Employee was gathering up surveying equipment and placing along the rib in the 7th North Development section. Moved surveying instrument and placed it along the rib when a piece of coal fell from the rib and struck the backside of EE's left hand requiring stitches.

The injured employee had just finished making repairs to a valve bracket on shield #8 on the 8th South longwall face. The shear was located at shield #40 cutting a taper towards the head gate. There was a bounce on the face. The injured employee was knocked down and hit EE's head on the valve bracket. 3 stitches were required.

Employee was installing a glue stick and bolt into the rib when the rib broke loose and struck the employee on the right leg.

Employee was in the 7 West Section rib pinning, when a rock fell from the corner of the rib line, striking employee on the small finger of right hand. The injury resulted in 8 stitches from a laceration. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was hang the miner cable along the rib in the last open crosscut in the 1st North development section when a piece of coal measuring 18 inches long by 18 inches wide by 8 inches thick fell from the rib and struck ee in the left knee.

EE was hanging monorail between the longwall stageloader & ribline, just outby the longwall face. A piece of the rib suddenly popped loose above a rib bolt and fell, hitting EE on the back of left leg, knocking them against the stageloader. EE suffered contusions and back spasms. EE was wearing required PPE & has training & experience.EE didn't violate any rules or regulations.

A fully trained miner was picking up block when a piece of the rib rolled out and landed on EE's left wrist. The injury was not the result of any compliance issues nor were any protective guards involved.

Injured employee was checking the charge level on scoop batteries. As EE was checking the batteries, the rib rolled out behind EE striking EE in the back and shoving EE into the scoop body and down to the mine floor. Employee has contusions and a right shoulder strain/sprain. Employee has proper training & experience and didn't violate any rules or regulations.

Rib bolting on tube side of miner in 6E 20 blk 3-2 crosscut. Bit portion of two piece rib drill steel got stuck in drill hole while extracting steels. While retrieving the drill steel a portion of rib (3'x4'x2-5" thick) dislodged and pushed ee into the miner e-stop, resulting in a bruised tail bone.

While acting as a helper to a rib bolter on the longwall face, a piece of the face sloughed off, striking employee on the left side, causing contusions to the lower body.

A fully trained miner injured left ankle when shoveling belt. A coal rib rolled out striking the ankle. The injury was not the result of any compliance issues nor were any protective guards involved.

While operating the longwall shearer, there was a small coal outburst and small coal particles hit employee in the face and left eye. Injured employees eyes were flushed and EE finished EE's shift. Employee followed up with a doctor who removed a small piece of coal from EE's eye. Employee was wearing safety glasses and has proper training. No rules or regulations were violated.

A fully trained miner was installing a rib strap when a piece of rock fell from between the roof straps striking right shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

The individual was shoveling under the belt, with their back to the rib, when approximately 1' to 3' of rib rolled out. This covered their feet and lower back.

While operating the headgate shear drum and cutting the bottom from tailgate to headgate, a piece of coal measuring 20"X8"X9" came off of the face and hit the employee in the right leg resulting in a contusion of the right thigh. Employee was examined at the clinic and released to work with no restrictions. The operator voluntarily placed EE in light duty work for 4 days.

A fully trained miner injured their head and neck while working on a miner. A piece of rock fell from the rib that was rib bolted per regulation, striking their hard hat and neck. The injury was not the result of any compliance issues nor were any protective guards involved.

IE was digging bottom prepping for installation of steel set supplemental roof supports when piece of rock dislodged from rib and struck injured in the left foot and leg

Employee was installing a 6' bolt on the 4E longwall face under the mesh when a rock rolled out from the binder area striking employee's right middle finger resulting in a laceration. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

While cleaning out area to change a dog bone on the longwall face and waiting for supplies, a piece of the face rolled out pinning the employees foot against the pan. This did not involve compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.

A fully trained miner injured their back when loading material onto the belt a piece of rock fell from the coal rib striking miner in the lower back. This was not a result of any compliance issues nor were any protective guards involved.

A fully trained miner was assisting move the fan from 3 to 2 entry when the bologna skin became caught on corner. As EE was freeing the caught piece, a rock fell from between the bolted rib straps and hit left foot. The injury was not a result of any compliance issues nor was any protective devices involved.

Rib rolled out between two straps hitting the individual in the upper Rt. thigh.

Stepped out of Loader to re-position cable hanger and while re-hanging cable in hanger a piece of rib rolled out and struck employee on foot. This did not involve any compliance issues, no equipment involved, and it did not involve training or attitude.

