EE was cleaning when bucket with lime bumped a step causing lime to go into the air and enter EE's eyes.

Employee was placing lid on sample canister when it slipped and hydrate enter eye under safety glasses.

Employee stepped off loader and twisted ankle. Did not feel pain or observe any swelling until EE took boot off at home, but a "near miss" was reported at work. EE later went to doctor and was awarded light duty.

On 10/15 EE notified of leaving ER & due to sciatica. After having pain on 10/15 while outside of work EE believes it is related to near miss on 10/11. No previous indication of any injury or pain leads Worker's Compensation to investigate, but no determination made yet. Interim report is filed to meet 10 working day req until determination made.

Employee was moving a stone from catwalk when EE felt a strain on shoulder. EE went to doctor and was indicated to remain on light duty until 7SEP.

EE was opening inspection door with right hand when it slipped from EE's hand and fell onto the tip of EE's left index finger severing it. EE was taken emergency room by supervisor issued 2 days lost time and restricted duty.

While loosening brake on rail car, employee felt pain in abdomen area. Taken to hospital and diagnosed with strain.

On 05/04/2017, Beitzel Corporation employee, was using a 6' grinder with a cutoff wheel while working to cut metal as part of a skirt skid fabrication project at the Carmeuse Lime & Stone processing plant in Strasburg, Virginia. While cutting, the grinder appears to have caught in the metal, kicking back and striking ee in the left leg just above the knee.

Replacing wiring harness on C13. Pulled engine hood enclosure down and felt pop in Left Shoulder. Has continued to bother employee. Dx 2/1/17 with left tendon strain.

Employee hit C-channel with wire snips to nudge it into place. Slipped and jammed hand onto metal clip. Has a puncture in right hand. Had medical treatment and returned to work on the Monday following with doctor's release.

Employee was working on a gearbox motor assembly when the motor slipped from the assembly striking employee's hard hat causing lacerations to the left side of face and causing employee to fall and fracture right wrist.

Employee conducting pre trip inspection of vehicle and fell off of side landing on right shoulder/arm. Conditions were somewhat wet/muddy. Unknown extent of injury to right shoulder but could be rotator cuff injury, TBD.

Employee attempted to catch fuel nozzle that fell and cut finger requiring stitches.

Top of power pole was observed as smoldering with intermittant flames. Contacted local fire department and power company. Fire was approximately 12-16" in diameter at the top of the pole and did not travel down the pole. Approximately 3 hours elapased before it could be verified as 100% out. MSHA hotline called at 11:32 following inability to confirm it was out beyond 30 min.

Employee was dumping waste hopper at lab into loader bucket and pinched leg between hopper and exterior wall of lab causing injury to leg.

Employee stepped into a pile of lime and water. Minor burns to both ankles noted, first aid provided. Employee refused further medical treatment. Two days later employee wanted to be seen by doctor and was told diagnosed with 2nd/3rd degree burns.

Attempting to remove sheave on crusher while hammering wedge, wedge flew out and hit right hand.

Victim experienced stomach pains on site and died during surgery which was necessary to treat an abdominal aneurism.

Employee tripped while running and received a contusion to left knee.

Fire was noticed on conveyor belt, local fire department notified and extinguished approximately 40 min later.

Injured was dismounting a Cat 972 loader when his hands slipped from the ladder causing him to fall to the ground.

Injured was standing on a platform 4.5 feet off the floor, attempting to attach a come-along when he lost his balance and fell to the floor.

EE was wearing all PPE, when flipping welding hood down particle went into right eye. Could not was particle out with eye wash. EE went to urgent care .

Injured was helping to install a new 30" conveyor belt at the HiCal crushing plant. Injured had put clamps on the belt in order to pull the belts together when one side hung up on part of the frame of the conveyor. Injured lifted up on the chain that was hooked to the clamp and pinched his right hand between the clamp and the angle on the conveyor.

