Was replacing a hydraulic hose when it slipped out of their hand and hit them on the nose.

Employee was cutting liner material with a utility blade, the blade slipped from the cutting path and cut the employee's hand.
Employee was changing screens on the wet shaker when the rail fell on EE's right hand injuring EE's middle finger. Employee went to clinic for treatment for possible fracture. Employee was found to have a BAC of 0.04.

EE lifted the hood to replace a welding rod and a foreign object landed in EE's right eye.

EE was in the employee parking lot after finishing EE's shift and 2 co-workers noticed that EE was acting confused and suddenly fell to the ground. 1 worker took EE's shirt off and put it under EE's head, they noticed EE was going into some sort of seizure so the other worker called 911, the paramedics arrived in a few minutes.

While servicing screens on a dry shaker a 48' x 40" section of deck was removed to gain access to a lower deck, about 4.5 feet below the top deck. This section was located at the bottom of a stairway. EE came down stairway, forgot that the section was removed and fell forward catching himself with his right forearm on the edge of the opening.
