 9:00 AM.

Employee was cleaning in #5 Cooler Pit. When he stood up he twisted his left knee. NO LOST TIME - however employee has not been able to perform his normal job duties since 11/24/03 due to restrictions.

EE WAS USING A CHAIN STEP TO GET INTO A TRUCK AND HURT HIS LEFT FOOT. NO LOST TIME, RESRICTED WORK DUTY ONLY.

EE IS A HYDRATE BAGGER/STACKER. HE ASKED TO SEE A DOCTOR ON 10-30-03 SAYING THAT HIS SHOULDER HURT. NO SPECIFIC INCIDENT HAD OCCURRED. HE TOLD THE DR. INJURY HAPPENED ON 10-16-03, HOWEVER NOTH ING WAS REPORTED UNTIL 10-30-03. NO LOST TIME-PHYSICAL THERAPY AND RESTRICTED WORK DUTIES ONLY.

EE THOUGHT THE BELT HE WAS WORKING ON WAS 1-BC-3 & LOCKED IT OUT. THE BELT WAS ACTUALLY 3-BC-1. HE WAS IN THE PROCESS OF PUTTING A CLAMP ON THE SIDE OF THE BELT. THE BELT INADVERTENTLY STARTED PULLING HIS ARM INTO THE IDLER ROLLER & ALSO WEDGED HIS LEG BETWEEN THE HANDRAIL & THE BELT. NO LOST TIME, RESTRICTED WORK DUTY & PHYSICAL THERAPY ONLY.

EE WAS REMOVING CAP FROM ROTARY VALVE ON BLOWER PIPE WHEN IT BLEW OFF & HIT HIM IN THE FINGER.

WAS CLEANING OUT EQUIPMENT AT THE HYDRATORS. HE SLIPPED AND FELL.

EMPLOYEE WAS CUTTING A PIECE OF EQUIPMENT TO SIZE. THE PART BEING CUT OFF WAS NOT SUPPORTED AND WAS ALLOWED TO FALL. WHEN IT FELL IT BOUNCED AND HIT HIM IN THE CHEST. THERE IS NO LOST TIME OR RESTRICTED WORK DUTIES. HOWEVER, HE IS TO RECEIVE PHYSICAL THERAPY FOR 2 WEEKS.

THE EE WAS DUMPING COAL MILL REJECTS WITH A WHEELBARROW. THERE WAS ICE ON THE GROUND AND HE LOSTHIS BALANCE AND SLIPPED AND FELL.

EE REPORTED BACK PAIN AFTER OPERATING LOADER. NO SPECIFIC INCIDENT KNOWN.

EE WAS USING A SHOVEL TO CLEAN UP MATERIAL IN THE SCREEN HOUSE. INJURY WAS NOT REPORTED UNTIL 6-17-02. NO LOST TIME. HOWEVER, HE HAS SOME WORK RESTRICTIONS AND WILL BE RECEIVING PHYSICAL THERA PY.

EE WAS CLEANING OUT HYDRATORS WHEN HE LOST HIS FOOTING, SLIPPED & FELL. NO LOST TIME, HOWEVER HE HAS SOME WORK RESTRICTIONS & WILL BE RECEIVING PHYSICAL THERAPY.

P-LANT TRUCK HAD RUN OUT OF GAS. EE WAS PRIMING CARBURETOR WHEN IT BACKFIRED AND BURNED EE.

EE WAS RAKING CLODS ON THE KILN FLOOR AND STRAINED LOWER BACK. EE IS RECEIVING PHYSICAL THERAPY AND HAS SOME RESTRICTED WORK ACTIVITY- NO LOST TIME.

EMPLOYEE WAS CLEANING SCRUBBERS. HE WAS USING A HAMMER AND HIT HIS ELBOW ON A BEAM. INCIDENT WAS NOT REPORTED UNTIL 11/12/01 AND EMPLOYEE DID NOT AST TO SEE A DOCTOR UNTIL 11/21/01. NO LOST TI ME. MEDICAL ONLY AS HE IS TO GO TO PHYSICAL THERAPY FOR 4 WEEKS.

