Miners finger was caught in a belt being tested on an air compressor.

Technician removing a relief valve from the rail carriage hydraulic system to troubleshoot Shuttle Wagon asset causing rail carriage assembly to lower and contact technicians left thigh. Minor laceration and a contusion to thigh.

IP set hammer to the side to clean pieces of concrete when hammer slipped and smashed finger.

IP was maneuvering a small electrical motor for alignment when tool slipped and motor landed on EE's left hand index finger causing superficial damage.

Alignment hand tool hit injured person in the chin while EE was attempting to free it.

Miner was walking when the miner tripped over a 10' piece of 6" steel C-channel lying along side the opening to a walkway.

Miner was climbing into a Dozer when they felt a sharp pain in their right groin area.

Miner smashed finger with hammer.

IP and other employee where using a chain hoist to pick up an electric motor for the #2 Swing Mill into position for the mobile crane to hoist into proper placement. The IP claims the IP disregarded it in the beginning and finished the job but the days following the pain had increased.

Employee had placed a 5'x10' sheet of metal on the north edge of table while vertical. Employee then went to lower sheet metal to a flat position when the west end of the sheet metal slipped and cut the lower left forearm of the employee.

Employee placing metal on sheer table. While doing so the sheet teetered and struck employee upper right arm just above the elbow causing laceration.

EE tripped on a step and fell against handrail striking elbow and forearm. Something in left shoulder popped. Lost range of motion in left arm. EE was seen by Dr on 11/30 & 12/8. Continued to work until 12/14, when EE could not do their job. Going to get an MRI done on 12/26. The company decided to have EE off until the MRI results are completed. Lost time begins 12/15/2023.

Employee was adjusting the deflector shield on a belt at the East quarry. Walking, they tripped over their feet, fell, hurting their shoulder. Went to orthopedic doctor on 10/10/2023. Received steroid injection in left shoulder. No work missed - still working. Unknown if any further treatment at this time.

Employee was moving transite from one roof level to an upper level. EE would crouch to support the transite while another person pulled it up with a rope. EE's right foot slipped while crouching to boost the transite. EE dropped down to knee and the transite sheet hit EE's chest on the left side.

Doing maintenance work on M117 Dust Collector.

Employee was climbing into the Grove Crane cab and pulled something in right shoulder. Employee continued to work, only receiving physical therapy. EE will be going in to repair tendon tears in the right shoulder on 1-24-2024. Off 8-12 weeks.

Employee claims EE got overheated on the job on 7/19/23. No accident of injury to report was completed until today. Employee never reported anything to the company before leaving work on 7/19/23.

Working on the 7th floor of the Mill Bldg, re-wiring the tripper motor and checking proximity switches. Employee got overheated causing heat exhaustion. Employee was taken to ER where EE received IV fluids, and sent home. Work release to return to work the next day.

Replacing the head pulley on the K420 clinker elevator. Extremely hot temperatures in the area. Employee got overheated, causing heat exhaustion. EE was taken to local ER where EE received an IV fluid and sent home. Work release to return the next day.

Employee was walking in the center cement loading bay area going west into the sun. The sun light was in EE's eyes and EE was placing EE's hands over EE's eyes to shade eyes and tripped over EE's own feet causing EE to fall into a piece of pipe; striking EE's right side of EE's body, cracking 2 ribs.

EE was lifting pan to top of 332 silo when the pan EE was guiding caught EE's pinky finger between the pan and piping. Laceration to pinky finger, received 3 stitches and returned to work.

Customer truck driver fell from the lid closing station after failing to correctly align the truck at the closing station. We assert that the incident does not meet the definition of an accident and the injured party was not a miner, so 30 CFR 5a does not require submission of the report.

Employee was walking down stairs from the baghouse, EE missed a step or lost EE's footing, missing 3 steps when EE fell on the transition platform twisting left ankle, landing on EE's left knee, straining EE's neck and must have hit EE's head on the hand rail causing EE to have a mild concussion.

Employee was lifting elevator K019 chain and buckets. Cable on hoist twisted and slipped off of head pulley. Cable caught employees left thumb when it became untwisted.

Employee was crawling into #4 Kiln riser on the 3rd floor. Began complaining about elbow being tender. Had been receiving first aid treatment since 2/28/2022. Condition has gotten more painful. Doctor has put EE on work restrictions for 6-8 weeks to allow elbow to heal, 6/3/2022.

Employee was replacing decking at the feed shelf. When EE was stepping onto the ladder, EE's foot slipped on the ladder. EE grabbed a pipe causing EE to swing into the ladder. EE received three broken ribs and a strain to the left shoulder. Will be off work 3-4 weeks to allow the shoulder and ribs to heal.

Employee was changing valves at bulk silos on the roof. They stepped down off the roll around ladder and slipped on a small patch of ice. Employee fell, twisting left knee. Grade II MCL strain. Will be off work 3-6 weeks.

