Employee was in a haul truck when a rock fell and jarred the truck. Employee complained of back pain and utilized the RICE treatment until the January 24th. Up until the 24th, the employee stated multiple times they were feeling better each day. Employee went to ER with back pain and was released with prescriptions.

Employee slipped descending a dozer and fell on the tilt cylinder, breaking two ribs. EE had surgery on 4/11/2025 due to ribs not healing properly. Was released to full duty on 8/5/2025.

Injured employee was assisting with the replacement of a 42' conveyor belt. The conveyor belt slipped, striking employee in the ribs. The results in this action of the conveyor belt caused the employee to be knocked over the handrails of the cat walk he was standing on.
Working in the cab of a D9R, when employee opened the door, the wind blew dirt into his eye.

Injured employee was using a chain wrench to correct brushing alignment. His right foot got caught between the wrench and drill tower which resulted in a contusion and laceration.
Injured employee was stepping backwards while directing the loader to pick up a pallet, and felt his right knee pop. Employee was not carrying anything, did not trip on anything or was not on uneven ground. Received medical care on 12/06/07 and put on restriction starting 12/06/07.
The injured employee was troubleshooting the drill. As he accessed the area, his left ring finger came in contact with the pop off valve of air compressor resulting in cut.
