He went to the ER after work requesting they remove his fingernail for comfort. He was prescribed pain medication for comfort and antibiotics for preventive reasons.

Employee was performing maintenance at Jaw Crusher and was using a pocket knife to cut a plastic tie strap, when the knife cut through the strap and cut the top of his left hand through his glove. Employee received medical treatment consisting of 2 stitches and a tetanus shot. No restrictions.
Employee was working under the hood of the Cat 988F loader when the lower support bolt that holds the safety support cylinder sheared off allowing the hood to fall striking employee in the back. Injury is bruise on back and is currently on restricted duty.
EE & co-worker were replacing & repairing track groups on the 330 excavator. In an attempt to align the lock slots with the slip fit pins to join the tracks together, EE tapped on the pin with a sledge hammer. The pins did not move. EE took a more forceful swing, the hit glanced off the pin toward the track pad where his hand was caught between the pad & the sledge handle.
The Counterweight did not line up with the saddle, a come-along was attached to the frame on the conveyor. He took a closer look to see what it was hanging up on and in the process put his hand on the frame of the take-up system, the weight shifted catching the tip of the middle and ring finger on his right hand between the frame and the weight.
Striking the hammer against the chisel cause a small piece of the hammer face to came off and went into index finger on left hand.
He was attempting to dislodge a rock in the winged tail pulley with a long piece of flat bar. He pulled down on the bar, the end of the bar rolled out of the tail area, the flat bar smashed his left index (pointing) finger resulting in a broken bone, 5 stitches. He was wearing gloves on his hands at the time.

Employee was removing driveshaft from 972 loader. Employee was striking driveshaft T with hammer to remove from yoke. At the area of impact on the bearing cap a piece of metal broke free from the cap and punctured right calf of employee. Size of metal was approximately eigth of an inch in diameter.
3 EE's were assigned the task of streching out 500' of #8 SO cord to be used to install an electrical pump. When the proper length was reached, the cord required cutting. One of the EE's went to get wire cutting shears. One of the other EE's had the injuried EE hold the cord and and the other EE cut through the wire with a box knife and cut the injuried EE.
While puling on a bar to line up an I-Beam, employee felt a strain in his right shoulder.
Employee was splicing the wire for the installation of a replacement light fixture on the #2 Hazemag crusher, as he was cutting off the outer cover of the cable the knife slipped and cut his thumb.

Employee was changing a bearing on the stedman with a portapower jack and while pushing off the bearing the ram popped out and hit him in the jaw area. NOTE: (Employee did not seek any treatment at the time due to mild pain and did not seek medical treatment until 8-29-2005: and was then taken to workers Compensation for treatment)
Employee was chipping slag from a weld with a hammer. A piece of the slag penetrated his left wrist. He visited the doctor and the incident was initially treated as a first aid. Upon returning to the doctor (8/17/05) for a follow up visit, the doctor recommended the piece of metal be removed. The metal was removed on 8/19/05 and a stitch was placed over the wound.
Moving a cable tree when the chain used to move the tree slipped and pinched the employee's finger.
Employee was clearing a plug on the FK Pump for #3 finish mill. He had cleaned out the pump and screw and was working on the flapper when cement that was in the line to the silo came back down the pipe and hit him. It knocked off his hat and glasses and got into both eyes. He wasn't wearing goggles as required.
Employee was helping finish up conveyor belt change-out. While using a utility knife, the knife slipped cutting his left index finger, requiring sutures.

Employee was removing a bearing sleeve from a shaft with a hammer and a shard of metal broke off and cut the employees left arm.
EE WAS CUTTING A CONVEYOR BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT HIS RIGHT HAND BELOW THE THUMB. THE LACERATION REQUIRED THREE STITCHES TO CLOSE.
Dump truck driver was assigned to deliver a solo load (truck only) & needed to leave his trailer at the Mine site. When jacking up his trailer, the driver needed to use excessive force on the crank handle to raise the trailer. With both hands on the crank handle, left hand over right hand, his right ring finger was fractured on the middle segment.
EE STRUCK HIMSELF IN THE MOUTH WITH A WRENCH WHILE PERFORMING MAINTENANCE REPAIRS.
EE WAS ADJUSTING THE FEED END AIR SEAL ON KILN USING A PORTAPOWER. THE SEAL SHIFTED QUICKLY AND DISLODGED SOME KILN DUST THAT ENTERED THROUGH THE GOGGLES AND INTO HIS EYES.
EE WAS MOVING HAMMERS IN THE STEDMAN WITH A PRY BAR WHEN THE HAMMER JUMPED OVER THE RAIL. EE GRABBED THE HAMMER TO KEEP IT FROM FALLING OFF THE TABLE AND STRAINED HIS BACK.
STRUCK AGAINST CONVEYOR FRAME WHEN BAR HE WAS USING TO DISLODGE A ROCK FROM ROLLER WAS CAUGHT IN CONVEYOR. NO MEDICAL TREATMENT UNTIL 9/8/03.
EE WAS HELPING INSTALL A PIPE UNDER A CLASSIFIER SYSTEM WHILE USING A CROWBAR TO APPLY PRESSURE TO KEEP PIPE IN SLOT SO IT COULD BE CLAMPED, WHEN HE PULLED HIS LUMBAR MUSCLE.
