Miner was using a man lift basket to observe feed to cone crusher at a height of 28 feet. The miner was wearing fall protection harness and a retractable lanyard but was not secured/tied off to the man lift basket. The miner was ejected from the man lift basket to the surface below.

Right pinky finger got pinched between the spacer and the housing of the bearing unit. While trying to remove the lower shaft and bearing on a double screw material washer. Received 1 stitch at ER to close up laceration.

The employee was cutting 2" x 2" x 1/4" square tubing for handrail supports and catwalk structure. The employee had cut three pieces 3'6" long. Each stick weighed approximately 19 pounds. The employee grabbed three sticks of the tubing and carried them up the stairs to the catwalk. As EE was walking up the steps EE felt a pain in EE's lower back.

On 07/11/2018 while doing routine shop maintenance the employee realized something was in EE's eye. EE attempted to wash EE's eye out but did not report the incident. On 07/12/2018 EE went to the doctor who removed a piece of dirt from EE's eye. The employee was not wearing EE's safety glasses.

Employee was cutting wires from CB in loader-knife slipped and cut his point finger on left hand.
While working at the main plant in the morning EE and another EE were changing screens. EE had one end of the screen and another EE had the other end. One EE's end slipped and fell on his right foot & went through his boot punctured his foot. On date of injury EE didn't want to go to the clinic & get it checked out.

Employee was helping set a pulley on a belt at main plant. The pulley was being lifted by crane. Pulley gave & hit employee's hand. Broke bone on L/ring finger and cut it.
Employee was standing on truck steps - he slipped & fell on back.

EE was tightening a bolt. Ratchet slipped and hit his left ring finger around the fist joint.

EE was cutting a 2" hose on astec #2; knife slipped and the palm of his left hand.
EE was driving truck on rough ground and continued to do so unitl he felt he could not continue, reported to Plant Mgr at 1245 and EE sat for a 45 min period to see if the pain continued, before reporting to the Palnt Mgr.
The shop foreman was changing a fuel filter on a truck, had trouble getting filter to loosen. He pulled hard and hand slipped off and hit drain pan - cut left hand pinky finger.
EE was hammering a bolt on a gear box at the rewash plant, a piece of the hammer broke off and hit him in the chin, he thought he just cut it and put a bandaid on it. 9-10-08 he reported to another EE (spanish) so he could interpret. EE can feel something in his chin and a magnet can pull on the embedded object.
Setting Slurry Pump on Base Frame with Backhoe & chain. Bolt hole was not lined up & EE tried to push in place. Pump went past edge & tilted . EE's hand was on edge & was caught between pump & backhoe edge. He had on gloves but did not prevent a cut and fracture.

MINER WAS SITTING IN A CHAIR GRINDING ON METAL. PULLED GRINDER AWAY TO CHECK WORK AND REST ARMS, GOT GRINDER TOO CLOSE TO KNEE WHILE PAYING ATTENTION TO WORK.
