At time of filing it is believed a front-end loader struck the back of the tractor trailer. The position of the victim at the time of the accident is still under investigation. The victim was found under the rear driver's side tires of the tractor.

Employee was blowing product off a Pnuematic truck when line plugged up. In the process of investigating the plugged line, employee got LKD in their eyes. We immediately took the employee to the ER where they flushed EE's eyes.

Injured worker was installing a variable frequency drive. Injured worker attempted to place a bar underneath and then fell and struck their knees on the concrete. Injured worker continued to work after incident without restrictions. After physical therapy injured worker had surgery on September 6, 2024, to repair left meniscus tear and has now missed work.

Employee was retrieving a hanger bearing that EE cut out of a screw and that had fallen on a pile of rock. As employee was climbing, EE slipped and felt a discomfort. EE subsequently felt a bulge in groin area. When EE returned to work on 4/17/23, employee was sent to the doctor.

Employee was working on the south silos cleaning out a blow line. Employee over-heated and vomited a couple of times. Vitals were checked by an ambulatory EMT and employee was monitored closely by other individuals at the plant for the rest of EE's shift.

Miner was exiting desk chair and tripped over lunchbox below the desk causing miner to fall to the floor. Miner hit left knee first, then left arm, then head on the wall. Miner refused medical attention at the time of incident(12/18/2020). Miner later determined on 04/07/2021 that medical care was needed. Miner's first date of lost time was 04/09/2021.

Bottom rung of ladder broke causing employee to hit EE's left knee on the rung above the rung that broke. Employee did not receive restrictions or miss work at the time of injury, therefore this was not a reportable or lost work time injury. On 4/12/2022, employee was placed off work due to restrictions received from EE's doctor.

Employee was replacing bolts and wrench slipped off causing employee's left thumb to contact the hammer base. DOI was 09/03/20, did not seek treatment on DOI. Emp was scheduled for vacation on 09/04 thru 09/13. Upon empl return of 09/14/20 from vacation, emp informed the employer that treatment was sought on 09/12/20 and the care facility confirmed a fractured left thumb.

Employee was in the act of filling an automatic greaser on a Cat 374F excavator and miss stepped on the catwalk of the machine. When they stepped back they lost balance and fell from the cat walk to the ground injuring right leg.

Dr. said could cause flare-ups periodically. 7/6/18 aggravated pre-existing condition, off work til further notice

Employee was cleaning the lower part of 8-WS-6 around the water drop area with an air chisel when ee bared fitting became unscrewed from the air chisel striking the employee's right ear with compressed air entering the ear canal.

EE slipped in the parking lot on 2/27/2018 spraining ankle and was released with restrictions but able to do job at full capacity. After a Dr. follow-up visit on 3/28/2018 and more x-rays it was discovered that EE had a slight fracture. We are filing this accident as MSHA reportable at this time after the x-ray diagnosis showed the slight fracture.

Miner attempted to move forks of a fork lift. Miners gloved hand slipped off the fork and little finger of left hand was caught between heel of fork and beam below bar where fork hangs from.

Miner walking from one location to another, uneven ground with loose rock. Miner slipped on loose rock causing miner to fall to the ground and land on right shoulder. Miner sought medical care on 11/18/16 and was released without restrictions. It was later determined that miner needed further medical care with restrictions beginning on 01/24/2017. Return to work full duty 01/30/17.

Miner placed wheel chocks behind rear wheels. Miner then picked up 5 gallon bucket and started to walk away when the rope attached to wheel chocks was tangled around foot causing miner to lose balance and fall on right shoulder.

Reported to immediate supervisor, however medical attention was not warranted per Miner request. Miner went to the doctor on his own behalf on 09/29/2014,the employer became aware on 10/07/2014 when notified by Miner that medical attention had been sought.
