The blaster had just set off a blast and smoke was observed on the south side of the mountain after the dust from the blast had cleared. The Plant Manager checked the smoke, noticed a small fire on the south side of the mountain and notified the local Fire department and MSHA.

EE1 was working along EE2 and EE3 on re-torqueing the drive belt on the primary jaw crusher. EE3 was in control of the functions of the portable hydraulic ram. While EE1 was holding the hydraulic ram and spacer in position to tighten the drive belts, the ram and spacer slipped upward pinching EE1's right thumb between the motor and the spacer.

EE was lining up washer and wedges to install adjustment cap and floating hopper on a 5-1/5 cone crusher. The floated hopper separated from the adjustment cap and pinned the left forearm of the EE between the hopper and the bowl. The EE received medical treatment for cleaning of a puncture wound from a cotter pin and was released to full duty. No other injury occurred.

Changing shank on loader. In order to repair, had to remove wear plates that were bolted in. Using impact to remove nuts from bolt, cut palm on bolt. Received 12 stitches in right hand palm.
EE was welding for 15 min when received minor burns to eyes. The auto-darkening filter automatically shut off and the employee forgot to re-energize before EE began welding. EE reported eyes were burning the next day. EE was taken to ER where EE was diagnosed with minor burns to the eyes and released to full duty. EE was given a prescription for eye drops.

EE attempted to reinstall an A/C vent louver. EE placed fingers beside and over the edge of the louver trying to hold the screw so ee could screw the louver back on. The louver abruptly turned resulting in right middle finger and right ring finger to make contact with the squirrel cage fan. EE suffered a laceration and received 5 stitches to the right 3rd finger.
EE working on 992C Loader in shop. EE removed the Shut-Off Solenoid from RR of loader; it dropped to the floor. EE descended loader to pick up the part. EE slightly bent knees & bent over at waist to pick up part when ee felt a pop & pain right knee; ee then fell to the floor holding the knee. EE suffered a Transient Patella Dislocation (small knee cap dislocation).

A power outage was reported by the plant operator. Smoke was visible in NE Corner by east pond on property. It was determined the grass fire was caused by a downed electric line and area was secured; the fire dept and SW Rural Electric (SWRE)were called to the plant. SWRE secured the elec line and Fire Dept began to spray the flames. The fire was extinguished at 9:50 am.

Employee was opening the VSI crusher lid and middle finger got stuck between the hopper and the crusher lid. Employee obtain 10 stitches on the bottom of his middle finger.
Two employees were in the process of making welding repairs to the back of a skid steer loader bucket. The hoist cylinder continued lowering until it free fell forward and downward. When the employee realized the bucket was falling, he moved backward and the attachment ear on the right side of the bucket landed on top of his right foot crushing three toes.

Employee was working on P-3 skirt rubber. Skirt rubber bracket was stuck. He used a silver bar to pry the bracket loose, bracket sprung loose causing silver bar to slip striking him in upper left side of lip.

Employee was adjusting skirt rubber at tail section of conveyor while standing inside the bed of a 1/2 ton pickup. Employee loosened fastener bolts when ratchet came in contact with moving conveyor belt. Employee's finger was smashed between ratchet and conveyor.

Employee performing maintenance/welding repairs to the plant, stepped down from a platform (about 10" drop) and stepped on a rock, twisting his ankle. After first aid he was released with no restrictions. Later taken off work on 9/19/07.

Shift began 4:00 PM on 11/13/06 and ended at 4:00 AM on 11/14/06. Employee was using an aluminum step ladder to gain access to the lower section of the crusher he was working on. As he was decending the ladder he cut his left hand on a sharp edge on the top of the ladder.

WAS ATTEMPTING TO MOVE HYD. CYLINER WHICH WAS TOO HEAVY WHILE IN AWKWARD AND OR IMPROPER POSTION
EE WAS MAKING A DRILL STEEL CONNECTION. THEY HAD MADE THE CONNECTION. EE REACHED DOWN TO PULL THE V WRNECH OFF OF THE STEEL WHEN THE ROTATION KICKED IN TURNING THE STEEL AND WRENCH. EE S FOOT WAS CAUGHT BETWEEN THE WRENCH AND THE WRENCH STOP. HIS STEEL TOLD BOOT PROTECTED HIS FOOT, BUT ALL THE TOES ON HIS RIGHT FOOT ARE VERY BADLY BRUISED.
EE WAS DISASSEMBLING A SPRING STYLE THROUGHING ROLLER WHEN THE ROLLER FRAME HIT HIS LEFT LEG AROUND THE KNEE AREA.

EE WAS DESCENDING FROM A 30" CONVEYOR VERTICLE TAKE-UP WHEN HE LOST HIS FOOTING AND FELL LANDING ON A 4' PRYBAR THAT WAS IN A VERTICLE POSITION. PUNCTURE WOUND TO THE GROIN AREA. X-RAYS AND SO NOGRAM TEST CONFIRM NO DAMAGE DONE. CLEANED WOUND AND SUTURES SENT HOME SAME DAY.
