EE was assisting for the third time at greasing the plant. While the grease fitting was attached to the button on the grease line, pressure built up and as EE removed the line off the button, grease under pressure was injected into EE's finger. EE was wearing only latex gloves. The two associates decided not to report the injury to management.

As EE was leaving work for the day on Thursday, EE noticed the hangar gate was open so EE pulled in to shut it. While in the process of closing it, EE tweaked EE's lower back. EE worked all day Friday, but it continually got worse & EE was unable to work Saturday or Monday.

While removing torque out of truck, ee reached over frame and started pulling wrench towards ee and felt and heard something pop in shoulder .

Two associates installed and tightened the four liner bolts. Another associate was trying to determine if the liner was seated correctly by running their fingers behind the bottom edge of the liner. At that instant, the liner snapped into position crushing the tips of the index and middle finger on their left hand.

EE was struck in the forearm with a metal fragment from the tip of a grader that became dislodged when another associate was removing the end with a 3 pound hammer. The incident occurred on 8/31/17 & was first aid only. After a few months, the embedded object became infected & resulted in a medical procedure to remove the object at the doctor's office on 12/8/17.

Associate was removing ground engagement tool (wear plate) from the bottom, leading edge of a CAT 990 front end loader bucket. While pressing the bolt up to re-engage the square shoulder the associate triggered the impact wrench causing the bolt to spin and the sharp edges of the bolt to cut through the glove and cut his left index finger.

Associate was assisting with torch cutting on screen bed, turned and stepped on a rock deflector while twisting, felt pain in knee.

Scrap metal bin door fell open striking the associate on the thigh just above the knee

Associate was welding the new portable plant and slag from weld burned thru pants and burned his left shin.

Associate was attempting to clean windshield of loader when slipped while trying to step on the front left fender. Felt something strange in back.

ASSOCIATE WAS UNREELING WELDING CUTTING TORCHES, BECAME HUNG IN REEL. HE JERKED ON HOSES TO RELEASE, CUTTING TORCH CAME UPWARD HITTING HIS MOUTH & CHIPPING LEFT FRONT TOOTH.

EE WAS LOADING OLD SCREENS INTO A LOADER BUCKET.HE LOST CONTROL OF THE SCREEN, BENDING HIS THUMBBACK.

ASSOCIATE WAS WORKING ON THE VARIABLE SPEED CONTROL BOX IN THE ELECTRICAL HOUSE. HE WENT TO SET THE BOX DOWN AND TURNED AS HE WAS SETTING IT DO WN AND FELT SHARP PAIN IN HIS BACK.

EE WAS STANDING ON THE DECK OF CRANE, PULLING AT A HYDRAULIC HOSE THAT WAS BEING REMOVED WHEN HIS HANDS SLIPPED CAUSING HIM TO FALL BACKWARDS OFF THE EQUIPMENT.

EMPLOYEE WAS DRIVING A PUNCH THROUGH CONVEYOR WHEN HE MISSED PUNCH AND STRUCK HIS LEFT HAND.

EE REACHED INTO TOOL BOX FOR TOOL, UTILITY KNIFE WAS IN TOOLBOX WITH BLADE OUT. EE CONTACTED UTILITY KNIFE.

EE WAS REMOVING ROCKS FROM HAUL ROAD, STRAINED BACK.

EE WAS EXITING WET SCREEN TOP DECK VIA THE ROLL-A-WAY CHUTE, WHEN HE MADE HIS LAST STEP WHICH HE MISSED THE ANGLE THAT HE INTENDED TO STEP ON. HE FELL AGAINST THE ANGLE THAT HE WAS TRYING TO S TEP ON. Y Y

TWO PEOPLE WERE ATTEMTING TO ROTATE A SPRING-HOODED GIVOT ON A MOTOR MOUNT EE LOST HIS GRIP DID HIS FINGER WAS CAUGHT BETWEEN THE MTOR MOUNT AND THE PIVOT BASE.

EE WAS GRINDING WALL STEEL WITH GRINDER. STEEL FINES BUILT UP ON SAFETY ISDR WHEN EE TO VISOR OFF THE STEEL FINES GOT IN HIS EYES.

KNEELED DOWN TO PICK UP A PART [VERY LIGHT] KNEE GAVE AWAY.

EMPL CARRYING PORTA POWER UNIT IN RIGHT HAND WHILE WALKING UP THE WALKWAY. THE HYDRAULIC RAM CAUGHT IN THE CONVEYOR FRAME WORK AND YANKED RIGHT ARM SHARPLY.

TWO EMPLOYEES WERE TRYING TO LOOSEN A LOCKING NUT COVER FROM A 4 1/2' CRUSHER. THE TWO EMPLOYEES WERE AT OPPOSITE SIDES USING 12-16 1B. HAMMERS TO PRY IT OPEN-A PIECE OF METAL BROKE OFF AND ST RUCK ONE OF THE EMPLOYEES IN THE CHEEK.

AFTER CRUSHER HAD BEEN TIGHTENED WEDGES WERE BEING APPLIED TO CRUSHER. ONE HAD WORKED BACK OUT & FLIPPED UP INTO AIR & STRUCK EMPL IN MOUTH. REQUIRING STITCHES EMPL LOST TO TEETH.

EMPL WAS CHANING SCREEN LATER AFTER JOB WAS COMPLETED UPPER BACK BECAME PAINFUL. WENT TO DR NEXT DAY, UPPER BACK PULLED MUSCLE.

AFTER SNOW STRO4 EMP WAS SHOVELING SNOW FROM AROUND SCALE OFFICE HAD SHARP PAIN DEVELOP IN LOWER BACK IN SPINE AREA

EMP WAS CLIMBIN1 ONTO TRUCK WITH WATER COOLER IN HAND. HE REACHED FOR HANDRAIL AND MISSED RESULTING IN FALL FROM TRUCK ONTO A WORK BENCH

EMPLOYEE WAS RE4OVING A PIECE OF METAL FROM MAGNET WHEN MAGNET WAS TURNED BACK ON CAUGHT EMPLOYEES FINGER BETWEEN MATEL AND MAGNET.
