Strip #12 Coal

A second EE backed his hauler into the hauler injured EE was sitting in waiting to get loaded. Proper safety procedures were not used in this incident. Both men said they were OK & did not need medical attention. On 12-15-15 injured EE came into work and said his back was hurting and wanted to get checked out. EE went to ER & was told to follow up with his Dr.

Employee was doing maintenance on a dozer, when he slipped & twisted his ankle - ankle sprain.

Employee was using a 980H loader to bust up coal, when he said he felt a sharp pain in his neck. (Back of neck).

EE alleged that the bed of the Cat 789C hauler free fell and undestinational distance, causing an injury to his back. EE said he raised the bed of the hauler & then as he was lowering it the bed stopped, he pushed the down lever & the bed of the hauler; FREE FELL down against the frame. The hauler was checked by the mechanic & no problems were found.

The steel braided oil supply line running to the left hand rear turbo charger ruptured. The oil sprayed onto and around the engine compartment area causing the fire.

Employee was moving a wooden pallet when the pallet moved causing the employee to jerk his hand from underneath the pallet. This caused a laceration to his left index finger requiring stitches.

EMPLOYEE WAS CHANGING A TIRE ON A PIECE OF EQUIPMENT. HE WAS ROLLING THE RIM FLANGE AWAY FROM THE PIECE OF EQUIPMENT AND SOMEHOW FELL FACE DOWN ON THE RIM FLANGE.

EE was walking to his service truck when his feet flew out from under him due to the snow covered ground. EE slipped, fell and broke a rib.

Operating a D-11 dozer, backed up hitting a rock that gave him a strong jolt, caused pain in lower back.

Employee was grinding metal and the grinder kicked back and cut index finger on his left hand. (Required stitches).

Employee was taking plug out of air tank. Pressure pushed hand sideways away from the tank into the housing, breaking his pinkie finger on his right hand.

Employee was putting a wear plate on bottom of 994 loader bucket using a pry bar to lift the plate. The bar slipped, mashed right hand.

Employee turned over a piece of metal and it fell on his left foot.

Cut leg on sharp edge of drum that had been cut in order to use it as a drain pan.

He fell through trailer floor while unloading trailer and twisted knee and back.

Employee was stepping down from dozer & his right foot slipped on a slanted rock, twisting ankle.

EE was using a pry bar to align the push-arm of a dozer, he felt pain across his left shoulder down to his thumb.

Employee slipped off of tire and he slid to the ground. The impact jarred him.

EE was dismounting from a mack800 powder truck, put his foot on the ground, his foot slipped and EE twisted his ankle.

Employee was putting a tooth on the bucket of 992 loader and slipped fell into the bucket landing on his left shoulder.

EE was originally injured on 12/29/04 at MSHA ID 4406991. This report was filed. EE now said he was operating his loader and when he changed direction, the loader jerked, causing some pain in his neck.

EE was working on drill, got dust/rust in right eye. Rinsed and thought it would be okay. It was worse the next day, sought medical attention.

D-78 cat D-11-R dozer caught fire from a ruptured hyd. hose spraying on the turbo. Fire burned longer than 30 min's. No one was injured. The fire was reported and investigated.

EMPLOYEE UNDERNEATH DRILL LOOKING FOR AN OIL LEAK, CAUSED DUST AND GRIT TO ENTER HIS LEFT EYE.

EMPLOYEE WAS OPERATING A LOADER WHEN A ROCK CAME OFF THE HIGHWALL, WENT THROUGH THE WINDSHIELD STRIKING THE OPERATOR'S FOREARM & LEFT LEG. STITCHES WERE REQUIRED IN HIS LEG.

Employee was loading a tank of oxygen on his welding truck, it caught on the bumper of the truck, twisting his back.

EMPLOYEE WAS FUELING GRADER (MG-6). WHILE PULLING HOSE FROM TANK, SLIPPED AND TWISTED HIS BACK.

EE WAS HELPING TAKE TRACK SEGMENTS OFF OF DOZER. ONE SLIPPED AND HIT HIS RIGHT HAND, INJURING HIS MIDDLE FINGER. REQUIRED SUTURES (LACERATION/FRACTURE).

Employee was operating a dozer, said dust came into the cab & got into his eyes.

EMPLOYEE WAS DISMOUNTING A DOZER, FELL OVER ON THE TRACK, TRIED TO BREAK HIS FALL AND HURT HIS LEFT ARM AND HAND. POSSIBLE HAIRLINE FRACTURE IN LEFT HAND.

CUT LITTLE FINGER PICKING UP A FUEL PUMP. THIS REQUIRED FIVE STITCHES.

EMPLOYEE MASHED HIS FINGER WHILE HELPING A MECHANIC TORQUE WHEEL LUG NUT. HE PINNED HIS RING FINGER BETWEEN THE RIM AREA AND THE MULTIPLIER WRENCH.

EE WAS GETTINT OUT OF MACK BULK TRUCK AND STEPPED ON A ROCK AND TWISTED HIS ANKLE.

EE TRIED TO CATCH A 12' LADDER THAT WAS FALLING. THIS CAUSED HIM TO FEEL PAIN IN HIS RIGHT SHOULDER AND ARM.

LARGE ROCK ROLLED OUT OF SHOT WHILE EE WAS BEING LOADED. THE ROCK STRUCK THE RIGHT REAR TIRE. THIS JOLTED THE TRUCK. EE SAYS HIS RIGHT HIP HURTS AND HE FEELS SOME NUMBINGS IN HIS HIP. HE WAS T AKEN FOR MEDICAL TREATMENT.

EE COMPLAINED OF LOWER BACK PAIN EE DID NOT INFORM HIS SUPERVISOR THAT HE WAS GOING TO SEEK MEDICAL ADVICE HE WENT TO DR. & THE DR PRESCRIBED PHYSICAL THERAPY & PAIN MEDS. WE WERE FIRST INFO RMED OF THESE ACTIONS ON 2/6/01

EE BENT OVER TO REMOVE RADIATOR CAP. HE TWISTED HIS BACK, FELT A SHARP PAIN.