Employee and co-worker were attempting to wash out #2 ram at the preheater due to a fusing issue. While washing with high-pressure water, a mixture of lime and water splashed back into employee's face and eyes. He was wearing safety glasses and a faceshield at the time of injury.

A fire in the plant was noticed. Gas line was observed leaking and on fire. Columbia Gas was called and informed us to let it burn until someone came out. 911 was then called. Columbia Gas then showed up at 6:15pm and turned the main valve off. We found the gas line leaving the pre-regulator clamped to a piece of angle and it was deteriorated at this point which caused the leak.

Employee was cleaning the pre heater. When he finished the task he felt some discomfort in his lower back. He requested medical attention the following morning. He was diagnosed with a lumbar strain.

Employee #1 was holding the slug wrench in place while employee #2 was hitting wrench with a hammer. The hammer hit employee #1's left middle finger. Employee was diagnosed with a fracture to the tip of finger. He received a tetanus shot, glue for the laceration, a splint and an antibiotic prescription.

Employee was hitting the clogged chute and pinched his ring finger between the moving sledge hammer handle and the vibrator. He felt the impact but thought it insignificant. A few minutes later he noticed some blood on his finger. No problems with the injury until 12/28/10.

The employee was involved in snow clean up at the rotary kiln tunnel. We had 24 inches on the ground and it was still snowing. The loader cleared the area which compacted the remaining snow on the ground and made the area slick. The employee slipped on the snow and struck his left knee on the ground. Employee sustained a knee sprain.

Employee was using a pipe wrench to tighten a fitting next to the 02 analyizer. The wrench slipped off the pipe causing the employee's right arm to contact the flu gas discharge pipe which was hot. The employee sufferd 2nd degree burns to his arm and hand.

Injured was raking a piece of coating from kiln grates when a piece of lime entered his eye.

Injured was attempting to open a steel rail car lid. When he lifted the lid he felt pain in his lower back.

EE repairing rubber hose at Agg plant using utility knife to cut the hose, the blade broke and knife lacarated top finger on right hand. EE stated he was wearing gloves. This resulated in MSHA reportable (stiches) no lost time accident.

Employee was climbing ladder into haul truck and his knee "gave way". There was no cause, did not slip or bump knee. Employee has torn medial meniscus of the left knee requiring surgery. This is a reportable lost time accident.

Emp was performing housekeeping duties at jaw crusher using shovel to remove buildup around crusher. Emp developed pain in lower back. The injury resulted in a MSHA reportable restricted duty incident.

Emp was cleaning out duct at hydrator, went to step down and mis-stepped, falling backwards. He caught himself, but caused lumbar strain. This is a lost time accident.

Emp was climbing down from a loader and missed the first rung, falling to the ground. This is a reportable accident because he was fitted with a brace on his left knee. There was no lost time.

EE was placing a container of hydrated lime on an overhead shelf, the container fell striking the employee, the lid came off and lime splashed in EE face and eyes (EE was wearing safety glasses) burning cornea of right eye. This resulted in two days lost time.

While EE was observing a malfunction at top of rotary kiln dust blew into left eye (EE was wearing safety glasses). This is a no lost time reportable accident due to eye medication.

EE was removing wet material blocking an airlock using a 2.5' pry bar (wearing heavy gloves). While gouging out the material his left ring finger struck against a connection flange. This resulted in sutures which made it a reportable, no lost time accident.

EE helping to clear a stone vibrator in the rotary feed tunnel, while exerting pressure with his upper body, he misstepped and turned foot inward, resulting in a sprained right ankle.

Reparing water line with two shop EE's who were using a bar (~15lbs.)to raise line, the bar slipped and struck the EE on the right shoulder. The foreman was aware but EE did not think he needed medical attention. Monday when EE reported to work with pain in right should was sent to medical treatment. This resulted in MSHA reportable.

EE was waiting to clock in to start his shift. EE was arm wresling with an off duty foreman when his arm fractured.