The employee was struck by a section of coal rib when EE squatted down in front of it. The incident occurred because the employee did not view this rib as a potential issue. The employee was properly trained and has a good safety attitude. The incident did not involve any mining equipment or any aspect of the mining system. EE had EE's proper PPE.

a fully trained miner was putting fire suppression pipe together at the take up . The employee was bent over facing the rib and a piece of stone 6"x2"x1" fell from the rib striking him in the neck and shoulder area. The employee never lost consciousness and PPE was not a factor.

Walking by miner in 1 East #3 heading inby 7 block and a piece of loose rib (2' by 8-12") fell and struck him in the head and neck. X-rays were negative, given time off for possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.

Employee was in the process of hanging a shuttle car cable on the rib. The rib rashed out causing laceration to his head and chin. Lost time as of 06/29/2015 laceration to chin and head. Compliance with rules and regulations was not a factor. Miners proficiency and job skills may have been a factor.

A trained longwall shearer operator was drilling the coal bottom floor in the 12 East longwall. The victim was struck by the rib falling and striking him in the knee. The roof control and protective equipment were compliant.

A trained employee was shoveling on the longwall face to clear coal bottom away. He had his back to the face. The roof from the shield tip to the face was roof bolted. The victim was struck on the left shoulder by stone and rock. The roof control and protective equipment were compliant.

Trained foreman was standing near #4 shield on the longwall face in 12 East supervising employees in clean up work. He was between the face and panline. The roof was roof bolted. A rib fell off the face hitting his knee. The roof control and protective equipment were compliant.

Individual was rib pinning off the Alpine Miner when a rib rolled striking him on the right shoulder and leg.(sprain to right shoulder) This did not involve any compliance issues, no equipment involved. This did not involve proficiency or attitude. All personnel safety items were being utilized. Individual continued to work until 3/02/2015, removed from work by doctor.

Employee was installing bolt next to rib. Vibration of drill caused rock to separate and fall striking left lower arm resulting in a small laceration. Cause of the injury was body placement. PPE was not a factor, compliance rules/regs was a factor, mining equipment/systems was a factor, miner proficiency may have been a factor.

The accident did not involve: rules or regulations, mine equipment or mining systems, job skills, miner proficiency, training or attitude, or protective clothing, devices on equipment. The employee had a section of stone slide into his back as while cleaning other loose material from a gob pile. the employee suffered a contusion to his middle back as a result of the incident.

Injured was hanging feeder cable in the belt entry, bent over to grab feeder cable a rock fell out between rib line and roof screen striking him in the lower neck and shoulder. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.

Employee was pulling the roof bolter through the curtain when the ATR caught on the curtain. Employee got off the bolter to remove the curtain when a rock on the rib fell and his employee.Lost time injury

Employee was tending the miner cable and as the miner was backing up, he bent over to grab the cable to move it, when the face expelled a lump of coal that struck his thumb on the left hand resulting in an open fracture of the left thumb. 6 sutures were required to close the wound.

Employee was advancing a shield at mid face when there was an outburst of coal from the face area and the employee was struck in the left elbow and left shoulder by coal which was expelled from the face. 2 stitches were required to close the laceration on the left elbow.

Employee was changing a damaged hose on shield #61 on the idle face when there was a bounce/outburst and the employee was hit in the back by a piece of coal that was expelled from the face.Emplyoee sustained 1 broken rib and 3 cracked ribs on the left side of his rib cage.

Employee was standing near the rib when the rib rolled out and struck employee's right lower leg. Lost time injury.

Employee was standing along side of a shuttle car looking into the cable reel compartment of the car when the rib sloughed off and the employee was hit in the back of the right knee by a piece of coal.

Employee was walking down the feet between the shields and the panline when rock slid out and struck him. Restricted duty injury.

The employee was operating the right side drill head of the Fletcher roof bolter in the rock room of 15th East when the pillar yielded and the employee was struck in the mouth with a small piece of coal, resulting in a small cut to his upper lip, which required 3 stitches.

The injured employee was assisting another employee to hang a 5 hp flight face pump from the roof screens along the rib line in preparation for mining in the face. As the pump was hung from the edge of the screen, the screen sagged down away from the rib and a piece of coal sloughed off from the top of the rib and struck him in the right ankle.

As EE was shoveling the mine floor between the RA and the stopping, a piece of coal fell from the rib and struck him on the left shoulder.l

EE was shoveling the ribs in the face as part of the clean up cycle, when a piece of rock sloughed off the rib near the roof and struck the EE in the right ankle.

As EE was loading bolting supplies into the scoop bucket, the rib sloughed off and knocked out a rib timber and the timber struck the EE in the right shoulder. Thus fracturing his collar bone.

EE was tending miner cable in the #1 entry of 11th west, when a piece of the rib was dislodged and struck the EE on the right side of his body and knocked him into the shuttle car with the left side of his body.

Employee was troubleshooting C.O. system when a rock measuring 8"x30"x8' rolled off the rib striking him along the left side.