EE was walking up the stairs in coal handling, when EE was on the fourth stair, EE slipped & fell forward on right wrist. EE cracked a bone & dislocated right wrist. Surgery was performed on wrist on 2/9/22 & will be off work for an unknown amount of time.

While packing cement the employee was not aware the cement bag was not in the #2 cradle properly as it filled. The bag pushed EE's right foot under the #1 cradle, cutting the top of EE's right foot. Right foot required 6 sutures.

Employee was emptying out a railcar loaded with Gypsum rock. EE was removing an air powered vibrator (weighing approx. 45 lbs.) from the railcar. EE removed the vibrator and turned to lay it down on the ground, approximately 36 inches. When EE turned, EE felt a sharp pain in back.

Employee was water blasting the pre-heater tower on both kilns from the 6th floor to the feed shelf. Employee experienced heat exhaustion and dehydration.

Employee was throwing rocks off walkway when a rock smashed EE's finger against handrail. Employee smashed finger, tore nail off finger and out EE's finger. Received 6 sutures to EE's left hand ring finger.

Employee stepped away from a railcar and got right foot hung up on an air hose and fell on right shoulder. Employee immediately reported accident but did not want to receive any treatment at the time. No time loss or treatment beyond first aid until 12/9/2020.

Employee was operating the loader in the east quarry, cleaning up the remaining rock on the quarry floor when they ran over a 2 ft. diameter rock that fell out of the loader bucket. The loader front tire ran over the rock causing the loader to rock to the left, pushing the employee into the ROPS inside the cab, hitting their head, causing a cut to the head requiring stitches.

Employee was moving scrap metal in a wheelbarrow when they brushed against a wall, causing the scrap metal to slide into their right hand. This resulted in a 2 cm long laceration requiring 5 sutures.

EE was climbing up onto the 35 ton haul truck after doing the pre-shift inspection. EE slipped on a glaze of ice on the step and mud on EE's boots, causing EE to lose grip with hands. EE fell on left shoulder and fell about 2 feet. X-rays taken and later an MRI indicating rotator cuff tear. EE continued to work until surgery on March 18, 2019.

EE and another worker were removing an analyzer from the kiln feed shelf. As the analyzer was pulled out ee dropped end due to sudden pain in right shoulder. The weight of end was no more than 40 lbs. Results of MRI indicate possible rotator cuff tear.

Employee received a first degree burn to right cheek and chemical burn to eyes from kiln feed during an inspection. EE was not wearing proper PPE. EE was treated at the hospital and then later by an eye Dr and by another Dr. ee will be released to go back to work the evening of 8/7/17.

EE was carrying 40lb sump pump motor from work bench to the pickup, when the pump cord got tangled on the work bench. The EE turned to see what the cord was tangled on when foot slipped and EE fell onto the concrete floor holding onto the pump. Pump landed on left hand causing a comminuted fracture to left hand. EE will see ortho doctor on 1/9/17 for further evaluation.

Replacing a motor on K660 coal drag. While lowering the motor to the ground it got caught on a mounting bracket causing the motor to roll, resulting in side-loading the gate on the hook of the Tuggit. Failure of the gate caused the motor to fall. In the fall the motor struck the removed guard causing it to slide into the employee's left shin and right ankle.

EE was cutting a piece of metal on metal shear. EE was lining up piece to be cut when ee pushed down on pedal causing plunger to engage and drop down. When ee plunger dropped down left pinky finger was under plunger, smashing finger on tip and pulling off nail and breaking bone at tip. EE had fingertip skin sewn up and no lost time was involved.

Employee was coming out of the cab of the dozer, walking on the tracks to step down to the ground, when EE stepped into the area of a missing plate on the track. EE's right foot slipped into a missing track piece which is around 8" x 24" in diameter. Employee twisted right knee with a possible MCL strain. An MRI was done, waiting for results. This will be a lost time accident.

Employee was sandblasting and was climbing down to platform when EE's foot slipped allowing EE's right hand, that was holding onto a piece of angle iron, to slip off cutting the inside part of EE's right hand below EE's thumb. Employee received 4 stitches to EE's right hand. Employee did not miss any work.

Was walking down the stairs and his foot caught a plug weld on the landing causing him to lose his balance and fall down the 6 steps in the stairway. He landed on his right shoulder and hip. He had a laceration to his R elbow requiring 3 stitches and a scrape on his R knee cap. X-rays revealed no broken bones. EE will RTW once the drug screen is received, could RTW after accident.

Employee was packaging cement into 70lb. Masonry bag when bag exploded blowing cement underneath his safety glasses into his right eye. Employee was taken to local eye specialist. The eye specialist required the employee to be off work for 3 days to allow the eye to heal. Eye received a chemical burn and abrasion around the cornea.