Closing lid on pneumatic trailer using a 8' pole from catwalk above loadout chute. Leaned over the handrail to reach lid, it was stuck requiring extra force. He felt a slight pull in lower back but continued to work. Reported injury on 4/07/06 but did not seek medical attention until 4/11/06.

EE was tightening bolt with a rachet, the socket broke, EE fell backwards landing on his left side causing lumbar strain. This is a restricted duty report.

EE was standing on engine compartment of drill engine-EE removed panel-EE stepped on raised side of engine-compartment panel fill causing the EE to lose balance. EE fell approximately 6.5 feet. EE received fracture to left hip and lacarations to left elbow.

Ee rolled ankle when bolt broke from mushroom damper. Ee was bracing himself, standing on conveyor walls of the chute. Ee left foot was forward, right foot behind. Left foot was used to apply pressure to break the bolt. When bolt broke, ee weight shifted to right foot causing his ankle to roll over on the chute wall.

EE tripped getting away from a back flow of dusty material from a clean out port and fell to the catwalk upon his left knee. EE was using an air hose to remove blockage in the feed chute to the Gayco.

EE received an electrical shock when he pressed a reset resulting in 2nd degree burns. Section B is not completed since the matter was already immediately reported, in good faith, to MSHA and based on the medical information we have received to date, does not appear to be a serious injury as defined by MSHA regulations.

Employee was exiting right side of a dozer to remove a rock between dozer arm and sheild. Employee was using both hands on the hand rail while climbing down onto the dozer arm. His left foot slipped and he jumped backwards and turned landing on his left hand.

Employee was walking to fueling area to talk to his shift foreman, a gust of wind blew dust under his safety glasses. Employee took his glasses off and started rubbing his eye which contributed to scratching of eye.

REMOVING A SEAT BELT FROM CAT LOADER, STANDING BENT AT WAIST INSIDE CAB USING RIGHT ARM FOR SUPPORT WHILE USING LEFT ARM TO LOOSEN BOLT. EE USING RATCHET WRENCH APPLYING TORQUE TO REMOVE BOLT WHEN WRENCH SLIPPED. THE WRENCH STRUCK THE EMPLOYEE IN THE LEFT EYE GOING UNDER SAFETY GLASSES.

Employee backed his truck in place to be loaded with pond mud, excavator operator started to load truck at the same time operator decided to get out of his truck to clean mirrors, when excavator bumped truck bed door slammed into driver head.

ACCIDENT IS STILL UNDER INVESTIGATION. TO DATE UNABLE TO SPEAK TO INJURED PARTY FOR DETAILS. EXTENT OF INJURIES UNKNOWN AT THIS TIME.

ITT AND PAYROLL ADMINISTRATOR WHO SPENDS APPROX 75% OF HER TIME AT A COMPUTER. EMPLOYEE HAS DEVELOPED CARPAL TUNNEL SYNDROME.

CLEANING OUT UNDER BIN WITH SHOVEL, TURNED TO DUMP DEBRIS OFF SHOVEL & BACK WENT OUT.

THERE WAS APPROXIMATELY 3 INCHES OF SNOWFALL OVERNIGHT AND SNOW WAS CONTINUOUSLY FALLING. EE WASWEARING RUBBER OVERBOOTS.

EMPLOYEES WERE ATTEMPTING TO MOVE AN ID FAN MOTOR. ONE WAS HOLDING A 2' LONG 4X4 WOODEN POST WHILE KNEELING ON THE FLOOR. ANOTHER WAS USING A SLEDGE HAMMER TO ALIGN THE MOTOR WITH THE ID FAN. A 10'X4" HEAT SHIELD WEIGHING OVER 100 POUNDS FELL FROM THE IMPACT ONTO THE FIRST EMPLOYEE'S LEG.

EE WAS WALKING AROUND CORNER OF SCREENHOUSE AND DUST AND EXHAUST FROM THE BY-PASS STACK CLEAN OUT DOOR BLEW INTO HIS EYE.

