Employee was carrying bags of rock dust while kneeling over, when he felt his back pop and pain shooting through both legs.

**This accident was not reported until 12-11-2006** Employee stated that while shoveling she felt a sharp pain in her left shoulder. The area was muddy, slippering, and uneven. Employee has a pulled muscle in the backside of her left shoulder.

Employee was working on the header. The template that was being used fell off of the tandem roller and smashed his left hand, causing his hand to bruise and swell.

Rock fell out between pins striking employee in the head causing a muscle to pull in his back.

Slippery, wet, uneven surfaces that caused the employee to twist his left ankle cracking a bone in his ankle.

Over exerted pulling on come alongs, pulling a muscle in his back.

Third shift men were pulling a car cable, EE was standing beside miner and car cable hit him on the legs, knocking him down. He fell backwards onto the side of the miner and bruised lower back.

Hydraulic oil sprayed out of the boom lift jack of the roof bolter, spraying oil in miner's face, getting in his eyes. Miner was wearing glasses.

He was helping move miner cable, picked the miner cable up and felt pain in lower back.

Miner walked around shuttle car when he noticed small rocks chipping from the roof. He turned to avoid the falling rock, slipped in the mud and twisted his left knee. Hyperextened his left knee - strained and bruised.

Employee was putting up roof bolt in the face of the #3 right cross cut when the boom came down unexpectedly and struck him on his left knee. Possible torn ligements to the left knee.

Employee was hanging miner cable, picked the cable up and felt a pain in his back. The roof was 5 1/2 to 6 feet high and he was lifting cable by himself.

He was helping lap belt with three other employees and felt something pop in left shoulder.

Employee was pulling and lifting a steel cable, used to pull the gob truck as it was stuck in mud. He felt a pain in left shoulder and neck.

EE WAS CHECKING THE WATER LEVELS IN A RADIATOR ON A DIESEL SCOOP. WHILE ATTEMPTING TO LOOSEN THE RADIATOR CAP, THE CAP BLEW OFF AND HOT WATER AND ANTIFREEZE BLEW OUT, BURNING HIS RIGHT ARM AND SIDE. 1ST, 2ND & 3RD DEGREE BURNS TO ARM; 1ST DEGREE BURNS TO SIDE.

Employee strained lower back while hanging miner cable.

While employees were getting instructions at shack, a rock app 3" - 12" thick and 2' x 3' long, fell striking 2 employees on their heads shoulders and backs.

Employee was changing out bearing on slope take-up and felt pain in his chest. Pulled muscle in chest.

Employee was getting off of step ladder and twisted his right knee when he turned to step away from the ladder.

Taking water line apart with a hammer. Swung hammer to remove line and missed causing right elbow to strike belt framing. Soft tissue and ligament damage.

Draw rock, 7 inches thick, tapering down to 1 inch thick 2 ft. wide, 4ft long broke at ATRS smashing left fingers, mainly index finger, against pot while employee was trying to get pinner still out of pot.

Employee was directing traffic on unit, when scoop dropped bucket on top of his foot. Scoop was sitting half way thru a curtain. Employee was on inby side of curtain and scoop operator was on outby side of curtain. Didn't know he had his foot around or under scoop bucket.

The brakes locked up on the #3 shuttle car causing the car to slide into the rib. The employee had his hand on hand rail and his hand was mashed into rib of coal. Cut little finger and ring finger, broke little finger.

Employee was removing pin from splice hitting two hammers together when a small piece of the hammer head broke off and entered his left arm

EE was trying to move an empty pallet at #4 header. The pallet was stuck in mud and when he picked up on the pallet, he felt something pop in his right shoulder

Employee was loading a box of glue onto pinner and twisted his back causing pain in his back and right leg.

Making a splice in #4 belt. Knife slipped and cut left index finger. Bandaged on unit and went to Dr. for stitches.

MUD SLIP IN INTERSECTION FELL ABOVE ANCHORAGE ZONE. AREA IS APPROX. 20' X 20' X 8-10' HIGH.

A mud slip occurred in the #5 entry at the last open intersection spad #6 to 50. this lsip caused an unintentional roof fall app. 20' x 20' x 6'. the intersection had been previously bolted.

Employee was shoveling on slope belt header and threw mud and coal up onto the belt and strained neck and pinched nerve.