Employee was preparing to setup to transfer cement. When preparing to loosen bolts on the cement line employee had a very sharp pain in left shoulder and bicep. Was seen by Dr on 6/19 & 6/26. Dr reported he had slight tear in left bicep/shoulder. Could continue to work without any restrictions. Employee reported he was still in pain. Returned to Dr and surgery is 11/12.

Employee was walking from mill building to our shop area when he fell on a patch of ice at lab building. He struck his hip and back of head on concrete. He had a CT scan on head & X-Ray on hip, neither showed broken bones or swelling. He went to his physicians a few days later because pain and they diagnosed a bone bruised. He will under go therapy/rehab on hip.

Employee was changing out the K410 DDC burner gasket. While installing the gasket between the 8" flanges the pry bar slipped and the flange closed together pinching the end of his index finger on his right hand. The end of the finger was cut off and surgery performed later that afternoon. Employee returned to the doctor on Thursday, 11/7 for evaluation. Could be off 2-4 weeks.

EE was helping lift a rolled up piece of rubber belting measuring 36"x25' when he felt pain in his back on 5/16. EE was involved in shoveling & sweeping on 5/29 around the plant when he felt more pain in his back, more of a strain. EE was seen by a doctor on 5/28 & sent home on 5/29 to rest, ice & take medications for strained back after more pain. He will see specialist on 6/11.

EE was shoveling small rock and fine material onto cleanout belt when he felt a pop in his right bicep. EE cont. to work on the day of the injury. EE sent to local Dr. 4/9 then on 4/11 EE could not work due to pain in right bicep and shoulder areas. An MRI was done 4/12 when determined there was a tear in the rt. bicep tendon. EE will be off 8-12 weeks.

EE climbed up on tip of railcar to do preparatory work for loading. As ee started to stand up on the roof of the car his right knee popped our of place and locked up where he was unable to bear weight on his leg. EE was seen by a local doc and released to return to work. On 9/18 ee complained about his knee locking up and sent to another doc, on 9/21 ee was not release

EE was assigned for plant clean up when he began to feel light headed and irregular heartbeat. The outside temp.. was over 100 degrees. EEG was taken to the hospital for further checks and it was determined the EEG was dehydrated rec. IV fluids and was sent home a few hours later.

Employee was moving (dragging across floor) a stool over to the threader when his back popped. This resulted in lower back pain.

Employee was preparing to use a hand wood router, setting the depth of the router bit to the wood. Employee turned on the router, holding the wood down with his hand; the wood slipped pulling his left index finger into the router bit, resulting in cutting his finger and severing the top tendon.

The EE was walking along the west side of the CVR building when a gust of wind blew some dust in the EE's eye. EE was taken to the first aid room where his eyes were washed out, thus not able to get a small piece of dust out of his right eye. EE was taken to a local optometrist to remove the piece of dust from his eye. EE was told to stay home for 2 days to allow eye to heal.

Employee was doing a conveyor belt splice and employee was going to move to get something. Employee tripped over a hose causing him to fall into the wall, hitting his left elbow on a piece of angle iron on the wall. Cutting his left arm just above the elbow, requiring 8 stitches.

Employee was welding on K670 drag, crawling around inside of the drag on his knees. Went to get out of drag and his left knee was sore and hard to bend. Employee never missed any days from work due to this injury. Went to see a Dr and had an MRI on 11/8/2011. Will have surgery on his left knee around December 9, 2011 and will be off 1-4 weeks

EE was lifting coal buckets full of refractory material on the 2nd flr. of the #5 side of the preheater. No medical treatment was sought at the time of the injury. EE was sent to a local Dr. on 3/28 then an MRI and X-rays on 3/29 and was seen by a back specialist on 3/31/11. Possible herniated disk of L4-5. EE will be off at least 6 weeks doing P.T.

Cold front moved through the area re-freezing the surface of the road. Slipped on the ice twisting leg.

EE was helping the painter move a cart loaded with angle iron where it could be picked up by a forklift. The angle iron was resting on 2X4's. The cart hit a rock causing the cart to suddenly stop causing the 2X4's to roll forward onto their sides. When the 2X4's rolled one rolled onto EE left hand, Middle finger causing a fracture on the last digit of the finger.

EE was loading obsolete pumps onto a pallet. While lifting a pump the EE turned to place the pump onto a pallet and felt pain in his left knee. No medical treatment sought at the time of the accident. EE was still having knee problems and was sent to a doctor on 12/10/10. EE rec. an MRI and saw a Ortho. Dr. on 12/22/10. EE is still working until his knee surgery on 2/15/11.

Employee was walking on snow and ice, slipped and landed on his right forearm, causing pain in his right shoulder.

EE was helping work on the #4 Kiln gas pipe, when he became overheated and began experiencing abdominal cramps.