EE WAS WALKING TO HIS CAR, SAID HE WAS WALKING FAST, AND FELL. THERE WAS NO TRIPPING HAZARD, ETC.

EMPLOYEE WAS ATTEMPTING TO CUT A LOCK OFF OF HIS LOCKER WITH A HACK SAW. THE SAW SLIPPED AND CUT HIS LEFT THUMB.

EE WAS TESTING A NEWLY INSTALLED LIGHTING CONTACTOR WHEN IT SHORTED OUT CAUSING INJURY TO EE'S EYES.

WHILE UNLOADING A RAILCAR TRYING TO VIBRATE LIME DOWN THAT THE SHAKER WOULD NOT REACH DECIDED TO PUT WOOD BLOCKS BETWEEN THE CAR AND THE SHAKER, WHILE GETTING THE BLOCKS IN PLACE HE GOT THUMB CAUGHT AND WHEN PRESSURE WAS APPLIED ON THE BLOCK HIS THUMB WAS LACERATED AND MASHED.

LIFTING A HEAVY OBJECT ONTO THE TAILGATE OF TRUCK.

WORKED AS A DOZER OPERATOR, INVOLVES USING LEVERS. HE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROM IN BOTH HANDS. REPETITIVE M;TION

CUTTING OFF TOP OF BOX WITH KNIFE.

GRINDING ON WATER SEALS ON CALCIMATIC KILN, PIECE OF METAL GOT IN HIS EYE.

WAS WORKING ON EUCLID, GOT FOOT CAUGHT AND FELL.

EE BENT OVER TO TURN WATER FAUCET ON, CAUSING MUSCLE SPASM IN LOWER BACK. EE HAS SUSTAINED PRIOR SPASMS IN BACK BEFORE WHICH WERE NOT WORK RELATED.

EE WAS LOADING BLASTING AGENT INTO SOME LARGE BOULDERS AND WHILE DOING SO WAS STANDING ON ANOTHER BOULDER APPROX 3 FT OFF GROUND LEVEL. WHEN HE ATTEMPTED TO STEP DOWN HE LOST HIS FOOTING FALLI NG ON HIS RIGHT ARM.

LOOKING FOR A PROBLEM WITH THE CALCIMATIC PREHEATER & HIS FOOT MISSED A BEAM HE WA GOING TO STEPON CAUSING HIM TO FALL APPROX 4 FT

WORKING ON STONE ELEVATOR THROUGH A PORTHOLE IN THE BOTTOM OF THE EEVATOR AND A COWORER DROPPED AN ELEVATOR BUCKET FROM A PORTHOLE ON THE SIDE ABOVE WHICH HIT EE'S RIGHT HAND.

EE WAS STANDING OVER TOP OF THE PULVIZER LOOKINGDOWN ON THE PULVERIZER PLATE, WHEN HE TURNED ON THE AIR, HE WAS TOO CLOSE TO THE PLATE & IT BLEWA PIECE OF LIME INTO HIS EYE

WORKING AT SIMPLICITY SCREEN, PICKED UP ONE OF THE SCREENS WITHOUT GLOVES ON AND SHARP EDGE CUT FINGER.

TWO EMPLOYEES 9 ONE ON VEHICLE AND ONE ON GROUND) WERE TALKING. EMPLOYEE ON VEHICLE DID NOT THINK EMPLOYEE ON GROUND HEARD HIM SO HE MOVED THE VEHICLE CLOSER. THE WHEEL OF THE VEHICLE CAUGHT T HE RIGHT HEEL OF EMPLOYEE ON GROUND.

SPRAYING WATER IN ONE OF THE PORT HOLES TO LOOSEN FUSED MATERIAL & EXPERIENCED A BAD BLOW BACK OF HOT LIME DUST & SPRAY FROM PORTH0LE

CUTTING BOLTS OFF UNDER TRUCK THAT HOLDS DOWN WELDER, SPARK WENT INTO LEFT EAR CANAL.

