Employee was washing off scale with water hose. She lost her footing and fell backwards landing on her left hand and buttocks. She suffered a sprain to her left wrist.

Employee was walking along the peak of the excavated stockpile in order to measure heights and calculate volume of the pile for production. Employee lost his balance when material gave way underneath him while climbing. Employee rolled down the entire face of the stockpile approximately 90 feet before coming to rest.
EE was changing screens on the plant and strained his back.
The employee decided to assist with a crane load which was beyond his normal scope of duties. He was walking backwards with a tagline when he stepped off of the structure contacting a lower level.

While shoveling a pile of stemming, he hit a large rock in pile & felt sharp pain in shoulder. Shoulder strain
A piece of rebar struck the employee (Reinforced Ironworker Helper) in the shin. The resulting abrasion became infected.
Two EE's were placing 6" PVC pipe at a tire wash. After gluing into place, they were back filling a Bobcat. As EE swung the backhoe for more material, injured EE reached down and pulled up on the pipe, the bucket of the backhoe passed, caught the tip of EE's right index finger between it and the end of the PV pipe, cutting a piece of skin.
EE misjudged distance from last step on loader to the ground.
Employee was on a work platform beating tank pins down tight. After completing the task he pivoted and felt a pain in his right knee. When he stepped down to the ground, he felt his knee "pop".
Mechanic was pulling on prybar when bar slipped hitting him in the mouth.
EE was bit by a spider while shoveling in the conveyor tunnel.
Employee had his rt. foot caught in/between a winch drum and the floor plate, resulting in the steel toe on his boot being smashed. There were exposed bolts on the winch drum that pinched his foot and resulting in a cut/laceration of the rt. big toe.
Employee was holding a form in place when it slipped out of his grip and fell into injured's hand. Improper method for setting forms in place was employed.
Employee was working with a shovel cleaning around the jaw crusher. After completing an 8 hour shift, he felt some discomfort in his lower back. The employee drove home and felt spasms in his lower back. He could not exit his vehicle without help. He then contacted his supervisor and was taken to the hospital.
Tripped over welding curtain and lost footing and twisted knee/ankle.
Employee tried to maneuver bearing around the frame of tailpulley. His right ring finger got caught and cut underneath bearing on tailpulley.
Employee was trying to obtain a sample from the clinker conveyor belt. While getting the sample, the sampling scoop got stuck between the guard and the conveyor. The motion of the conveyor pulled the scoop upwards smashing the employee's finger against the guard.
Employee was lifting an 80 pound wear liner into crusher. During the lift he felt a pop in his left shoulder and experienced pain. Employee was taken to the doctor the next day and treated for muscle strain.
Employee was exiting Mack Stock Pile truck and missed the bottom step on the fuel tank. He fell and hit his tail bone causing a bruise.
Employee was welding a plate in the Jaw feeder when a rock fell approximately 2 feet from the side of the feeder and hit or rolled on his middle finger on his left hand. Employee injury resulted in 4 sutures to his finger.
EE was removing a wire choker from a section of equipment. The wire choker fell off the equipment and struck him on the head, resulting in two lacerations to the head. He was treated in the ER and Trauma Center for wound closure and observation.
EE was assigned to replace a bearing on the lower bend pulley (BP) of Conveyor 22B. Crouching on the catwalk with his back to the midrail he attempted to remove the bearing cover by pulling on it, at which time it slipped from his hands causing him to fall backwards against the midrail. The midrail broke and he fell aproximately 12 ft landing on his left shoulder.
Employee was welding -- flashburn to the eyes.
Employee was working on shaker screen. He had lifted and secured a discharge lip on the top deck of the screen so that he could work on the second deck of the screen. After climbing into begin work on the screen a gust of wind caused the discharge lip above him to fall. The lip struck the employee on the back and caused him to hit his chest on the deck in front of him.
The employee was replacing bolts on the cone screen wear plate. He was using a 1/2" ratchet with extension lying on the catwalk to access the bolt on the bottom side. The socket slipped off the bolt and the ratchet handle hit him in the nose resulting in a contusion.
Employee was operating the Cat Skid Steer loader when he swerved off the roadway and hit a small drainage ditch to go by an oncoming Cat 980G loader, that resulted in a lower back injury. Employee received cortisone shots, steroid packs, pain pills and was put on restricted duty pending his follow up visit by the physician.
While installing a radiator on a 972 loader, jammed his finger while handling the radiator.
Employee fell from loader step striking head. ***Safety Manager notified our local MSHA Office in Las Vegas, NV within the designated time frame.
Employee was opening a box with a box cutter. As the employee turned the box around, on the third turn, he accidently lost control of knife as it came towards his left arm. The knife lacerated the left wrist and at the same time puncturing an artery.
While in the process of water blasting the kiln, the preheater feed chute material was being water blasted & expelled back toward the employee & entered the protective hood. He gasped & inhaled (hot)dust into his mouth causing mouth burns.
Employee was moving an aparatus from a top shelf in a storage container. An arm of the aparatus swung around and hit him in the face fracturing his nose
Employee was driving a T-post stake into the ground with a sledge hammer when he struck his right ring finger resulting in a laceration. He went to the ER after work requesting they remove his fingernail for comfort. He was prescribed pain medication for comfort and antibiotics for preventive reasons.
Employee was washing windshield on loader and fell off.
Employee was replacing vertical shaft with finger between pry-bar and shaft when the shaft slipped and pinched finger.
Employee was dislodging material stuck in a discharge chute when his right hand got pinched between the shaker deck and the chute. Medical treatment consisted of a rigid splint and pain pills. He was on light duty for 2 days and no follow up visits were required. His injury was a bruise to the top of his right hand.
During the process of replacing a backstop on conveyor 3, both employee & co-worker began to prep the motor. EE had not placed his safety harness on while on the catwalk. His co-worker loosened the bolt & took the nut off, at that instance the bolt shot out, the reducer rotated forward and struck EE throwing him off the catwalk 10 ft into the chute.
While replacing screens, one fell and landed on his foot.
While inspecting a expansion joint on a kiln exhaust duct. Hot dust came through fabric and contacted right hand forearm causing 1st degree burns to right arm & top of right hand and neck.
Employee stated she was having pain in her right shoulder and left wrist. She states the pain is from years of repeated motion due to operating a wheel loader.
EE was walking with a 5 lb. GPS rover pole in his hand. He looked at the GPS to give directions. He was walking on fallen trees. The bark was smooth, wet and slippery. His foot slipped off and rested on soft and uneven ground 18" to 24" below, causing him to sustain a sprain of his left ankle. This Report was filed on 11/20/06, I called MSHA to inform internet problem.
Employee's left index finger was smashed between jib-boom and platform on drill rig.
Technican attempting to cut wiring harness with knife. Knife slipped and cut finger.
Employee was performing maintenance at Jaw Crusher and was using a pocket knife to cut a plastic tie strap, when the knife cut through the strap and cut the top of his left hand through his glove. Employee received medical treatment consisting of 2 stitches and a tetanus shot. No restrictions.
