Employee was trying to remove material that had built up on an I-Beam with a hammer while working from a step ladder. In the process of removing the material, the ladder spun and folded, which caused employee to fall to the ground and strike the tail pulley guard below. This incident resulted in the Employee having a contusion of the left ribs and contusion of right leg.

Injured was helping welder line up pipe to weld to plate. Injured stuck right hand between pipe and plate when welder pushed on pipe to to line up for welding. Injured was wearing gloves but still fractured three fingers on right hand.
Injured was helping remove wear plates off loader bucket. Injured tried to lift a plate and throw in loader bucket. Injured states they felt lower back strain.
Employee failed to unplug chop saw prior to changing gout blade. While holding wrench on nut, they activated the saw which twisted wrist back. Injured saw doctor for strain but lost no time. On 3/10/2021 injured is now going to physical therapy but still able to work with restrictions.

Injured employee states they fell into gravel chute when they stepped out beyond walkway to weld edge strap on shaker lip. Injured state they bruised back and side of torso. EE was sent to Doctor who released them to return back without limitations. 4/01/2020 Injured went to first PT visit. Still not losing time.

Injured employee was shoveling material in finish tunnel when Injured felt sharp pain to lower back. Injured did report to Foreman but refused to go to doctor. Five days later Injured wanted to get back checked out by doctor. Treated and released with restrictions. No lost time from work.

The hydraulic hose on boom truck crane malfunctioned causing the tire on the crane to swing and strike employee on the right leg/knee area.
Injured struck back of left hand against control house metal door frame. Injured quickly turned to go into control house when EE struck and fractured 3rd. metacarpal bone on hand. Employee loss no time and is working light duty.

Employee was lifting heavy gears to install at plant. Injured states he felt L. shoulder pain but he failed to report. Injured later saw his personal doctor and just recently reported to Supervisor. Not loosing time, but going to therapy.
While working on cone crusher feed plate, Plant welder swung sledge hammer, head of hammer came off, ricocheted off feed plate and hit injured on top of left hand. Injured suffered hair line fracture to small bone on hand.
Equip. Operator was helping mechanic in shop when he went to get container from haul truck. Due to it raining Oper. fell when he reached for last ladder rung and landed on arm/shoulder. He refused medical attention. He recently decided to have doctor do MRI. Never lost time but now may need rotator cuff surgery.

On 04/30/2012, Injured Superintendent contacted me that he needs a MRI on Right Knee and he needs to report injury. I contested but Workmans comp will cover injury. Injured states he stepped on rock causing discomfort to knee. He didnt immediately report it thinking pain would stop. Orthopedic Dr. says he needs surgery for meniscus tear. At this time he has not lost any time.

EE was opening a metal container of welding rods, when he took his glove off to get a better grip he cut his finger on the edge of the container. EE needed four stitches to close the laceration. Working light duty, working the same hours.

Injured ee met with foreman prior to going to plant to assist in removing shaker guard to replace drive belts injured ee told foreman that morning that his back hurt he though that it was possible his kidneys, but he would be able to work the day. just after removing guard, EE now claims he strained back in same area of initial pain. Injured ee on light duty

Plant supervisor call for ee to assist others working on plant shaker bearing. EE stated that he did not feel good maybe indigestion; he walked over to operators control rm. and sat in a/c. After a few min the guy checked on him and found him on floor trying to get back to chair 911 was called as well as superintendent. CPR/first aid was given until help arrived.

INJURED WAS HELPING MECHANIC REMOVE IRON TOOTH FROM BUCKET OF EXCAVATOR. INJURED WAS HAMMERING ON TOOTH WHEN A PIECE FLEW OFF AND HIT INJURED AT THE BASE OF THE NECK CAUSING A 1" LACERATION. M ETAL WAS NOT IMBEDDED. DR. PUT STITCHES IN AND SENT INJURED BACK TO WORK.
EE WAS SERVICING WATER PUMP WHEN HE SLIPPED ON A LARGE ROCK AND HURT HIS RIGHT KNEE. EE HAD NOT LOST ANY TIME UP TO THIS POINT, BUT NOW HE HAD OUT-PATIENT SURGERY TO CLEAN UP HIS KNEE. EE WILL BE OUT OF WORK FOR FOUR WEEKS.
INJURED STATES THAT HE FELT PAIN IN HIS BACK WHILE CHANGING OUT ANVILS ON THE VERTICLE SHAFT CRUSHER. INJURED SAW HIS PERSONAL DR FOR SEVERAL MONTHS AND NEVER CHANGED OR MODIFIED HIS DAILY WOR K DUTIES. LATER REPORTING DUE TO ADMINSTRATIVE ERROR.
INJURED STATES HE HURT HIS BACK LIFTING A 5 GAL BUCKET OF WATER FOR HIS DRILL. INJURED SAW HIS PERSONAL DR FOR HIS PAIN AND DIDN'T REPORT THIS TO HIS FOREMAAN UNTIL 4-17-96, AFTER HIS DR TOLD HIM THAT HIS INJURY WAS UCH MORE SERIOUS AND WAS PROBABLY JOB RELATED.

EMPLOYEE WAS STEPPING OFF HIS DUMP TRUCK AT THE PIT AREA AND HE STEPPED ON A NAIL ATTACHED TO A BOARD WHICH PUNCTURED HIS LEFT FOOT.
