Note: Accident initially was not a MSHA Reportable. While using the vacuum truck to clean up a spill behind #l Mill, the vacuum hose suctioned itself to the employee's right leg consequently pulling his legs apart which caused him to slip and twist his left knee.

EMPLOYEE WAS LOOKING OVER OPENING TO DETERMINE HOW MUCH CLEANING WAS NEEDED WHILE MILL WAS OUT OF OPERATION. LOST HIS BALANCE AND FELL TO FLOOR. THE FALL RESULTED IN A FRACTURED PELVIS AND SOME BRUISES.
While working in No. 1 Mill, which included several walks up and down stairs, employee felt the start of a headache, a nauseous feeling and problems breathing. A co-worker called 911 and an ambulance transported the affected employee to Manatee Memorial Emergency Room.
WHILE USING A 16 LB SLEDGE HAMMER TO KNOCK GYPSUM FROMS WALL OF A HOPPER, EMPLOYEE FELT A POP AND SHARP PAIN IN HIS LEFT SHOULDER.
