The employee went to the company clinic on 11/19/07 and was placed on restricted duty. The employee states he used a sledge hammer to remove a wedge that was stuck on the crusher. As he attempted to hit the wedge, he missed the target and hit his R-thumb.
The employee went to the company doctor on 6/14/2007 and was placed on restricted duty. While driving through the plant yard and attempting to make a call on a company radio, the employee lost awareness and visibility and drove into a stock pile resulting in a contusion/superficial abrasion to R-lower leg.
The employee went to the company clinic on 5/10/07 and was placed on restricted duty. The employee's back was hurting approximately 1 month prior due to cumulative job duties, but it got better. On 5/3/2007 while carrying rollers and a belt scale to the conveyor, he allegedly strained his back.
