Employee was hooking up light plant to his pick up, he was liptims upon the ongue of light plant to align it with the hitch on his pickup. The light plant rolled forward and caught his middle finger on his left hand between the light plant and the trailer hitch.

Employee was blocking up loader to fix flat tire, used a metal strip, this strip came out when the loader set down striking the employee in his chest and bottom of his chin.

Employee was stepping down from dozer, stepped on the push arm of the dozer, his foot slipped and he fell to the ground.

Blasting incident, one hole blew out and employee dove under his truck, pulling a muscle in his neck.

Employee was greasing auger heads on auger & bumped his right knee against auger head.

Employee was stepping down from dozer and while dismounting, lost footing, striking ribs on grab iron.

Employee was unloading parts from parts truck and felt pain in his right leg.

Employee was lifting slate rock from the rock crusher and hurt his back. He first sought medical attention on 9-26-06. Did not miss work until November 1, 2006.

Employee was taking final drive off of D11R dozer and hit his head on the bottom flange of fender, cutting into his scalp. This required stitches.

Employee was working on a drill when he bumped his elbow on the undercarriage, chipping a bone in his elbow.

Employee fell off the ladder while mounting a 7858 hauler. His hand slipped off the hand rail and he fell backward to the ground.

Employee was bent over to pull the entrance ladder up and felt a sudden sharp pain in his left lower back area.

Employee using come-along, both hooks were pulled open. The force released caused him to smash the end of his little finger on his right hand. Required surgery and two stitches.

Employee was operating a D-11 cat dozer when he backed over a rock. The dozer came down hard and jarred him. Neck was hurting and right shoulder.

EE WAS ATTEMPTING TO MOVE LIGHT BY PULLING ON LIGHT CORD AND THE LIGHT SPRANG TOWARD HIM AND HIT HIM ON THE LEFT TOP SIDE OF HEAD. REQUIRED 5 STAPLES.

EE WAS SHOVELING MUD OFF THE EDGE OF HAUL ROAD AND CLEANING OUT A DITCH. ALLEGES HURT BACK

EE HAD BEEN HELPING MECHANIC HOLD HOSES IN PLACE UNDER DOZER AND WHEN EE TRUNED AROUND, FELT BACK POP

Employee was preparing to change undercarriage on D11 dozer. After blocking machine and coming out from under it, he felt a sharp pain in right leg groin area.

Employee was cutting a hole in a battery box, finished, removed safety glasses and was putting his torches down. The rain was hitting the hot metal slag and it popped up into his face. A piece went into his right eye.

EMPLOYEE WAS OPERATING A DOZER, PUSHING IN THE SHOT & HE SLIPPED OFF A ROCK, JARRING HIS BACK & NECK.

EE working on a pump wrench slipped and hit him above the right eye making a small cut. Required sutures. EE did not miss work.

EE was welding on the end cap of a dozer push arm. He thought he had flashed his eyes, but it continued to get worse. He went to the doctor on Jan 7, 05, The Dr cleaned his eye and told him to stay off work on the 7th and he could return to work on the 8th of Jan. EE missed one day.

Employee was installing a hose on a Cat 5230 shovel in the boom section. When he let go of the hose, it popped out of the flange and hit him on his nose and under his right eye. This caused a cut that required (3) stitches.

EMPLOYEE WAS ATTEMPTING TO CLIMB ONTO A DOZER. HE HAD ONE FOOT ON A STEP AND ONE HAND ON HANDRAIL. WHEN HE PULLED HIMSELF UP A SHARP PAIN WENT DOWN HIS LOWER BACK AND LEFT LEG CAUSING HIM TO FALL.

EMPLOYEE WAS USING A 12 LB SLEDGE HAMMER WHILE WORKING ON AN UNDERCARRIAGE. HE EXPERIENCED A SHARP PAIN IN HIS LOWER ABDOMEN.