Employees were attempting to jack up one side of tailpiece with a railroad jack to prevent #3 belt from contacting the tailpiece. The jack slipped out (kicked out) and off of a piece of wood that the jack was sitting on, striking injured in the left knee causing pain and swelling in the knee.

Employee was crawling and climbing around on #3 header, when left knee started hurting, by the time he got out side his left knee started to swell, which was around 7:00 p.m.

Employee was watching cable to insure that there was slack in cable. Supervisor heard a pop and a "grunt" or noise and lost power on his equipment. Supervisor discovered EE laying on ground, unconscious with with no apparent injury.

#3 head drive is powered by tires. It appears the roller stopped and tires kept turning creating friction that ignited tires. Slip protection did not shut belt off.

MINING THROUGH SLIP AREA. HAD ALREADY ROOF BOLTED AREA AND PUT STEEL SETS UP. TOP FELL, TWISTING LEGS AND ROCKFALL OCCURRED. FALL WAS APPROX. 12 FT. LONG, 16 FT. WIDE, 8 FT. HIGH.

MINING THRU SLIP AREA. OUTBY APPROX 30' LONG X 17' WIDE X 10-12' HIGH FELL IN PULLING PINS OUT PINS WERE 5' TO 7' GLUE BOLTS, #5 ENTRY INBY SPAD #12+00 2ND NORTH PANEL OFF 2ND MAIN CAST.

EE WAS USING A PIPE WRENCH TO TIGHTEN UP A UNION ON THE MINER PUMP. THE PIPE WRENCH SLIPPED & HIT EE IN THE RT. EYE AND BROKE HIS SAFETY GLASSES.

EE WAS SETTING BITS IN THE LEFT MINER ON #1 UNITAND BENT OVER TO PICK UP A BUCKET OF MINER BITS WHEN HE PICKED UP THE BITS HE STRAINED HIS LOWERBACK.

EE WAS TRYING TO REPOSITION THE WELDING TRAILER SO IT COULD BE HOOKED UP. HE WAS MANUALLY PULLING IT AROUND AND PULLED LOWER BACK.

EE WAS INSTALLING FIRE SUPPRESSION SYSTEM ON OUTSIDE BELT DRIVE TO CLEAR UP MSHA VIOLATION. HE STEPPED IN MUD AND HIS RIGHT FOOT BECAME STUCK IN THE MUD. HE SAID HE TWISTED HIS RIGHT KNEE WHEN HE TRIED TO PULL IT OUT.

EE WAS HANGING OUTBY END OF WING CURTAIN IN #6 ENTRY. LEFT BOLTER WAS IN LAST OPEN BETWEEN 6-7. BOLTER CABLE WAS HUNG ACROSS #6 ENTRY OVER HEAD OF HELPER WHERE HE WAS HANGING CURTAIN. BOLTER O PERATOR STARTED HIS MACHINE NOT KNOWING HIS REEL WAS ENGAGED. CABLE PULLED LOOSE FROM WIRE IT WAS HUNG WITH KNOCKING EE TO GROUND. HE FELL ON HIS RT HAND, INJURING HIS THUMB.

EE WAS HELPING TO ALIGN A 75 HP MOTOR ON A COAL FEEDER. CO-WORKER WAS PRYING THE MOTOR UP WITH A PRY BAR AND EE PLACED HIS LEFT HAND UNDER THE MOTOR. THE PRY BAR SLIPPED LOOSE, MASHING EE'S LE FT SMALL FINGER UNDER IT.

CO-WORKER WAS PUSHING THE COAL FEEDER TO MOVE IT WHILE INJURED WAS TRAMMING FROM THE CONTROLS. THE BUCKET OF THE COAL SCOOP SLIPPED UPWARDS PUSHING A LID LOOSE AND ON THE RIGHT THUMB OF INJURE D BREAKING THE FIRST JOINT ON HIS LEFT THUMB.

THE INDIVIDUAL WAS CROSSING BELT LINE, WHEN HE STEPPED ON SOME BELT STRUCTURE. CAUSING THE BELT STRUCTURE TO LACERATE HIS LEFT UPPER LEG.

PUTTING THE MINER BREAKER UP - THE BREAKER FIRED AND BLEW BACK ON HIS HANDS.
