Searcy Quarry Metal/Non-Metal

While cutting the top from a cardboard blast hole tube, the knife the employee was using slipped and punctured left forearm. This injury required 4 sutures.

Injured employee was climbing back onto machine, using three points of contact when EE heard/felt a pop in the back of EE's right leg.

Injured employee was rocking the feeder back and forth with the v-belts when the counter weights broke over the top, causing his finger to be caught between the v-belt and the v-belt pulley

Closing door on company field service truck and right hand was holding the handle on side of truck while stepping down and pinky finger was exposed (not closed in hand holding handle) and tech closed the door on finger. Finger tip laceration.

While task training in a Volvo A40, IE jarred back while sitting in buddy seat due to lack of seat suspension. Diagnosed with lower lumbar strain.

Employee was dismounting haul truck to help work on conveyor belt. While climbing down the ladder (with safety harness in one hand), he missed the last step and fell to the ground. He injured his wrist while trying to catch himself.

Employee was picking up the pads and attempting to put them back on the pegs located on the scene. While doing so he slipped and his finger got caught between the crane body and the pad, smashing/cutting his left hand finger.

South Alabama Drill employee was changing a drill bit and caught the tip of his left ring finger between bit and chuck.

Employee diagnosed w/soft tissue neck strain after hitting pot hole on quarry road. Doctor said to rest & return Mon. regular duty. Did regular job on Mon. attended training class Tues. On Wed. 2/08/06 employee thought he might need to go back to doctor; did not have any pain or a headache, but some stiffness in his neck. Physician scheduled physical therapy.

Employee was climbing the telsmith crusher frame to adjust the water spray. His foot slipped off the frame, some 2-3 feet above the walkway, and fell backwards to the side. He caught himself on his left hand resulting in a fracture to the left wrist.

Employee received a strain to his R upper arm when he slipped and fell from the drill platform to the ground; due to powerhead blowing out oil.

EE WAS DRIVING CHIPPER HAMMER UNDER BAKELITE SEAL. FELT PAIN IN HAND. FOUND 1/4" LACERATION IN HAND. X-RAY LATER DETERMINED A METAL FRAGMENT IN HAND. EE WAS SENT TO A HAND SPECIALIST FOR FURTH ER EVALUATION.

INJURED EE AND ANOTHER EE WERE TIGHTENING WEDGE BOLTS AT PRIMARY JAW CRUSHER WITH A 3/4 DRIVE RACHET AND SOCKET. THE SOCKET SLIPPED OFF THE NUT AND INJURED EE'S LT. INDEX FINGER; WAS MASHE D BETWEEN THE CHEATER PIPE THAT WAS ON THE RACHET AND THE BEARING HOUSING OF THE JAW.

EE WAS CUTTING LACINGS. OXYGEN LINE JUST BELOW CRIMP BLEW OPEN DURING A BURN BACK, CATCHING GLOVE ON FIRE AND BURNING HIS HAND. THE VANGUARD PROTECTION WAS NOT INSTALLED ON THE UNIT AFTER IT H AD BEEN REBUILT.

EMPLOYEE WAS HOLDING A PIECE OF METAL THAT WAS BEING WELDED TO A BUCKET. A LOOSE FIREBALL FROM THE WELD GOT DOWN INTO HIS SHOE. THE FIREBALL BURNED THROUGH EMPLOYEES SOCK AND INTO HIS SKIN. DI VISION OFFICE WAS NOT NOTIFIED UNTIL 9/19/01 OF EMPLOYEE'S INJURY. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 9/19/01 WHEN PLANT MANAGER INSISTED HE GO TO DOCTOR.

EE WAS UNLOADING PIPE FROM SEVICE TRUCK WHEN A BURR LEFT FROM DRILLING A HOLE CUT HIS MIDDLE FINGER ON HIS RIGHT HAND.

JUMPED OUT OF FRONT END LOADER.

STEEL WEDGE BERCAME LODGED BETWEEN JAW CRUSHER & BOULDER ROCK WEDGE DISLODGED FROM JAW SPRINGING INTO THE AIR HITTING EMPLOYEES ARM

EMPLOYEE WAS CL9ANING UP WITH A SHOVEL AROUND THE CRUSHING EQUIPMENT

EMPLOYEE HAD BEEN WELDING AND HE RAISED WELDING HOOD TO INSPECT WELD. HE WAS USING A CHIPPING HAMMER TO REMOVE WELD SLAG AND A PIECE OF SLAG HIT LEFT EYE.

EMP WAS CLEANING WINDSHIELD AND STEPPED ON SOME GREASE CAUSING EMPLOYEE TO START TO FALL AND EMPLOYEE CAUGHT HIMSELF BEFORE FALLING OFF MACHINE

EMPL FINISHED CLEANING OUT HOPPER ON 4\ X 8\ SCREEN AND WAS CLIMBING OUT AND STRUCK FOREHEAD ON CORNER OF STEEL.

EMPL CLEANING UP AROUND CRUSHER AND WIND BLEW SOMETHING IN EMPL EYE.

EMPLOYEE WAS STANDING ON A LADDER TO TIGHTEN BOLTS AND THE LADDER SLID TO ONE SIDE THROWING EMPLOYEE TO GROUND.,

EMPLOYEE WAS USING SLEDGE HAMMER TO MOVE METAL BASE THAT MOTOR MOUNTS ON AND THE HAMMER GLANCED OFF AND HIT LEFT FOOT

EMPLOYEE WAS INSTALLING BATTERY IN 980 CAT LOADER. HE ATTEMPTED TO LIFT BATTERY WITHOUT ADDITIONAL HELP AND STRAINED BACK