Employee was standing on two hydraulic hoses tightening a bolt when the extention to his wrench broke causing him to fall. He caught himself with his left arm, causing him to injure his left shoulder

EMPLOYEE WAS PUSHING IN SHOT ALONG BESIDE ANOTHER DOZER AND THE OTHER DOZER PUSHED A ROCK OVER INTO HIS RUN AND EMPLOYEE BACKED OVER THE ROCK HURTING HIS SHOULDER AND NECK. EMPLOYEE DID NOT RE PORT THIS TO HIS FOREMAN UNTIL 2-28-04. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION OR MISS WORK UNTIL 3-8-04.

EMPLOYEE PULLED HIMSELF UP ONTO HIS DOZER BY USING THE GRAB RAILS. HIS BACK STARTED HURTING.

EMPLOYEE WAS STARTING EQUIPMENT, STARTED TO CLIMB ONTO THE EXCAVATOR GRABBED THE HAND RAIL TO PULL HIMSELF UP AND FELT PAIN IN LEFT SHOULDER AND A SEPARATION.

EMPLOYEE WAS INSTALLING A VALVE ON A 994 CAT LOADER. THE VALVE SHIFTED PINNING HIS LEFT HAND AGAINST THE FRAME OF THE LOADER. NO LOST TIME.

WAS CUTTING BITS ON ROLLER FRAME, WHEN HOT SLAG OF METAL WENT BEHIND SAFETY GLASSES INTO RT. EYE.

EMPLOYEE WAS PUSHING REFUSE WITH A D10R DOZER AND WAS HAVING PAIN IN HIS BACK. THE PAIN GOT SO SEVERE HE HAD TO STOP OPERATING THE DOZER. HE SOUGHT MEDICAL ATTENTION. THE EE DID NOT INDICATE T HAT THIS INJURY WAS JOB RELATED UNTIL 8-4-03.

EE STATES HE WAS PUTTING FUEL INE ON TRUCK ENGINE WHEN THE WRENCH SLIPPED STIKING RIGHT HAND.

EE WAS INSTALLING HOIST JACK PIN WEIGHING 40-60 LBS. HE FELT PAIN IN HIS STOMACH.

EE WAS CHECKING OUT BOBCAT LOADER, HE OPENED DOOR TO CHEEK ENGINE OIL AND DOOR SWUNG BACK CLOSED CATCHING HIS FINGERS IN THE DOOR. CUTTING HIS LEFT INDEX FINGER REQUIRING 2 STITCHES.

EE WAS WORKING ON A MINOR BOGIE & ROLLER ON D-30DOZER THE BOGIE SHIFTED FROM ITS POSITION ON THEHYDRAULIC JACK. THIS PINNED THE EE HAND BETWEEN THE GROUND AND THE AIRGUN. THE EE CUT HIS FINGER AND REQUIRED STITCHES.

EMPLOYEE WAS OPERATING A DOZER AND SAID HE RAN OVER A ROCK CAUSING THE BACK OF THE SEAT TO SLIP, JERKING HIS NECK.

EE WAS REMOVING TRANSMISSION FROM A D-11 CAT DOZER, KNOCKING A PIN OUT WITH A SLATE BAR. WHEN THE PIN CAME OUT, COUNTERWEIGHT MOVED CATCHING END OF BAR. BAR CAME AROUND AND HIT HIM IN THE JAW. HE BROKE HIS RIGHT JAW AND REQUIRED SURGERY.

WHILE WORKING ON A 992G TOP BOOM PIN, LINING THE PIN UP TO INSTALL IN THE BEARING, EMPLOYEE FELT A "POP" IN HIS RIGHT WRIST.

EE WAS USING A SLAP HAMMER ON THE DOZER BLADE HE WAS WELDING ON TO KNOCK OFF THE METAL THAT WAS LOSE A PIECE OF SLAGG METAL CAME OVER THE TOP OF HIS HELMET INTO HIS EYE.

WHILE WORKING UNDER A D11R INSTALLING A RADIATOR HOSE, HE WAS LYING ON HIS BACK AND ROLLED OVER AND FELT A PAIN.