LOADER OPERATOR DROVE UNDER PLATFORM WHERE EMPLOYEE WAS WORKING TO REMOVE WASTE MATERIAL. AS MATERIAL WAS BEING REMOVED STEAM AND QUIKLIME ROSE UP CAUSING LIME BURNS TO EMPLOYEE. NO LOST TIME. RESTRICTED WORK DAYS ONLY.

EMPLOYEE WAS CUTTING A BELT. WHEN HE MOVED BACK TO STAND UP. HE SLIPPED ON A ROCK AND CUT HIMSELF REQUIRING 5 STITCHES. MEDICAL TREATMENT ONLY. NO LOST TIME.

EMPLOYEE WAS POKING/CLEANING SCRUBBERS AND SLIPPED ON LADDER RUNG HITTING LEFT KNEE.

EMPLOYEE WAS SERVICING #950 LOADER. EXITED CAB DOOR FAILED TO LATCH, EMPLOYEE CAUGHT HOLD OF HANDLE, DOOR CAME OPEN AND EMPLOYEE LOST BALANCE AND FELL TO GROUND.

EE WAS STACKING BAGS. A BAG ROLLED OFF THE BELT AND BROKE. LIME SHOT IN IS RIGHT EYE AND KNOCKED OFF HIS GLASSES AND HARD HAT.

EE WAS LOADING AN OXYGEN BOTTLE WHEN THE CAP CAME OFF AND HIT HIM ON THE CHIN. THIS INJURY REQUIRED MEDICAL ATTENTION WHICH INCLUDED THREE STITCHES. NO LOST TIME OR WORK RESTRICTIONS.

THE SCREW SHAFT HAD BROKEN & EE WAS REMOVING LIDTO CLEAN OUT FOR REPAIRS. HE HIT HIS ELBOW ON THE LID. HE DID NOT ASK TO SEE A DOCTOR UNTIL 4/24& WAS SEEN AGAIN ON 5/1 AT WHICH TIME THE DOCTOR RECOMMENDED PHYSICAL THERAPY FOR 4 WEEKS. NO LOST TIME - MEDICAL TREATMENT ONLY.

EMPLOYEE WAS REBUILDING A SET OF STAIRS. HE WAS TRYING TO LINE UP THE CHANNEL IRON BY HAND WHEN HE INJURED THE LEFT RIB AREA OF HIS BODY. THE INJURY WAS REPORTED ON 3-14-01 AND HE SAW A DOCTOR ON 3-15-01. THE COMPANY DEEMED THIS AS FIRST AIDCASE BECAUSE THERE WAS NO MEDICAL TREATMENT, OTHER THAN DIAGNOSTIC, NO LOST TIME AND NO WORK RESTRICTION.

HYDRAULIC LINE FAILURE RESULTING IN IGNITION CAUSING FIRE TO ENGINE THE CAB OF VEHCILE. DRIVER SUFFERED 2ND DEGREE BURN ON BOTH ARMS.

EE WAS MAKING A NEW SHAFT FOR TAIL PULLEY ON 2 BC-10. EE WAS GOING TO CUT A KEY WAY IN SHAFT. EE WAS REMOVING THE KEY WAY CUTTER FROM BOX AND FELT MUSCLE PULL WHEN LIFTING. EE IS RECEIVING PHYSICAL THERAPY AND IS ON RESTRICTED WORK DUTY

THE INJURED EMPLOYEE WAS SUPERVISING TWO OTHER MAINTENANCE EMPLOYEES WHO WERE CHIPPING AND PRYING MATERIAL AWAY FROM THE TRANSFER CHUTE AT THE FEED END OF #5 KILN. WHEN HE WENT TO INSPECT THE WORK MORE CLOSELY, THE SCAFFOLDING PROVIDED FAILED AND THE INJURED EMPLOYEE FELL APPROX. 5 FEET ONTO A PILE OF SPILLED KILNSTONE.

EE WAS ATTEMPTING TO PUSH THE QUENCHER BOWL OUT TO BEGIN CLEAN OUT OF SCRUBBER. HE WAS STANDING ON A SUPPORT LEG WHEN HE FOOT SLIPPED OFF FELL INTO THE SCRUBBER WATER. HE RECEIVED ONLY FIRST A ID TREATMENT, BUT IS ON RESTRICTED DUTY DUE TO THE FACT THAT HE CANNOT WEAR A REGULAR SHOE.
