Employee slipped and fell.
Employee was walking upstairs when they missed a step and hit their right knee on the step.
Miner attempted to carry 2 pallets downstairs. The pallets got caught on the handrail. Miner lost EE's footing and fell downstairs. The stairs were clean with non- skid diamond plate throughout.
Employee was cutting interior flooring of dump truck installing new ones and cutting to size. The blade slipped out of flooring and cut into left wrist which was holding flooring up to cut flooring easier. 911 was called and employee was transported to hospital where they received stitches in left wrist as well as prescribed 600mg of Motrin and 500mg of Tylenol.

Scrapped right shin on the basket of a manlift
The mechanic was attempting to reinstall a lift cylinder on an articulating dump truck in the parking area at Holliday Rock Jobsite. The strap that was attached to the crane and the cylinder slipped causing the cylinder to fall and strike the mechanic on the right-side causing accident to ribs and right arm.

Employee was lifting a steel plate and twisted their back.
Employee was loading a cutting edge up onto the back of a truck and it cut their pinky finger.

Employee was loosening the bolt for a turn buckle of a motor to release the tension to replace the v-belt. When loosening the bolt, the turn buckle fell from its position crushing the tip of employee's left hand pinky finger crushing it between the turn buckle and the platform.

Miner was using pry bar to free up a chain on a sprocket. When miner pulled back on bar it slipped off the sprocket, hit a steel plate pinching finger between bar and plate.

While cutting a belt with a razor knife. Employee cut his left 2nd finger when the magnet above where he was working pulled at the razor knife, causing the knife to veer from its intended path.
While using the Stairs at Jadair, EE foot slipped from the stair causing his right ankle to twist.
