While troubleshooting an electrical issue on the drum miner the employee inserted their hand into an electrical cabinet making contact with an energized conductor and receiving a shock. EE was given a prescription for 800 mg of ibuprofen 7-day antibiotic and Silvadene cream and antibiotic for the wounds. Restrictions - Lt Hand work only.

Once given the "ALL CLEAR" to resume work activities, employee began to gather materials to return to work. A bolt of lightning struck very close to them. Employee fell to the ground. Foreman ran to employee's aid.
Electrician was troubleshooting the limit switch circuit on #8 control sled on #20 dragline pumping system. Electrician entered the main disconnect cabinet to trace the wiring for the circuit. While standing on 4 ft step ladder, electrician reached toward back of cabinet to twist the label on the wiring for the limit switch to read it & experienced electrical shock.
Injured employee ground checked junction box. Used bang stick arc occurred ground fault did not trip.
While removing an isolation shutter from a spare cubicle an arc flash occurred injuring a plant electrician.
EE was called to #2 MLP on the 26 side to reverse motor for maintenance to remove impeller. After he changed motor back he closed the motor disconnect and an arc fault occurred inside the switchgear cabinet. The arc blast blew the door open. EE was standing away from the door but received minor burns.
The employee was in the process of replacing the radio for the Clay system. He had opened the radio box on the P2B Dam - North side when lightning struck in the vicinity of the employee. The employee fell to his knees.
Emp. was changing a battery on a battery car. Emp. was nipping in car cable to #2 battery to put in use and emp. was shocked. Emp. went to ER and was given prescription medication, no restrictions.
Work being performed on desliming screens. Feed valves to screens closed for repairs. When lang brine started, lang brine ran over onto mechanics working in screen. Energized welding lead got wet. Emp. assisting to get another emp. out of screen and received electric shock.
While attempting to tighten an energized 440 volt starter, connection made contact with a ground within a starter cubicle chassis. Starter was not locked out or otherwise secured. Employee was wearing an electrical flash shirt which greatly reduced potential injuries.
Electrician was reversing motor for mechanic to remove impeller. Impeller was removed & electrician went back to reverse motor leads when his left arm came in contact with line side 4160V contactor. (Knife blades) Three phases on disconnect were still in closed position.

EE HAD LOCKED OUT POWER CENTER AND DISCONNECTED PUMP CABLE. CABLE LEADS WERE LYING ON DECK OF POWER CENTER. WORKERS LIFTED PUMP WITH FORKLIFT AND STRUCK OVERHEAD POWER LINE. POWER WENT THROUGH CABLE AND ARCED ON DECK OF POWER CENTER, BURNING ELECTRICIAN'S HAND. GIVEN RX MEDICINE.
MOTOR BREAKER HAD TO BE CHANGED OUT, BURNING CONNECTION LOADSIDE OF BREAKER. 800 AM MAIN BREAKER FEEDING LOAD CENTER WAS TURNED OFF. TOPE SIDE OF BREAKER WAS CHECKED FOR VOLTAGE WITH TIC TRACE R AND VOM, NO VOLTAGE PRESENT. PROCEEDED TO CHANGE BREAKER AND 2 WIURES TAKEN OFF LINE SIDE OF BREAKER TOUCHED STARTER CAN, FLASH BURNING CAUSING 2NE DEGREE BURNS TO TOP OF LEFT HAND.
EE REVERSED THE 5B TAILS MOTOR. WHEN HE CLOSED THE MAIN DISCONNECT FOR THE PUMP MOTOR, IT MALFUNCTIONED. HE PULLED THE MAIN DISCS ON TOP OF THECAR AND THOUGHT HE HAD THE MAIN POWER OFF TO REP AIR SWITHC. THE DISCONNECTS ON TOP OF THE CAR ARE IN THE CIRCUIT ONLY WHEN FED FROM AN OVERHEAD POWER LINE. EE INSERTED WRENCH TO ALIGN DISCONNECT KNIFE BLADE & ELECTICAL ARC OCCURRED, BURNING
CHANGING BATTERY IN BACKHOE, BATTERY BLEW UP IN FACE.
EMPLOYEE RECEIVED A SERIOUS ELECTRICAL SHOCK WHEN HE CAME IN CONTACT WITH A METAL WATER PIPE THAT HAD BECOME ENERGIZED BY AN UNINTENTIONAL GROUND FAULT.
EMPLOYEE CUT POWER CABLE WHICH WAS LIVE. RECEIVED ELECTRICAL SHOCK AND BURNS.
EMKPLOYEE WAS CHECKING FOR LOOSE CONNECTIONS ON THE 480V/100 AMP BREAKER. DURING THIS PROCESS HE STUCK AN ALLEN WRENCH ON THE PRIMARY SIDE OF THE BREAKER. THIS CAUSED AN EXPLOSION AND PHASE TO PHASE CONDITION.
EMPLOYEE INADVERTENTLY CAME IN CONTACT WITH ENERGIZED ELECTRICAL PARTS AT FUSIBLE DISCONNECT.
EMPLOYEE WAS CONNECTING A CAT CH PAN & HOSES TO A SODA ASH RAIL CAR IN ORDER TO OFF LOAD THE SODA ASH INTO A S TORAGE TANK THE CONNECTION POINT FOR THE PAN & HOSES ARE CLOSE TO THE GROUND & DI FFICULT TO CONNECT TO
SHE RECEIVED AN ELECTRIC SHOCK FROM A STARTER PUSH BUTTON WHEN A MOTOR SHE WAS TRYING TO START SHORTED TO GROUND. THE MOTOR FAILURE CAUSED AN ELECTRICAL FLASH.
EMPLOYEE WAS STARTING W. WATER JACK, SUFFERED ELECTRICAL FLASH AND FORCE KNOCKED EMP. BACK INTO HIS TRUCK CAUSING CONTUSION TO LEFT SHOULDER.