EE was trouble shooting noise reported around the pusher area of the tire conveyor with another EE. He heard the idler noise & crawled under the conveyor belt for a better look. He reached his hand into the idler area when the belt suddenly started. Co-worker pulled the pull cord to stop the belt & went over to release him from the belt. EE received only bruised left hand & wrist.

Employee was remving bin pulley drum from the c024 tripper and strained his middle/lower back. He continued to work through July 13,2009. Company sent employee to the dr on July 14,2009 because he was complaining of having back spasms and pain in legs and feet. Dr wants to evaluate in one week after resting back.

EE was installing 1/4" metal plate over hole in the Mill Building motor control center room. EE stepped down into the hole 7' x 2 1/2' x 16" to set the plate. While in the hole the EE twisted his body thus twisting his right knee causing a sharp pain. EE continued to work the entire day. EE woke up the next morning to come to work, but his knee was in pain. Sent to the Dr. for exam

Employee was lifting door on the roller mill fan with his right hand, slamming door onto the left hand of the employee. Left hand swelled up and took employee to the emergency room to x-ray hand & wrist. Employee received a small distal radius fracture to his left arm. Employee is estimated to be off work around 3 weeks.

Possible injury from operating the overhead crane(Carpal Tunnel on left wrist).

Employee was beginning to waterblast the #5 T/A duct, when hot clinker sand blew out the port hole. The hot sand blew up the sleeve of his aluminized coat causing 2nd degree burns to his right wrist and forearm.

Painting door outside by the paint shop,EE stepped on rock & fell to his knees on the hard surface. The EE reported the accident to the Co. on the date of the accident & never received any medical treatment until Friday, December 5th. The EE received an MRI on Dec. 5 & will see an orthopedic doctor on December 22 & probably will need surgery later. Currently no lost work days.

Water blasting the T/A ducts in the Preheater. EE experienced heat exhaustion.

EE was helping to install the gas pipe on the kiln, the gas pipe spun pinning his right index finger between the gas pipe and forklift rack. The right index finger-tip was broken and required surgery.

Employee was bringing out a 2x12 board from the cement separator and tripped on the steel toe plate around the ducting, fall on his knee.

Employee was lifting a large container of glycerin in the lab onto the table countertop, felt pain in right arm, hand & shoulder. Employee did not receive any medical treatment at the time of the injury. The employee went to a medical specialist on July 23, 2008 & will require surgery to his elbow & wrist on August 19, 2008 & will be off 4-6 weeks. Lost time begins 8/19/08.

EE was cutting a large piece of iron on a welding table. EE was sitting on top of the material he was cutting and when he had finished cutting the material, The EE fell on the floor, hitting his tail bone, lower back, right hip, left shoulder and left arm on the floor.

Driver failed to lower bed on 10 wheel dump truck, drove approximately 1/4 mile and struck bridge which removed bed from truck. He was taken by ambulance to hospital for evaluation then transferred to Wichita Hospital for further treatment.

The ee was driving and blacked out hitting a speed limit sign, with a concrete base, tipping over the sign, stopping the pickup. There was no injury to either employee, no property damage to sign or the pick. He driving was taken to the hospital for observation and was sent home around 7 pm and released to rtw on 12/28/07.

EE was flagging the crane operator while the forklift was being used to pull the crane into gear. The crane went into gear. A 3" nylon strap was hooked up to the crane and a 3/8" choker was hooked up to the forklift. The strap and choker were connected with a 7/8" shackle. Due to too much tension the choker broke. A slingshot effect occurred and the shackle hit employee's leg.

EE was stepping between dust collector compartments when his pant leg caught on the hold down for the door. Twisted his right knee. No medical treatment accident. He was still having knee problems and the company sent him to received a MRI on 10/17/07. He will be seen by a specialist at a date not yet known. He is still able to work at the present time.

Employee was cleaning the coal mill building and was filling 5 gallon buckets with material carrying the buckets down the steps and dumping them into a trash hopper, Injury to right bicep. Pain is now going into the right shoulder. The accident was not reported to the co. until sept 12. pain increase sept 19-send to dr.

Employee was finishing concrete under belt #3 take up pulley. He was inside the cage and stood up as the belt started. The belt counterweight hit him and knocked him into the support steel.

Employee was installing a spacer between a kiln gas pipe and the flex pipe. He was removing a 7/8" bolt from the flange because the bolt hole did not line up to the bolt. Employee was pulling the bolt and the bolt let go coming out hitting his right front tooth, breaking the tooth.

Employee was standing on an 8 ft ladder, leaned aganst the #5 clinker elevator. The ladder kicked out causing the employee to fall about 3 ft, landing on his back. Received a fracture to his L1 lumbar spine. No surgery necessary, just a few weeks off to allow fracture to heal.

Repetitive millwright welding work with right hand. Carpal tunnel.

Employee was using a come-along when his hand slipped off handle causing him to strain left shoulder. Treated on date of injury and was released to return to full duty with no restrictions or prescriptions. On 05/11/2006 insurance carrier contacted employer about results on MRI with possible surgery.