CUTTING EUCLID RIM WITH CUTTING TORCH WHEN HOT METAL SLAG WENT INTO RIGHT EAR BURNING EAR CANAL AND EAR DRUM. EMPLOYEE DID NOT THINK HE NEEDED MEDICAL ATTENTION AT THAT TIME. HOWEVER HE DID RE CEIVE MEDICAL ATTENTION ON 02-02-99.

PICKING UP A SAWHORSE AND TWISTED BACK. EMPLOYEE HAS ENCOUNTERED BACK TROUBLE IN THE PAST, MAY OR MAY NOT HAVE CONTRIBUTED TO INJURY.

OPERATOR UPSET 50 TON EUCLID TRUCK. HIS EXPLNATION WAS THAT HE FELL ASLEEP, GOT IN GRAVEL ON SIDE OF ROAD AND TRUCK FLIPPED OVER.

WHEN TWO STONE CARS HAVE BEEN LOADED THEY HAVE TO BE MOVED ON DOWN THE TRACT SO THAT 2 MORE CAN BE MOVED INTO PLACE. IT HAD BEEN THE PRACTICE TO MOVE THE CARS AT ONCE. WE FEEL THAT THIS PRACT ICE PUTS TOO MUCH PRESSURE ON ONE SET OF BRAKES SO A SOP HAS BEEN WRITTEN TO MAKE CERTAIN THERE IS ALWAYS A BRAKEMAN ON EACH CAR.

IT HAD BEEN RAINING, THE GROUND WAS MUDDY AND THE QUIPMENT SLIPPER. THERE WAS PROBABLY MUD ON GENE'S SHOE WHICH CONTRIBUTED TO HIM SLIPPING

GAYCO FEED CHUTE PULGGED UP - MATERIAL BACKED UP & WHEN THERE WAS ENOUGH PRESSURE, SCREW OPENED UP & MATERIAL SPILLED OUT ONTO EE.

WELDING LEAD WIRE HAD WORN PLACE WITH THE WIRES EXPOSED AND EE HAD WIRE UNDER HIS LEFT LEG. THE WIRE ARCED UNDER HIS LEG BURNING HIS LEG WITH 1ST AND 2ND DEGREE BURNS.

CLEANING DUCTS AT THE ROTARY KILN WHEN THEY REACHED THE MIDDLE DUCT IT HAS NECESSARY TO CHANGE TO A LONGER ROD ON WATER GUN. WHEN THEY PULLED TRIGGER ON THE HIGH PRESSURE GUN IT KICKED BACK AN D HIT THE RIGHT SHOULDER.

THE GATES ON BOTTOM OF THE EUC BINS WERE FROZEN , EMPLOYEE WAS USING A HAMMER TO BEAT THE DOOR LOOSE, ACCIDENTLY HIT HIS HAND ON THE SIDE OF ONE OF THE GATES.

CHANGING PIN ON #1 DRIVE, HIT THUMB AND INDEX FINGER WITH HAMMER.

LIME DUST FROM CALCIMATIC TRANSFER BELT TO 600 TON BIN FELL IN HIS EYE.

HELPING TO MOVE PORTABLE LIGHT PLANT, WHILE DIRECTING DRIVER OF TRUCK EE PICKED UP TONGUE OF THE HITCH TO HELP IN THE HOOKUP.

THE EE WAS ATTEMPTING TO REMOVE A LEAKING FUEL CAP. HE DOES NOT REMEMBER IF HE FELL OR BLACKED OUT FIRST AND THEN FELL.

WHILE WALKING THRU DOORWAY ON SECOND LEVEL AT HYDRATOR SYSTEM EE STEPPED DOWN ONTO THE CORNER OF THE STEP AND TWISTED HIS LEFT ANKLE. HE HAS A AVULSION FRACTURE TO HIS LEFT FOOT.

