NO ISSUES UNTIL 5/26/23-SURGERY FOR ROTATOR CUFF**LOST TIME** Injury occurred after tripping over a timber while walking backwards when moving a ladder.

EE was assisting for the third time at greasing the plant. While the grease fitting was attached to the button on the grease line, pressure built up and as EE removed the line off the button, grease under pressure was injected into EE's finger. EE was wearing only latex gloves. The two associates decided not to report the injury to management.

As EE was leaving work for the day on Thursday, EE noticed the hangar gate was open so EE pulled in to shut it. While in the process of closing it, EE tweaked EE's lower back. EE worked all day Friday, but it continually got worse & EE was unable to work Saturday or Monday.
While removing torque out of truck, ee reached over frame and started pulling wrench towards ee and felt and heard something pop in shoulder .

Two associates installed and tightened the four liner bolts. Another associate was trying to determine if the liner was seated correctly by running their fingers behind the bottom edge of the liner. At that instant, the liner snapped into position crushing the tips of the index and middle finger on their left hand.
EE was struck in the forearm with a metal fragment from the tip of a grader that became dislodged when another associate was removing the end with a 3 pound hammer. The incident occurred on 8/31/17 & was first aid only. After a few months, the embedded object became infected & resulted in a medical procedure to remove the object at the doctor's office on 12/8/17.

Associate was removing ground engagement tool (wear plate) from the bottom, leading edge of a CAT 990 front end loader bucket. While pressing the bolt up to re-engage the square shoulder the associate triggered the impact wrench causing the bolt to spin and the sharp edges of the bolt to cut through the glove and cut his left index finger.
Associate was assisting with torch cutting on screen bed, turned and stepped on a rock deflector while twisting, felt pain in knee.
