Lost balance put left hand down to catch himself from falling and cut the top of left fingers.
While welding in a discharge chute, weld slag caught his left pant leg on fire. Causing a burn on his left calf.
Climbing up the ladder to cab on the Terex T-40 haul truck, missed a step while climbing and slid down the ladder to the ground. Was about 1-2 feet up when he slid down the ladder.

EE Was in the process of removing wedges from the upper liner (Gyro Crusher). During the process while swinging a sledge hammer he struck a handle of another hammer. The hammer became airborne striking him in the head.

While mucking with loader hit a pot hole and bounced in seat, when seat bottomed out felt pop in lower back. No missed work until 7/25/12
Cleaning up completed work area. Conveyor belt clamp standing on end slid down handrail and struck left hand pinching the smallest finger between the clamp and stand.
while prying screen panel loose to move it for lifting, pry bar slipped out of his hands while under tension striking his right elbow.
While Surveying a heading employee tripped on a rock on the road and fell striking his right hand on a rock, cutting his hand through his gloves

Employee was cutting belt with bad splice with left hand due to his Right hand being broken (none work injury). Knife slipped while trimming belt and struck him in his right leg causing a deep cut
While moving rock from stope to crusher staging area a rock fell out of the bucket. On returning he did not see the rock and ran over it causing him to loose control and hit the rib with his bucket. The jarring motion caused his leg to hit the wall of the cab.

While moving the Cat excavator 315 across a small ditch the rocking motion of the machine caused employee to experience a back spasm not allowing him to move.
While stepping from excavator, when his left foot went in a hole, knee popped causing him to fall on his right knee.

EE was hand scaling from manlift on GM Topkick powder truck. When he went to reposition, a piece of rock approx the size of a soccer ball/plate came loose falling approx 8' - 10' and clipped the back of his hard hat. EE had a sore head/neck and was taken to the Medical Center to get checked out. He was treated with cold compress.
During course of shift, EE alleges to have twisted his back while running a loader. He cannot recall a particular event that caused the injury.

EMPLOYEE ALLEGES TO BE SUFFERING FROM CARPAL TUNNEL SYNDROME IN BOTH HIS WRIST. HE NORMALLY RUNS A DRILL AND PERFORMS HAND AND EQUIPMENT SCALING, EMPLOYEE CLAIMS HIS HANDS HAVE BEEN BOTHERING HIM SINCE LAST SUMMER, HE FINALLY WENT TO A DOCTOR IN JAN.