EMPLOYEES' HAND GOT PINCHED BETWEEN SCREW CONVEYOR BOLT & AIR HOIST. EMPLOYEE RECEIVED SUTURES ON HIS RIGHT HAND.

Employee standing directly under 2x2x1/4" angle while millwrights fitting side walls of a duct collector screws housing when the angle fell 1' onto EE's hard hat. He was taken to the hospital for observation. He was released to full duty. During the night he went back to hospital & got restricted duty release. He resigned 2/24/06.

Cleaning brick saw and nicked his left pinkie finger. Washed out at first aid station. Then driven to chanute memorial hospital to assure no infection and had 4 stitches, then he went back to work with no loss of time.

EMPLOYEE WAS CLEANING WITH THE VACUUM SYSTEM AND LIFTING LARGE HOSE CAUSED PAIN IN SIDE.

EMPLOYEE WENT TO CHECK JUICE RAILS ON BRIDGE CRANE. STARTED GOING DOWN STAIRS AT WEST END OF ACCESS STAIRS. TWISTED RIGHT ANKLE ON CLINKER BUILDUP ON SIDE OF STEP.

Cleaning cement out of Silo 694. Slide of cement swept feet out from under him. He tried to jump to the side, but leg went thru the door, which up-ended him causing him to land on elbow, which caused injury to his right shoulder.

EMPLOYEE WAS WALKING DOWN STAIRWAY IN THE #5 FINISH MILL BUILDING CARRYING MATERIAL IN HANDS. AT THE BOTTOM OF THE STAIRS HE STRUCK HIS LEFT ELBOW ON HANDRAIL. FIRST TREATMENT TO PULL BLOOD FROM ELBOW WAS AUGUST 15, 2005.

STANDING AT THE STOREROOM LOCATION, A LOUD NOISE STARTLED THE EMPLOYEE CAUSING HIM TO JUMP AND IMMEDIATELY FEEL A SHARP PAIN IN THE RIGHT SHOULDER.

OPERATING THE FRONT END LOADER, LOADING SHALE INTO THE STAMLER. LOST TIME 6/3/05, DIAGNOSTIC TREATMENT ONLY BEFORE.

MOVING DUST COLLECTOR FILTER BOXES AND PULLED OR STRAINED A MUSCLE IN RIGHT FOREARM. (THE INJURY WAS NOT REPORTED TO SUPERVISOR UNTIL 4/7/05 AND SAW THE COMPANY DOCTOR ON 4/18/05 AND STARTED PHYSICAL THERAPY ON 4/19/05

Cotter pin that holds socket in place caught employee's glove which extended the pin. This pin cut employee's left index finger. Finger received 6 stitches.

While sweeping floor on kiln floor, the employee developed lower back pain and muscle spasms.

EMPLOYEE WAS INSTALLING A DUMP CHUTE TO DIVERT WET MATERIAL. A PIECE OF GRATING WAS REMOVED TO INSTALL THE CHUTE. THE WEATHER WAS FREEZING RAIN. THE EMPLOYEE SLIPPED ON THE ICE FALLING THROUGH THE GRATE OPENING. EMPLOYEE RECEIVED A HEAD LACERATION, BROKEN COLLAR BONE AND BROKEN RIBS.

EE BENT OVER TO CHECK EQUIPMENT AND STRAINED HIS BACK. NOTE: NO TREATMENT UNTIL 10-11-04.

Employee was closing bottom doors on railcar with a 25 lb bar.

EMPLOYEE WAS TIGHTENING BOLTS ON THE DUCKWORK WITH AIR IMPACT WHEN SAFETY PIN WHICH HOLDS THE SOCKET SLIPPED OUT WHILE SPINNING. EMPLOYEE RECEIVED 14 STITCHES.

Employee was passing through the scaffolding when he tripped over a scaffolding support and landed on the right side of his body. Employee had pipefitting in both of his hands. Employee was placed on restricted duty for one day.

ERECTING NEW DUCT IN PLACE BY CRANE. THE DUCT WAS HUNG UP BETWEEN STEEL & WHEN IT CAME FREE, THE EE'S LEG GOT CAUGHT BETWEEN STEEL & CRANE CABLE. TAKEN TO DOCTOR. RETURNED TO JOB & WAS PLACED ON OFFICE DUTY FOR OBSERVATION. OBSERV. PERIOD WAS NOT COMPLETED: EE WAS INCARCERATED FOR UNRELATED ACTIVITY. POST-INJURY DRUG TEST: RESULTS POSITIVE.

Opening and closing east door #4 kiln feed shelf in preheater building, door was at such an angle that it was difficult to open and close. Employee strained his back opening and closing door. Injury reported to safety supervisor on 6-28-04.

Employee was pulling the holding cable up over the drum on the K080 crane and the cable got away from the employee, slapping the employee's wrist.

