The final cut had been made on the 96" conveyor belt in prep for vulcanization. Employee stood up, remaining in a bent over position in the 42" space. While he was backing out from under the belt he inadvertently pressed the blade of his razor knife into his thigh causing a puncture wound about 2 cm long through the epidermis, stopping barely into the dermis (2 sutures).
He was changing bits on the continuous miner when the wrench slipped and he struck his left thumb against the wrench causing a laceration requiring 8 sutures.
Tightening roller bolts on a D11R with a ratchet and prybar, felt a pop in right forearm.

While jacking up the jack it kicked out and stricking him on both knees and legs. Bar hit him in the chest.
Hitting a piece of cold roll with sledge hammer, he missed the cold roll, hitting another piece of metal which caused the sledge hammer to "twist", which twisted the right wrist, causing injury.
The injured was cutting a piece of rock dust hose with a pocket knife. He was cutting with his right hand when the knife slipped off the hose and cut his left hand causing a laceration at the third knuckle of his left hand. This is not a lost time accident. This is a MSHA reportable accident because the injured received 6 sutures.
Employee was cutting a band on a stack of pizza pans with a hammer. The band broke and came back hitting him in the right forearm causing a cut that required 3 stitches.
EE was changing a bad top roller on 35 brk of #3 belt. He hit the structure with hammer in attempt to loosen clip. A piece of metal dislodged from structure & flew into left eye. He filled out an acc. report & sought med. attention to have eye flushed. It was discovered that the metal was still in his eye & 2 wks recovery was needed till he could return to work.
While repairing a set of airlock doors, just inside drift mouth, employee was making a hole in the door with pick hammer to mount rubber seal. The tip of hammer broke off, impaling into lower arm. EMT dressed arm.
The employee was loading supplies on the left side of the roof bolter, when he cut the bands on the bolts the bands came back and hit him on the left forearm cutting it.
Using a chisel as a spacer in a chain breaker while re-assembling a continuous miner conveyor chain. He had his hand on top of the chisel to remove it and the chain jumped causing the end of the chisel to strike against his right hand between his thumb and index finger. The laceration needed five sutures to close. No time from work was lost. This is a medical treatment incident.

Victim was assisting workers who were re-railing an off the track 9 ton motor. He was placing wood blocking material under the locomotive when the jack that was set under it kicked out and the motor dropped. His right forearm became caught under the motor resulting in a severe contusion.
He was using a sledge hammer to knock out the blocks that remained in a stopping after a scoop had been used to knock the stopping out when several blocks fell from the corner and struck him on the top of his right foot, fracturing it.

EE was using a slate bar to knock down rock. When rock fell, it hit slate bar, knocking the bar out of his hand. He then felt something pop in his shoulder.
While trying to disassemble an air pump, the injured was holding the pump steady with his right foot and turning a wrench to loosen pump part. When the injured jerked the wrench he felt pain in his groin. The injured was diangosed with a hernia. He continued to work until 7/28/07 the date of his surgery. Because the mine was on vacation his first day misseed was 8/6/07.
Technician was replacing bucket teeth on a 992G. Technician was using a sledge hammer on the pin driver when a small piece broke off the end of the pin driver and lodged in his right hand between the knuckles of his first and middle finger.
Employee was moving a beam with a rock bar and beam landed on foot.
Injured was working on razor knife, cut his left thumb.
Unit #1 - While roof bolting, employee cut band on a new bundle of bolts. Band flew back, striking left upper arm causing a laceration approximately 1 inch long requiring 3 sutures. MEDICAL TREATMENT ONLY. NO LOST TIME.
Using a slate bar to close the door on a ballast car and the bar slipped. He strained his lower back. Injured continued to work until 3/19/07 at which time he began losing time for a lower back strain.

(told this to mine supervisor) He has not called us back, we've left message with mother. (Reason report late.)
(LTA) : Slope Track - Employee was jacking low boy on track in slope. The jack kicked out striking lower left leg resulting in open compound fractures requiring surgery.
Injured employee was holding a piece of channel while another employee was striking it with a sledge hammer. Struck right ring finger with sledge hammer.
Employee was cutting plastic water line, knife slipped and cut left thumb.
He was using a knife to remove a zip tie when the knife slipped causing a laceration to the left index finger requiring 5 sutures.
Unit # 1 - Employee was at the back of the tail on 55 miner prying conveyor chain backward with a slate bar trying to get slack on the chain. The bar slipped out of the chain, throwing employee downward, overextended left knee causing torn medial meniscus resulting in surgery.
Removing bits from shearer cutting drum and a piece of the bit tool removal tool broke off and struck him in the left cheek, becoming imbedded. The piece was removed and one suture was necessary to close the wound. The employee did not miss any scheduled work due to the injury.