CLIMBING LADDER TO ELEVATOR AT ROTARY KILN WHEN DUST GOT INTO HIS EYE. HE WAS NOT CERTAIN WHETHER IT CAME OFF OF A RUNG OR THE WALKWAY ABOVE. SAFETY GLASSES WERE BEING WORN AT THE TIME OF THE ACCIDENT. EE WAS IMMEDIATELY TAKEN TO DR AND HADLIME EXTRACTED. THERE IS NOT PERMANENT IMPAIRMENT.

DESCENDING A LADDER AND HIS FOOT SLIPPED OFF RUNG AND TWISTED HIS RIGHT KNEE.

EE WAS PICKING UP BOX OF BLASTING POWDER, WEIGHING APPROX. 55 BLS AND HE FELT A SLIGHT STING IN HIS HIP AREA. THINKING IT WAS NOTHING SERIOUS EE DID NOT REPORT IT AS AN ACCIDENT. ON JAN 30 EE STARTED EXPERIENCING NUMBNESS IN HIS RIGHT LEG.

HYDRATE TANKER - DRIVER HAD PRESSURIZED ONCE & WAS ATTEMPTING TO DO IT THE SECOND TIME - HE WAS TAKING CAR OFF OF BLOW-OFF PIPE - THE PRESSURE IN THE LINE BLEW HYDRATE IN HIS EYES & MOUTH.

WHILE EMPTYING THE SPILLAGE CHUTE A LARGE PIECE OF MATERIAL HUNG UP IN THE DOOR. INJURED PULLEDON THE CABLE TO OPEN THE DOOR WIDER. HE FELT SOMETHING PULL IN HIS BACK.

EMPLOYEE WAS CLEANING UP A LIME SPILL CAUSED BY AN EQUIPMENT MALFUNCTION

EMPLOYEE STRUCK FINGER AGAINST LADDER ON SIDE OF CAT 930 LOADER

EMPLOYEE FELL FROM THE LEFT FRONT FENDER OF A 988B CAT FRONT END LOADER, APPROX. 10', LANDING FACE DOWN ON THE QUARRY FLOOR.

EE WAS CLEANING UP UNDER HYDRATOR. IN THE PROCESS LIME FELL FROM ABOVE FALLING BEHIND EE SHIRT COLLAR SETTLING AROUND HIS WAIST. THE LIME COMBINED WITH THE EE SWEAT CAUSED EE TO RECEIVE 2ND DE GREE CHEMICAL BURNS TO HIS LOWER BACK.

EMPLOYEE HAD PERFORMED ROUTINE MAINTENANCE ON SATURDAY 5-21-94 INCLUDING WELDING, CUTTING AND GRINDING OPERATIONS. IN THE EVENING OF 5-21-94, EMPLOYEE STARTED EXPERIENCING DISCOMFORT IN HIS LE FT EYE. ON SUNDAY 5-22-94, EMPLOYEE'S EYE WAS SENSITIVE TO BRIGHT LIGHT BUT CONTINUED TO WORK. DUE TO HIS DISCOMFORT, EMPLOYEE WENT TO SEE AN EYE SPECIALIST ON MONDAY, 5-23-94, AND DISCOVERED

EMPLOYEE WAS CLEANING OUT COOLER PIT AREA AT ROTARY KILN. LIME DUST WAS BLOWN, BY A GUST OF WIND, OVER THE TOP OF EMLOYEE, ENTERING HIS LEFT EYE.

EE WAS REPAIRING A CHUTE ON A SCREEN PICKED UP A METAL PLATE 1/2 THICK BY 10 FT X 4 INCH AND TWISTED INJURING HIS LOWER BACK.

