Employee was using a 2ft. pry bar to raise a tube into position to be welded. The bar slipped causing EE to pull the bar towards self, striking the safety glasses, which then rolled into EE's eye. The employee was prescribed prescription eye drops which resulted in the incident being a medical reportable injury.

Closing door on company field service truck and right hand was holding the handle on side of truck while stepping down and pinky finger was exposed (not closed in hand holding handle) and tech closed the door on finger. Finger tip laceration.

On Wed. 2/08/06 employee thought he might need to go back to doctor; did not have any pain or a headache, but some stiffness in his neck. Physician scheduled physical therapy.

EMPLOYEE WAS HOLDING A PIECE OF METAL THAT WAS BEING WELDED TO A BUCKET. A LOOSE FIREBALL FROM THE WELD GOT DOWN INTO HIS SHOE. THE FIREBALL BURNED THROUGH EMPLOYEES SOCK AND INTO HIS SKIN. DI VISION OFFICE WAS NOT NOTIFIED UNTIL 9/19/01 OF EMPLOYEE'S INJURY. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 9/19/01 WHEN PLANT MANAGER INSISTED HE GO TO DOCTOR.
