ANOTHER EMPLOYEE STARTED TO ASK THE OPERATOR QUESTIONS THAT EE COULD NOT RECALL PERIODS OF TIME OR WHAT EE WAS DOING. THERE WAS A NOTICEABLE CONTUSION ON THE EMPLOYEES HEAD BUT EE COULD NOT REMEMBER HOW IT GOT THERE.

Injured driving site pick up truck down access road approaching intersecting farm road. Upon entering intersection, injured vehicle struck by crossing farm tractor.
Dragline oiler assisting mechanic with removal of digging dog assembly. Both were lifting assembly by hand to relocate. Mechanic's grip slipped and dropped EE's end. Oiler's finger got caught in gearing, causing laceration and fracture of middle finger on left hand.
Mechanic was removing the digging dog assembly from a dragline with the assistance of the dragline oiler. Both were lifting the assembly by hand to relocate. Mechanic lost EE's grip, dropping EE's end of the assembly. Assembly struck EE's left leg, ankle, and foot.
Injured worker was counseling a truck driver located in the "tailgating" area. A second truck began to back into the area, injured began to walk backwards to clear the area. Injured did not look behind prior to stepping and tripped over a large piece of aggregate. Attempted to brace self with left hand when fell to ground.
Miner performing routine cleaning onboard dragline. While moving, lost footing and fell onto EE's back. Fell onto same level, steel deck.
Quarry Manager operating bulldozer, clearing for muck placement. Contacted old 10' pipe laying in the weeds. Pipe contained beehive. Bees swarmed bulldozer and operator.

Employee lost footing when exiting the excavator. Stress of body weight being applied to left arm holding onto hand rail, caused left bicep tendon to rupture.
Employee was cutting a dragline drag cable for removal and replacement using oxygen/acetylene torch. Oxygen supply line ruptured at connector on the base of the torch. Flash fire occurred and burnt employee on the right abdomen. Diagnosed as 2nd degree burn.

While pressure washing crusher box at Wash plant EE accidentally ran the pressure washer across right foot and the pressure punctured behind the steel toe of boot and left an approximately 1/2 cut on right foot.
On 7/11/17 EE reports that at approximately 10:30am EE fell to right knee and hit a rock while carrying a grease barrel to the dragline.

OPERATOR WAS GETTING OFF OF LOADER WHEN HE FELL BACKWARDS ONTO GROUND & LANDED ON BACK.
MECHANIC WAS TRYING TO REPLACE A BELT THAT HAD SLIPPED OFF OF THE ROCK CRUSHER. HE WAS USING A PRY BAR SLIPPED AND CAUGHT THE MECHANIC UNDER THE CHAIN/JAW AREA, (CENTER-RIGHT AREA) MECHANIC WA S TAKEN TO THE EMERGENCY ROOM FOR TREATMENT. REQUIRED 4 STITCHES AND MUST SEE A DENTIST FOR A TWISTED, CRACKED TOOTH.

WAS OPERATING DOZER WHEN ROCKS GOT UNDER THE TRACKS CAUSING THE DOZER TO ROCK. HITTING HIS LEFT K NEE, SEVERLY BRUISING IT.