Employee got his finger caught in pinch point between rail switch and railcar. Six sutures required on left ring finger.

LIFTING A PLATE, C/O PAIN TO RIGHT SHOULDER.

EE WAS REBUILDING A CARDOX TUBE THAT HAD BEEN INSIDE AN ACTIVE PREHEATER. ONE END OF THE TUBE WAS NORMAL TEMPERATURE, BUT WHEN HE TOUCHED THEOTHER END WITH HIS BARE HAND HE RECEIVED A SECOND DEGREE BURN.

FINGER CAUGHT IN PINCH POINT BETWEEN I-BEAM AND FLANGE OF PIPE DISLODGED THAT HAD BEEN CUT OFF. SUTURES REQUIRED LEFT INDEX FINGER, POSSIBLE HAIRLINE FRACTURE.

EE WAS CARRYING A PIECE OF METAL TO MILL BLDG AND SLIPPED ON ICE FALLING ON RIGHT KNEE. PAIN CONTINUED TO RETURN SO WENT TO ORTHOPEDIC DR FOR CONSULATION. MRI SHOWED A TORN MENISCUS. SURGERY 1 1-21-03 RESULTING IN LOST TIME. NO OTHER TREATMENTS.

WORKER CLEANING INSIDE OF #5 PREHEATER #4 CONE. WORKER GOT FOREIGN MATERIAL IN HIS RIGHT EYE FROM TOP PART OF FACE SHIELD BRIM.

CLIMBING INTO THE CYCLONE AFTER BREAK, A PIECE OF CONCRETE BROKE LOOSE FROM ABOVE AND CAME DOWNON HAND.

EE WAS STEPPING OFF A MOTOR HOUSING TO THE FLOOR BASE SUPPORT. HIS FOOT CAUGHT ON TOP OF AN OLD LIGHT FIXTURE HE HAD JUST REMOVED & HE LOST HIS BALANCE BACKWARDS. HE ATTEMPTED TO CATCH HIMSELF WITH HIS LEFT ARM BUT HIS ELBOW BECAME DISLOCATED. THIS ELBOW HAD BEEN DISLOCATED YEARS BEFORE IN SCHOOL.

MOVING SOME IRON AND A PIECE FELL STRIKING HIS FOOT RIGHT BEHIND THE STEEL TOE COVERING.

EMPLOYEE WAS CHECKING THE OIL ON 990 CAT LOADER,STEPPED BACK TO CLOSE DOOR, EMPLOYEE LOST HIS FOOTING AND FELL APPROX 4 1/2 FT ON BUTTOCKS AND HURT HIS BACK.

EE WAS INSPECTING TRUNION WATER CIRCULATION SYSTEM ON #5 KILN TRUNION. WATER BLEW OUT WHILE HOSE WAS DISCONNECTED CAUSING HOT WATER BURNS TOL. FOREARM AND KNEE

EE WAS CLEANING UNDER MILL USING A HAMMER DRILL BROOM & SHOVEL. HE HIT HIS HAND ON WALL WHILE CHIPPING CONCRETE

EE WAS GRINDING WHEN HE FELT SOMETHING IN HIS EYE

EE CUT HAND.

HE WAS TURNING AN I-BEAM OVER TO BE MOVED. BEAM ROLLED BACK STRIKING RIGHT FOOT CAUSING FRACTURETO METATARSALS.

EMPLOYEE WAS WALKING THROUGH AN AREA WHERE A FELLOW WORKER WAS GRINDING ANGLE IRON. A SLIVER OF METAL BECAME LODGED IN HIS EYE.

EE WAS ATTEMPTING TO LINE UP A BEAM USING A SLEEVER BAR. HAD CLIMBED UP ON A SAFETY LINE TO TIGHTEN IT UP. THE SLEEVER BAR SLIPPED CAUSING HIM TO LOSE HIS BALANCE & FALL TO THE SURFACE BELOW.

EMPLOYEE STATED HE WAS TURNING CHUCK KEY ON THE MAY DRILL WHEN HE ACCIDENTLY HIT THE DRILL WITH THE KNEE/ LEG. THIS ACTIVATED THE DRILL, CAUSING ARM TO TWIST AND CATCHING HIS GLOVE IN THE CHUC K KEY CHAIN. EMPLOYEE HAD NOT UNPLUGGED EQUIPMENT BEFORE REPLACING DRILL BIT.

EMPLOYEE WAS WORKING INSIDE FINISH CEMENT COOLERWHEN ONE OF THE CROSS MEMBERS INSIDE THE COOLER GOT HIT, CAUSING LOOSE CEMENT TO FALL. IT FELL BEHIND EMPLOYEE'S GLASSES AND ENTERED LEFT EYE.