INJURED EMPLOYEE WAS OPENING UP A ROTARY BY PASS DOOR THE AIR PRESSURE FROM INSIDE DOOR BLEW A LARGE PARTICAL OF LIME INTO EMPLOYEE FACE CAUSING LIME TO ENTER HIS RT EYE

EMPLOYEES WORKING OVERHEAD SWEPT LIME DUST OFF OF ROOF ONTO INJURED EMPLOYEE COVERING HIM WITH LIME DUST AS A RESULT LIME DUST ENTERED EMPLOYEES LEFT EYE

EMPLOYEE WAS CLEANINGOUT THE COOLER PIT AT THEROTARY KILN. IT WAS AN EXTREMELY WINDY DAY AND THE GUST OF WIND BLE WLIME INTO EMPLOYEE'S EYES.

EMPLOYEE WAS DISCONNECTING A PRESSURIZED HYDRAULIC HOSE FROM A CYLINDER ONCE THE FITTING WAS LOOSENED HYDRAULIC FLUID SPRAYED ONTO EMPLOYEE

EMPLOYEE WAS PERFORMING PREOPERATIVE CHECKS AT THE ROTARY KILN DURING HIS CHECKS IT BEGIN TO SLEET CREA TING SLIPPERY WALKING SURFACES WHILE WALKING ON A CATWALK ON TOP OF A B&D BINS EMPLOYEE SLIPPED ON ICY SURFACE & FELL BACK AGAINST A METAL BOX

EMPLOYEE WAS WORKING ON AN AIR LOCK UNDER A DUST HOPPER TO DETERMINE WHY A V-BELT WAS SLIPPING. WHEN EMPLOYEE STRUCK V-BELT WITH HAND, BELT CAUGHT & DUE TO A FAULTY AIR LOCK SPRAYED LIME DUST OVER EMPLOYEE.

EE WAS PERFORMING ROUTINE GREASING ON A CAT 988 LOADER. TO REACH GREASE FITTINGS EE WAS STANDING ON THE RIGHT FRONT FENDER. EE BACKED OFF FENDER AND FELL APPROX. 8FT TO THE GROUND STRIKING HIS CHEST AGAINST FENDER.

EWMPLOYEE WAS USING A METAL PRY BAR TO UNJAM ROCKS ON A CONVEYOR BELT. CAUGHT HIS RIGHT RING FINGER BETWEEN THE BAR AND CONVEYOR STRUCTURE MASHING HIS FINGER.

EMPLOYEE WAS USING A HAMMER TOP FORCE A SOCKET ONTO A RUST NUT. A PIECE OF METAL BROKE OFF OF SOCKET AND LODGED INTO EMPLOYEE'S ARM.

REFRACTORY BRICK HAD FALLEN OUT OF PREHEATER AT THE CALCIMATIC KILN. THE BRICK WAS LODGED BETWEEN THE ROOF AND FLOOR RESTRICTING ROTATION OF KILN. EMPLOYEE WAS USING A 5' X 1" DIAMETER STEEL P RY BAR PUSHED THROUGH AN EIGHT INCH PORTAL OPENING ON SIDE WALL OF KILN TO DISLODGE BRICK. THE PRY BAR SLIPPED OFF BRICKS CAUSING EMPLOYEE TO STRIKE SIDE/EDGE OF PORTAL WITH LEFT HAND.

EMPLOYEE WAS REPLACING A BROKEN PIN IN A HEARTH DRIVE AT THE CALCIMATIC KILN. USING A PUNCH AND A BALLPEEN HAMMER TO REMOVE PIN. EMPLOYEE STRUCK HIS KNUCKLE WITH THE BALL PEEN HAMMER.

EMPLOYEE WAS STEPPING OFF A RAILCAR, SLIPPED ON MUDDY SURGACE AND FELL TO GROUND. STRUCK LEFT ARM ON RAILROAD TRACK.

LOWERING A 1500 HP MOTOR ON TO ITS MOUNTAIN BOLTS THREE MEN WERE GUIDING THE UNIT WHEN IT CAUGHT THE TOP THREADS OF THE MOUNT IN BOLT WHILE WIPING DIRT OFF A MOUNT SHIME MOTOR SUDDEN DROPPED S EVERAL INCHES PINCHING THE TIP OF THE THUMB AT THE NAIL.
