EE stepped up on the rear work bumper of EE's truck to turn welder off. When EE stepped back down EE missed the step and fell to the ground which was less than 3 feet and landed on left elbow causing it to dislocate.

An employee was assisting a customer truck by cleaning off material from the tailgate. The employee notice the tailgate was not latched, so ee tried to close it and caught the tip of finger between the tailgate and the bed of the truck.

EE was operating the dozer and about 2:00PM he began to experience chest pains. He called another EE on the radio and he called foreman and the office. An ambulance was called and he was transported to the hospital. MSHA was notified at 2:10. EE was kept overnight for observation and tests and a heart attack was ruled out. No definate diagnosis was given, he RTW w/no problems.

EE was on the quarry site for demolition of Liebherr R984 front shovel. EE pulled cylinder pin out of the boom and the cylinder fell on his foot.

EE was assisting 3 other EE's in moving a starter disconnect. The box had 2 power cables attached that had been covered with mud. In attempting to uncover the cable, the starter box slid towards the injured EE, tipping over on him. The starter box struck him in the head, under his hard hat, resulting in 8 stitches. EE RTW the same day with no other problems.

3 employees were changing out troughing rollers on a belt conveyor. As employee was lowering a roller down to injured employee, the roller slipped out of his hands, striking co-worker on his left foot about 2 inches above the steel toe. He went to the doctor and an x-ray revealed a broken bone in his left foot.

MM Establishes that on the morning of 12/16/06, at approx 3:30 am two ee engaged in horseplay which resulted in physical altercaution. There were a total of three ee on the shift. Due to minor conflicts concerning the statement provided by the injured ee, this incident is under investgation.

WHEN FINISHED WITH THE HEAD THE THREE EMPLOYEES CAME OVER TO HELP WITH THE FEE D CONE. TWO WORKERS HOOKED THE CRANE CABLE TO THE OLD CONE. AS THEY STARTED DOWN OFF OF THE EQUIPMENT THE CRANE TAUT THE CABLE THE EQUIPMENT SHIFTED AND SLIPPED 6". EE FELL BACKWARDS ABOUT 40