WORKER WAS CHANGING A FEED TANK VALVE AT 7FT ABOVE FLOOR LEVEL. WHILE HE WAS HAMMERING HE LOST HIS BALANCE HE CAUGHT HIMSELF WITH HIS LEFT ARM CAUSING A SHOULDER STRAIN

WORKER STEPPED INTO HOT DIRT MATERIAL @ 15" DEEP IN THE #5 CLINKER ELEVATOR BASEMENT. THE EE RECEIVED 2ND TO 3RD DEGREE BURNS ON BOTH ANKLES AT THE TOP OF HIS BOOT LINE.

WORKER WAS AIR LANCING FROM STAGE #4 CONE ON PREHEATER #5 PLATFORM, WHEN AN AIR PURGE CAUSED HOT DIRT MATERIAL TO BLOW OUT THE OPEN DOOR. THIS CAUSED BURNS TO BOTH FOREARMS AND THE FACE.

WORKER WAS COMING DOWN THE LADDER ON #4 PREHEATER RISER, 7TH FLOOR. HIS FOOT SLIPPED OUT OF THE LADDER RUNG AND CAUSED A TWIST IN HIS BACK. HE ALSO REPORTED PAIN IN HIS LEFT THIGH.

EE WAS WORKING ON A PREHEATER PLUG UP.L HE HAD REMOVED A CLEAN OUT PORT COVER, THEN TURNED AROUND TO TALK TO HIS SUPERVISOR. PART OF THE PLUG UP BROKE LOOSE, BLOWING HOT DIRT OUT ON THE LEFT S IDE OF THE NECK AREA AND INTO HIS GLOVE, CAUSING2ND DEGREE BURNS ON HIS NECK AND WRIST.

EE WAS REACHING INSIDE #1 SWING MILL DUST COLLECTOR AIRSLIDE TO DISLODGE A PIECE OF METAL THAT WAS CAUGHT, WHILE PULLING HE BUMPED HIS LEFT FOREARM, BRUISNG A NERVE IN THE FOREARM.

EE WAS WATER BLASTING BUILDUP INSIDE PREHEATER ON 2ND FLOOR. WHILE BLASTING HOT MATERIAL BLEW BACK OUT BURNING HIM ON THE BACK OF HIS NECK AND LOWER BACK AREA.

LACERATED THE POINTER FINGER ON RIGHT HAND, ON AGUARD OR SLEEVE WHILE REPLACING BEARINGS ON COOLER VENT FAN

WORKER WAS WATER BLASTING BUILDUP ON THE INSIDE OF THE PREHEATER ON 2ND FLOOR. WHILE BLASTING, SOME OF THE MATERIAL BLEW OUT TOWARD THE UTILITY WORKER CAUSING HIM TO STEP BACKWARDS. WHEN HE ST EPPED BACK HE TRIPPED OVER THE BLAST HOSE AND FELL ON HIS BACK AND TAILBONE AREA. THIS CAUSED A BACK STRAIN.

EE'S WELDING HOOD WAS IN A LIFTED POSITION. THE HOOD CAUGHT ON THE UPPER PART OF THE DOOR FACING AS HE WALKED FROM ONE ROOM TO ANOTHER IN THE ROLLER MILL BUILDING AND STRAINED HIS NECK.

EE WAS MOVING THE GROVE MANLIFT FROM ONE LOCATION TO ANOTHER. WHEN HE REMOVED HIS FOOT FROM THE ACCELERATOR, HE LOST HIS BALANCE AND FELL AGAINST THE HANDRAIL AND STRAINED HIS RIBCAGE MUSCLES.

W0RKER TIGHTENING SOME 1" PIPE IN THE PREMILL AREA. WHILE WORKING HE IT HIS LEFT ELBOW ON METAL IN THAT AREA.

EE WAS HIGH PRESSURE WATER BLASTING ON THE 2ND FLOOR OF THE #5 PREHEATER. WHEN HE PULLED THE BLASTING WAND OUT OF THE PREHEATER PORT HOLE, HIS FINGER SQUEEZED THE TRIGGER & THE WAND FLIPPED U P HITTING HIS FACE SHIELD. THE IMPACT WAS HARD ENOUGH TO PUSH HIS SAFETY GLASSES HARD AGAINST HIS EYEBROW AREA, CUTTING THE SKIN

EE ATTEMPTED TO CLIMB STEPS ON CAT 990 FRONT ENDLOADER MISSED THE HANDRAIL AS HE STEPPED UP & LOST HIS BALANCE & FELL BACKWARD TO THE GROUND. HEFELL ON HIS ELBOW &BACK

EE & ANOTHER EE WERE SLIDING A HIGH PRESSURE CYLINDER DOWN THE #3 KILN STAIRWAY. EE LOST HIS GRIP ON THE CYLINDER & EE TRIED TO CONTROL THE CYLINDER WEIGHT WITH HIS RIGHT ARM

WORKER WAS INSTALLING KILN DISCHARGE METAL PLATES ON TOP SIDE OF KILN. WHEN HE STARTED COMING DOWN, HE CUT HIS LEG ON THE METAL PLATES. STITCHES WERE REQUIRED.

