Employee was helping replace the swing bracket on a Backhoe. EE was disassembling the crate that contained the new bracket. They struck a board downward on one side, causing the other side of the board to come up and strike them in the face. Employee's lip laceration was sutured and they were released with restrictions.

While removing a rock from the counter weight pulley the employees foot became caught between the pulley, the belt and a pry bar.
While greasing, employee removed part of a guard. Employee's glove got caught in tail pulley and pulled his hand into a pinch point. Employee received broken bones and lacerations to the left hand.

Employee dropped the screen on top of his hand which was on top of another screen.
Miner was adjusting the fork on the forklift - left hand was smashed on the frame. Improper hand placement resulted in this injury.

Employee was clearing a rock jam on the primary crusher discharge chute. He laid a pry bar he was using on a hand rail and preceded to adjust his body position. As he reached for the pry bar, a rock slid down the chute and struck the pry bar. The bar end struck the employee's upper lip which took four stitches to close the wound.

Employee was standing on a ladder that was leaning into a raised up and blocked up loader bucket. As employee was welding, the ladder slipped. Employee tried to break his fall and struck his left elbow on the loader bucket in the process. Employee continued working for another hour or so before realizing that the injury was going to need to be looked at.

[Late report by employee. Employee did not tell employer and tried to cover this via a chiropractor under his health insurance] Employee alleges that he was taking a short cut to his haul truck and while trying to vault over some rail cars, slipped and contused his tail bone. It was found out just this week that he actually has a hairline fracture of the coccyx region.
EE's feet got tangled up in the drawing table stool legs (causing him to lose his balance). He tried to catch himself by grabbing the back of a nearby desk chair. The chair (which was on casters) shot out from under him. He kept a hold on the chair as he fell to the floor. His right hand ring finger was smashed between the chair back and the floor.
Employee was shoveling on a pad located below the secondary crusher while it was running. Dust got past his safety glasses and into his left eye when he looked up for a moment. His eyeball was scratched which required medical attention.
**Note: This occurred back in January as a "report only" case, but injured received no treatment, so we held off on submitting this report until he actually received treatment on 11/02/05. INJURED SELF WHILE CLIMBING OUT OF PRIMARY CRUSHER, WHEN HIS FOOT SLIPPED. HE FELL AND LANDED ON HIS RIGHT HAND.
Employee was ducking under a section of channel iron in the plant . He rose up prematurely and struck his right shoulder on the channel iron. He suffered a strain and contusion from the incident.

Euclid r-65 haul truck he was backing up along the rail road, track on the side road, when the truck started to slide down the 5' high embankment and the truck overturned. ee did not seek, request or need medical attention until 4/19/04 ee wen to dr office complaining about pain in r. shoulder and neck and ee had worked the whole week his normal job drove truck ground.
