A miner entered a surge bin to free stuck material. The material collapsed and engulfed the miner.
Donning fall protection equipment, the miner entered the top of a primary feed hopper without the knowledge of management and contrary to mine safety procedures to remove a large rock. While trying to break up the rock, raw material that remained on the sides of the hopper sloughed off engulfing the miner.
Employee was engulfed when they entered the primary feed hopper/bin to clear clogged material.
Four employees were installing a steel plate. The plate was 20' wide x 8' high x 1" thick. A small loader put the forks against the plate 5 feet up. The operator re-position the forks 3 feet lower so employee 1 could stand on forks and get a better reach. The plate became unstable and fell on top of employee 1, pinning the employee.
Employee was installing a canopy on the shuttle car and the canopy fell on him striking him in the head and chest.
A truck driver was delivering a load of lumber to the mine site. While the fork truck operator was unloading the truck, the truck driver walked around the other side of the truck when the fork truck operator accidentally pushed over a bundle of lumber, crushing the truck driver fatally.
Two men were working atop an 80 ft silo attempting to reset the chain on a bucket elevator. One man was below and in phone contact with one of the two mechanics atop. As the call was ending, the assisting mechanic atop knocked a 'miner's bar' off the platform. The bar fell and hit the man below on the back of the head. The man died approx. three hours later in the hospital.
Employee was retrieving pry bar from storage area on the ground, when personnel above him lost control of new return roller they were changing out. The return roller fell, striking the employee on the ground. While the employee was wearing proper safety equipment at the time, the impact of the roller resulted in his death.
Employee was cutting a metal band on a bundle of rib screen. When the band broke, the screen expanded and fell on his chest, pinning him between the screen and the Wagner scoop. The screen was lifted off the victim with a skid steer and no pulse was found. The victim was transported to hospital.
WHILE ON THE GROUND SPOTTING FOR THE MAINTENANCECREW HE WAS STRUCK ON THE HEAD BY A DUST COLLECTOR BAG FULL OF LIME. EMPLOYEE WAS PRONOUNCED DEAD AT THE SCENE BY THE CORONER.