EE OPENED THE BOTTOM CLEAN OUT DOOR ON #5 PREHEATER TIRE CHUTE WITH GLOVED HAND. HOT MATERIALS FELL INTO GLOVE CUTT FROM ABOVE CLEAN OUT DOOR BURNING WRIST AND THUMB AREA OF RIGHT HAND.

EE WAS USING AN AIR LANCE ON 4 PREHEATER. A CHUNK FELL DOWN INSIDE, CAUSING MATERIAL TO BE BLOWN OUT THROUGH A PORT HOLE INTO BOTH EYES. EE WAS WEARING SAFETY GLASSES.

EE WAS PULLING GAS ANALYZER PROBE AND STRAINED LOWER BACK.

EE MOVED BATTERY CABLES ON HIS VEHICLE AND BATTERY EXPLODED AND BLEW INTO HIS EYES & ON FACE. CHEMICAL KERALOCONJUNCTIVITIS BOTH EYES AND MINOR BURNS TO NOSE & FOREHEAD

EE WAS IN HIS PERSONAL PICKUP AND OFF DUTY DURING LUNCH HOUR. HE WAS RETURNING TO HIS WORK AREA WHEN HE COLLIDED WITH A CONTRACTOR HAUL TRUCK. OFF DUTY EE SUFFERED A CONCUSSION. PLEASE DETERMI NE IF THIS IS A CHARGEABLE ACCIDENT.

EMPLOYEE WAS USING SKIP HOPPER TO HOIST SCRAP MEATERIAL FROM 7TH FLOOR OF PREHEATER, WHEN CRANE STARTED TO PICK UP HOPPER IT SLID AND CAUGHT EMPLOYEE FOOT IN PINCH POINT BETWEEN HOPPPER AND BLOCK WALL AND BRUISED FOOT.

SAWING A BRICK AND GOT A SPLINTER IN FINGER FROMDIAMOND BLADE.

EE WAS HOLDING COAL MILL ROLL AS A FELLOW EE SWUNG A SLEDGE HAMMER, THE HANDLE BROKE IN MID AIR. THE SLEDGE HAMMER STRUCK THE RIGHT THUMB OF THE INJURED EE-FRACTURED 1ST JOINT.

EE STEPPED ON SOMETHING AND BRUISED INSTEP OF RIGHT FOOT.

EE WAS SHOVELING ROCK & MUD INTO WHEELBARROW, FELT PAIN IN NECK & BETWEEN SHOULDER BLADES. [CHIROPRACTOR]

WHILE SUPERVISING BRICKMASONS IN A SMALL CEMENT KILN, A BRICK FELL OFF BRICK RIG & STRUCK SUPERVISOR'S LEFT-HAND INDEX FINGER, CAUSING A COMPOUND FRACTURE TO FINGER.

NO TREATMENT OTHER THAN DIAGNOSTIC RECEIVED FOR THIS INJURY UNTIL 12-4-97. EE WAS WORKING ON POPPIT VALVE OF DUST COLLECTOR WHICH CYCLED ON TO HIS LEFT HAND COMPRESSING LIGAMENT ON A NERVE. HE DID NOT TURN AIR OFF OR BLOCK POPPIT VALVE.

EMPLOYEE WSA PULLING CRANE CABLE UP TO DRUM AND STRAINED MUSCLE BETWEEN SHOULDERS.

EE WAS MOVING OVERHEAD HOIST WHEN HIS LEFT LEG FELL THRU A 2' SPACE IN FLOOR GRATING. HE PULLED THE MUSCLES IN HIS LEFT SHOULDER AS HE CAUGHT HIMSELF. NOTE FLOOR GRATING HAD NOT BEEN PROPERLY REINSTALLED BY A CONTRACTOR NOR WAS THE OPENING INDENTIFIED.

CLINKER SAND GOT INTO EYE WHILE EMPLOYEE WAS REPAIRING DROP OUT SCREW.

EMPLOYEE WAS TAKING TAKE UP PULLEY OFF ELEVATOR AND FELT PAIN BETWEEN SHOULDERS.

EE WAS TRYING TO LOOSEN BOLT ON CYLINDER AND PULLED MUSCLE WHILE USING WRENCH ON BOLT.

EMPLOYEE WAS POSITIONING CHAIN FALLS ON TROLLEY BEAM AND FELT PAIN BETWEEN SHOULDERS. CHIROPRACTOR.

EE WAS TRYING TO RESET CIRCUIT BREAKER AND CAUGHT FINGER IN PINCH POINT, LACERATION RIGHT INDEX FINGER.

EE WAS USING PIPE WRENCH ON AIR HOSE COUPLINGS AND HURT SHOULDER.
