EE knelt down to adjust hooder film. EE went to stand up and their right knee starting hurting. Their knee continued to hurt throughout the shift and the following days. EE called in sick their next shift to allow their knee to rest. EE saw a medical professional on 9/18/25 and was put on restricted duty.

Crusher plugged. Material was spilt onto the ground. Employee was shoveling material when EE slipped on some mud. Employee's knee started hurting. Employee reported the injury but decided not to see a medical professional. On 11/7/23 employee decided to see a medical professional and was diagnosed with a torn meniscus.

Employee slipped on some mud and fell. Employee informed supervisor their shoulder and elbow were sore. Employee did not seek medical attention at the time of the injury. On 1/18/23 employee had a medical exam. On 1/24/24 employee had an MRI. Employee diagnosed with a strained muscle in left shoulder. On 2/7/24 employee had physical therapy.
Employee was using an industrial vacuum hose when lower back started hurting. Employee decided to work with a sore back until 9/11/23 when employee determined the back was not getting better. Medical treatment provided on 9/11/23 at which time the employee was put on restrictive duty.
Employee was coming down stairs. While on the way down the employee's foot caught the third step from the bottom. This caused the employee to fall and land on the right shoulder. This caused an AC joint separation.

The employee was lifting 50 lb chemical bags from the pallet to the feeder when the employee felt a pop and sudden pain in the employee's back. The employee's back continued to be sore after the incident. On 8/17/2020 the employee saw a chiropractor and received medical treatment for the injury.

EE was lifting 100 lb. bags off of the ground and restacking them. While doing so, tripped on some bags on the ground possibly straining their back. The repetitive lifting and the trip caused them to have a sore back. The next day their back was having spasms and was in pain. Employee was diagnosed with a back strain on 01/11/2018.

On 2/17/17 while working on the outside of the Process Elevator the EE was pulling a bearing off from the outside while another employee had cut the shaft with an acetylene torch that the bearing was connected to. EE picked up the bearing by the shaft causing burns on EE's hand.

EE was getting material samples in 16 ounce cups and was returning to the slab when he slipped. EE did not fall. He regained balanced but felt a "twinge" in his back.EE did not want medical attention. On Sept 8 EE said back was still bothering him and made appointment with a PA for Sept 9.PA recommended EE not work until Sept 13 and then on restricted duty for at least one week.

EE was cleaning the grizzly feeder from obstruction with a bar when foot slipped between the grates in the screen and he twisted his knee. Knee injured, put on restricted duty per medical examiner's instructions.
Supervisor observed on 2/4/14 that EE's eye was tearing up. EE said he had gotten dust in it on 2/3/14 while working on the packer. Supervisor asked if he needed medical attention and EE said no. On 2/5/14 EE informed Supervisor he had gone to the doctor and his eye was scratched. EE received prescription eye drops.

EE was cleaning a plugged chemical induction pipe when some chemical (ferric sulfate) came out of the pipe and got in EE's eye. EE was wearing safety glasses. EE had eyes flushed with eye wash solution and received a prescription for eye drops.
EE did not report until 4/16/2013 that he had jarred his back sometime in December 2012 while operating a front end loader. On 4/16/2013 he told his supervisor about the incident and wanted to go to the doctor. EE was placed on restricted duty awaiting the results of an MRI. Upon receiving results all restrictions were removed and EE returned to work at full capacity.

Employee was removing a counter weight on a Doosan D-25S forklift to investigate a steering column leak. The counter weight fell on top of employee's legs. Employee recieved severe laceration to the inside left thigh resulting in muscle, tissue, and ligament damage. Employee was transported to Washakie County Hospital for surgical repair.

The wrench the employee was using to loosen a nut on the #3 hot bin auger bracket slipped off causing the employee to catch his finger on the edge of the bracket and cut it open. The cut on the employee's left index finger required 6 sutures.

Employee was sealing a rail car when a wind gust blew dust into his right eye. Employee's eye was flushed at the plant but this did not relieve the pain. Employee was taken to a physician where his eye was reflushed, dialized, and a small particle was removed. Pain drops were administered to the eye and antibiotic cream was given to the employee to insert into his eye.

Employee was climbing down the ladder at the bag house door when she lost her footing, fell, and landed on the frame around the bottom surface of the bag house door. She twisted her right ankle and struck her knee. The accident resulted in an ankle sprain.
While working on a flourescent light from an extension ladder, the ladder came in contact with the light bulb and fixture causing the bulb to break. A shard of glass from the bulb cut the employee's left arm. The employee saw a physician to have the wound cleaned and bandaged.

INJURY IN THE FORM OF DISLOCATION OF THE ELBOW AND CHIPPING THE SOCKET POINT REQUIRING REMOVAL OF THE CHIP BY SURGERY.
