Employee was at the tenth section of staves on the silo. An existing structure did not allow the employee full access to the scaffolding. While the employee was installing bolts on the chime, he overshot his swing causing him to lose his balance and fall forward. When retracting back, he caught his arm on the corner of the stave lacerating his left forearm.

Mostrando 50 de 166The ee was using a 4-inch flat bar on a conveyor belt as a straight edge for cutting. He started cutting the belt away from his body using a utility knife. While maintaining a grip on the knife, The force of pushing the knife and angle of the blade made the knife come of the cut, glance off of the 4 inch flat bar and made a 3.5cm laceration on the left index finger.
Employee was using a 6-ton come-along trying to move an I-beam into position as he torqued the come-along the pad eye came loose & flew back striking him on the left leg shin gashing his leg open resulting in 6 stitches to repair.
While employee was performing conveyor belt replacement, employee cut through belt with utility knife, cutting leg.
The miner was installing screens on the bottom deck of a rinse screen. As he was swinging the hammer hit the cross member above deflecting the hammer striking his left middle finger causing a fracture to the tip of that finger. The impact of the hammer chipped the bone on the end of his finger.
A 350 HP motor was being set in place by employee. EE was attempting to line up mounting bolts with motor base when the motor shifted causing his finger to become stuck between wrench handle and motor platform.
Employee at Coal Mill using a hammer to fill the distance between the tie rod eye he was pushing out with a hydraulic jack. Surface of the hammer not being flat caused the hammer to slip out and the hydraulic jack struck the employee on the mouth. Injury resulted in one tooth being broken and another fractured.
Employee was unplugging a chute. A tool in use moved in an unexpected direction causing the employee to lose balance and strike the left hand on a piece of angle iron causing a cut. 10 stitches were needed.
Employee was replacing roller on conveyor. During installation of the new roller, the employee attempted to strike the roller in order to tap it into place. In doing so, he missed the roller can and hit his left index finger causing it to become lodged in between the roller can and frame. EE suffered severe laceration to left index finger and received eleven stitches.
Employee used utility knife to open 5 Gal oil container. While opening, employee cut towards himself with knife. The bucket slipped in the process and he cut his hand. Employee received stitches and returned to work.
Replacing plastic guiding chain on JLG 600S boomlift. Plastic Chain links began to burr up and ee used pocket knife to shave off burred plastic. Knife slipped and stabbed ee in left arm causing laceration.
EE was attempting to drive out a stuck 11/2" bolt on the East bearing of the West Secondary Crusher with a sledge hammer. During the process, a metal shard came off imbedding into his right calf. Following removal, the incision required two sutures to close.
Yard laborer employees foot metatarsal bone was fractured after the raised 1400 pound barrell of grinding balls he had accidentally stepped under slipped out of the lifting device from a height of several inches and came down on top of his foot. His steel toe safety boot did its job as designed.

EE was changing tire and was straightening the rim lock holder with a tire bar. Tire bar slipped and hit his upper front teeth. Minor chipping of front teeth. No other injuries or impairment.
EE was clearing a cardox port with a pipe and a hammer. As the ee struck the end of the pipe with his left hand, his right hand was caught in a pinch point between the pipe and the cardox collar. As a result, his right index finger suffered a laceration which required 8 stitches.
EE was cutting rubber with a razor knife when the blade came out of the knife handle cutting his left hand
Employee was lifting a 5 ton chain fall into position when he strained his right shoulder. Employee requested medical treatment on 4/18/12. The injury became reportable 4/30/12.
Striking a pin with a 20 lb. hammer. Pin diverted hitting finger between pin and hammer handle.
Employee was tightening the cap on the core using a 16 pound sledge hammer. it was getting dark and light conditions were low. he swung at cap with hammer missing it. He attempted to stop his fallen thru and strained his lower back.
Employee was securing barge with capstan rope when the rope shifted and sent a shock wave through rope fracturing the employee's right middle finger.
Employee cutting a hole in liner for kiln feed pipe with lock blade knife. Lock mechanism on knife was accidentally disengaged and blade closed on top of right index finger causing laceration. Laceration required five sutures to close.
Employee was changingout ripper tooth on D11 dozer. He struck the tooth with a 4 lb sledge hammer. A piece of metal came off the tooth stricking him in the right calf. A small chunck of metal (1/4 size of a dime). Doctor removed from leg and put in 2 stitches.
While performing preventative maintenance on #2 Kiln Induction Fan, employee's left leg struck a puddy knife that had been left between a bolt & column by another employee. Sutures required to close laceration.
Employee was trying to roll the ring cover over when the pry bar slipped out of the bolt hole. When the pry bar came loose it allowed the plate to roll back over landing on top of the employee's right foot. Employee is on light duty.
While hammering a new tooth on a new shank on a 980H a piece of the tooth broke off striking him in the forearm. The metal chip penetrated the skin and lodged about 3 inches up his arm. The employee required 2 stitches and will need to return to the doctor to have the chip removed.
Employees were removing liners from ball mill. Employees were using metal wedge and sledge hammer to break liners loose. Upon hitting liner a piece of metal broke off (1" X 1/4") and stuck in miners thigh. 5 stitches.
Employee was working on fan trying to remove a coupling. The center bolt would not come loose. Tried using a large break over bar, then used a large pipe to get more leverage. When applying pressure the adapter broke allowing the employee to go forward twisting lower back as he went down. Employee is on restricted duty.
EE was removing the pfister feeder housing bolts. He was pushing an adjustable wrench when it slipped from the nut. The wrench hit a side brace of the pfister feeder frame, pinching the tip of his left ring finger.
Installing large bull gear on roll crusher at Wash Plant One, using a 8 lb. hammer to strike a hammer wrench to secure the nut that holds the gear. Hammer glanced off wrench and hit left knee.
Employee was cutting a zip tie off of a new tape measure using a knife. When the tie cut, the knife slipped stabbing his left hand between the thumb and forefinger. Employee received medical care. Received 3 stitches.

Removing bolts on a truck differential, two bolts broke off as he was pushing and pulling and pulled a muscle in back.
Employee was using a razor knife to cut conveyor belt. As he was making the cut away from him the knife slipped cutting his left forearm. The resulting cut was approximately 1/2" in length.
Employee was using a Sawzall when his finger slipped down between the blade head and saw housing, smashing his left little finger, resulting in a fracture and laceration.

Employee was breaking loose bolts on steel structure with a wrench, when he felt a pain in his right shoulder and neck.
Individual slipped off a 12 in. dia pipe he was standing on while operating a chain hoist striking his elbow on the corner flange of the duct work resulting in a deep contusion and bursitis in the right elbow. Individual was treated by the site nurse and then sent to the Doctor where it was eventually drained and injected with steroids.
EE was barring plugged up material in a bin and smashed his finger between bar and bin.
While cutting conveyor belt to make a splice, knife slipped and went into his right knee.
Employee was cleaning buildup at 291 BC3 head pulley. Smashed finger between bar and concrete - 6 stitches.
EE was untangling an extension cord when another worker (employed by a different contractor) dropped an unsecured chain fall onto him. EE was struck on his hard hat and the chain fall also struck his shoulder. The injury became reportable on 10/1/08 when physical therapy was prescribed.
While cutting plastic band off of door hardware, the utility knife slipped off of the plastic band and cut his his left thumb.
Employee was torquing bolts using a torque wrench. He used his shoulder to push the wrench upward causing a compression injury to his shoulder. Injury was not diagnosed as a tear to tnedon initially and after months of continuing issue he has elected to have surgery to repair.
