Employee was exiting a Cat D-8 dozer when EE missed the bottom step and fell approximately 2 ft. to the ground. EE sustained a sprain to the rt. wrist. Possible unspecified fracture. Diagnosed on 8/24 as a fracture to the wrist requiring a cast.

The employee was operating an articulated truck. Employee stated employee fell asleep. The tire tracks indicate the truck rode up the left side embankment causing the cab to turn over on the right side. Employee received a concussion without loss of consciousness and a laceration to the right ear without foreign body.

Employee was exiting the Volvo Excavator while carrying the DEF fluid after inserting DEF fluid into the machine, while stepping down onto the foot step the employee foot slipped off of the pivoting step causing the employees body to twist resulting in shoulder pain. This incident was designated reportable based on number of physical therapy visits.

Employee was tightening the cheek plate bolts on the Primary Jaw crusher with a slug wrench and a 4lb hammer. Employee struck the frame of the crusher causing the hammer to glance off of the frame striking his left hand. Employee was wearing proper PPE (gloves and safety glasses)

At the beginning of the shift the employee was fueling his haul truck. After fueling, he got back in his truck and started to the pit. After wiping his face and eyes with his hand, he felt something in his eye. After reporting this to his supervisor, he washed his eyes out. This effort did not improve the situation, so he was taken to the company doctor for treatment.

Employee was pulling on a conveyor belt when he experienced pain in his right shoulder. He continued with his normal job duties without any lost time. After continuing pain, an MRI revealed that the employee suffered a muscle tear that will need surgical repair. The employer became aware of these results on 10/3/05, which at that time this injury became reportable to MSHA.

Employee stated he experienced back discomfort while attempting to lift the hood of a Volvo truck

EE REPORTED BACK STRAIN TO COMPANY ON 8/9/04. HE STATED THAT DISCOMFORT OCCURRED WHILE LIFTING HOOD OF ARTICULATED TRUCK.

INJURED EE'S RIGHT HAND WAS CAUGHT BETWEEN THE INNER AND OUTER SECTIONS OF A BOOM TRUCK'S BOOM CAUSING FRACTURES TO HIS RIGHT HAND'S RING AND MIDDLE FINGERS.

INJURED PERSON WAS ASSISTING IN THE REMOVAL OF A MOTOR SHIV ON THE PRIMARY JAW CRUSHER. UPON DISASSEMBLY THE SHIV FELL TO THE GROUND PINCHING HIS LITTLE FINGER ON THE LEFT HAND BETWEEN THE SHI V AND THE GROUND.

EE WORKED HIS ENTIRE SHIFT IN THE HEAT 95 DEGREE AND DRANK A LOT OF FLUIDS. AFTER LEAVING WORK OAND RETURNING HOME DECIDED TO GO TO HOSPITAL BECAUSE HE FELT DEHYDRATED DR SAID HE HAD HEAT EXHA USTION AND INSTRUCTED HIM TO DRINK PLENTY OF FLUIDS AND TAKE A DAY OFF.

THE EMPLOYEE WAS FIXING A RUBBER WAER LLINE WHEN HE CUT THROUGH THE LINE WITH A KNIFE AND CUT HIS INDEX FINGER.

WHILE UNLOADING A NEW WELDING MACHINE, THE EMPLOYEE WAS HOLDING ON TO THE TIE DOWN STRAPS, WHICH WERE HOLDING WELDING MACHINE TO A PILOT. WHEN HE WENT TO DISMOUNT FROM THE VEHICLE, HE DID NOT LET GO OF THE METAL STRAP. HE CUT HIS RIGHT HANDRING FINGER AND HAD TO HAVE 5 STITCHES.

WHILE CLEANING OUT BUILT UP MATERIAL WITH A HOSE AROUND TAIL PULLEY INSIDE TUNNEL EMPLOYEE REACHED ACROSS BELT TO SPRAY OUT AREA WHEN HIS LOOSE SHIRTAIL BECAME ENTANGLED IN UNGUARDED ROTATING SHAFT HE REACHED DOWN TO PULL SHIRT FREE & AS HEDID HAND SLIPPED & FINGER WAS CAUGHT BETWEEN PLATE ON END OF SHAFT & BEARING HOUSING DISLOCATING FINGER

THE INJURED PERSON WAS AN ELECTRICIAN HIRED TO RUN A GROUND FAULT TEST. WHILE RUNNING THE TEST AT THE SCALPING SCREEN PLATFORM, HE SAT DOWN ON THE HAND RAIL. WHEN THE WELD BROKE, HE LOST HIS B ALANCE AND FELL TO THE GROUND.

INJ GOT FINGER LACERATED WHEN PIECE OF SCRAP METAL THAT HE & HELPER WERE TRYING TO MVOE WAS DROPPED BY HELPER. HELPER LOST GRIP ON END OF METAL CAUSING PIECE TO FALL. INJ'S FINGER WAS CAUGHT B Y METAL AGAINST STEEL BEAM THAT THEY WERE TRYINGTO GET PIECE OV ER. INJ TAKEN TO HOSPITAL EMERGENCY ROOM WHERE STITCHES WERE APPLIED TO REPAIR WOUND. INJ RETURNED TO WORK SAME DAY.

EMPL REMOVING RADIATOR CAP SO WATER LEVEL COULD BE CHECKED.CAP WAS STUCK SO TURNED TO EXIT OFF BLADE TO GET TOOL SLIPPED P FELL BETWEEN DOZER BLADE AND RADIATOR BRUISING SHIN AND ABRADING RIGH T FOREARM.TAKEN TO HOSPITAL EMERGENCY RM WHERE SCRAPE WAS CLEANED AND ICE PACK APPLIED TO BRUISE.RX ISSUED.

EMPLOYEE WAS PART OF SMALL CREW CLEARING TREES AT STIPPING SITE. EMPLOYEE WAS CUTTING HARDWOOD TREE ABOUT 5" TO 6" ACROSS USING TOP EDGE OF CHAIN SAW. 'E HAD HIS LEFT LEG FORWARD OF HIS BODY AND WAS CUTTING AT APPROCIMATELY A RIGHT ANGLE TO HIS LEG. THE SAW KICKED BACK AS THE TREE BENT AND STRUCK JUST BELOW HIS LEFT KNEE CAUSING A SHALLOW CUT ABOU 3" LONG. DOCTOR CLEANED WOUND,
