Employee partially stepped on a stair edge and rolled EE's ankle.

Employee was opening the front gate and caught finger in the locking mechanism of the gate.

EE stepped up on the rear work bumper of EE's truck to turn welder off. When EE stepped back down EE missed the step and fell to the ground which was less than 3 feet and landed on left elbow causing it to dislocate.

An employee was assisting a customer truck by cleaning off material from the tailgate. The employee notice the tailgate was not latched, so ee tried to close it and caught the tip of finger between the tailgate and the bed of the truck.

EE was operating the dozer and about 2:00PM he began to experience chest pains. He called another EE on the radio and he called foreman and the office. An ambulance was called and he was transported to the hospital. MSHA was notified at 2:10. EE was kept overnight for observation and tests and a heart attack was ruled out. No definate diagnosis was given, he RTW w/no problems.

EE was on the quarry site for demolition of Liebherr R984 front shovel. EE pulled cylinder pin out of the boom and the cylinder fell on his foot.

EE was assisting 3 other EE's in moving a starter disconnect. The box had 2 power cables attached that had been covered with mud. In attempting to uncover the cable, the starter box slid towards the injured EE, tipping over on him. The starter box struck him in the head, under his hard hat, resulting in 8 stitches. EE RTW the same day with no other problems.

Silicosis claim.

3 employees were changing out troughing rollers on a belt conveyor. As employee was lowering a roller down to injured employee, the roller slipped out of his hands, striking co-worker on his left foot about 2 inches above the steel toe. He went to the doctor and an x-ray revealed a broken bone in his left foot.

MM Establishes that on the morning of 12/16/06, at approx 3:30 am two ee engaged in horseplay which resulted in physical altercaution. There were a total of three ee on the shift. Due to minor conflicts concerning the statement provided by the injured ee, this incident is under investgation.

WASHING OUT UNDER SCREEN TOWER IN SECONDARY PLANT. ABOUT 2:30 PM EMPLOYEE SET HOSE DOWN & VALVE OPENED CAUSING HOSE TO FLY AROUND EE & HE HIT HIS HEAD TRYING TO GET AWAY.

THREE EMPLOYEES WERE WORKING ON THE CRUSHER HEAD AND TWO WERE WORKING ON THE BOWL ON THE OTHER SIDE OF THE CRANE. WHEN FINISHED WITH THE HEAD THE THREE EMPLOYEES CAME OVER TO HELP WITH THE FEE D CONE. TWO WORKERS HOOKED THE CRANE CABLE TO THE OLD CONE. AS THEY STARTED DOWN OFF OF THE EQUIPMENT THE CRANE TAUT THE CABLE THE EQUIPMENT SHIFTED AND SLIPPED 6". EE FELL BACKWARDS ABOUT 40

EE WAS WALKING 30-B CRANE WITH FRONT WINDOW OVERHEAD. WALK ABOUT 200' AND WINDOW FELL KNOCKING HARD HAT OFF AND CUTTING TOP OF HEAD.

EE WAS BREAKING STEEL FROM DRILL WHEN HE GOT HIS RING FINGER ON HIS LEFT HAND CAUGHT BETWEEN STEEL & STEEL GRABBER ACAUSING A FRACTURE & LACERATION.

EMPLOYEE WAS ATTEMPTING TO PAINT AN ELEVATED AREA FROM A POORLY PLACED LADDER. THE LADDER ROTATED SIDEWAYS DUMPING THE EMPLOYEE ONTO A CONCRETE FLOOR AND LANDED ON TOP OF HHIM STRIKING HIM IN THE FACE.

EE WAS HELPING PUT METAL IN SCRAP METAL TRAILER. HE WAS IN THE TRAILER UNHOOKING HOISTED LOADS. AS HE UNHOOKED LOAD OF SCRAP THE METAL HE WAS STANDING ON SHIFTED AND EE FELL OUT OF THE TRAILER TO THE GROUND.

EMPLOYEE WAS USING A PRY BAR ONE DESIGNED FOR THE JOB TO TURN A 30' LONG SECTION OF 10" CHANNEL. THE BAR SLIPPED OFF OF THE LIP OF THE CHANNEL AND THE BAR STRUCK EMPLOYEE IN THE FACE.

CUT MIDDLE AND FOREFINGER ON L HNAD WHILE REPLACING CUTTING EDGE ON LOADER. LACERATION REEQ STITCHES HAND SLIPPED WHILE USING AIR WRENCH.

I NAME & I WERE ATTCHING A PTO HOUSING TO A 12V71 ENGINE USING A TRUCK CRANE ON I NAME TRUCK PTO SHIFTED CATCHING MY FINGERS BETWEEN ENGINE BLOCK & PTO HOUSING CUTTING THE SIDE OF THE FINGERS

EMPLOYEE ATTEMP7ING TO CLIMB THE OUTSIDE STEPS,HANGING ON THE HANDRAIL HE LOST HIS FOOTING AND FELL ABOUT 5FT.INTO A HOLE AND AGAINST A CONCRETE WALL.

INJ EMP SANDING3NEAR STOVE IN PIT WARMING HOUSE, FELLOW EMP DASHED FUEL OIL INTO THE HOT STOVE. FLAMES ROLLED OUT OF STOVE INTO INJUREDS FACE CATCHING HIS GLOVES ON FIRE. EMP RECEIVED SEVERE BURNS TO FACE A3D BOTH HANDS
