An employee was changing out a bad motor on the 8 X 24 screen at the wash plant. EE removed the mounting bolts and then was using a prybar on the base of the motor to rotate it clockwise. While prying in an upward motion from the base of the motor the prybar slipped and EE's momentum caused EE's right hand to strike the electrical box on the side of the motor.

While cleaning in a tight area with shovels, another employee slipped in the mud and impacted the employee in the mouth with the wood handle of a shovel.

Employee was working on breaking bolts free on the waste pump to inspect inner components. When the second bolt and nut broke free it jarred employee forward causing a slight pop and pain in right shoulder. Employee was using an adjustable wrench for the job because employee did not have a big enough wrench/socket for the job.

Mechanic was torqueing boom bolts on a crane to 2100ft lbs., the employee switched from torque wrench to a 24" swivel head ratchet to remove the pressure from the torque multiplier. As EE pushed down, the ratchet handle slipped out of EE's hand and struck EE in the face resulting in a fractured and lacerated nose.

Swinging a 4# sledge hammer with both hands to free up a cut piece of metal from beam. Metal broke free and upon completion of swing the left hand middle finger contacted the metal beam, resulting in a fracture of the middle finger on the left hand. Employee is right hand dominant.

Employee was using a 5ft long steel bar to unplug a chute with 8"x18" material on the RIPRAP trammel when ee pinched finger between the bar and a steel plate causing a laceration.

Employee was removing chutes from the twin screw area preparing for its removal and replacement. EE was moving a section of a chute that was cut off; it was suspended by a 1-1/2 ton chain hoist. As EE was lowering the chute EE grabbed the bottom of it with right hand to guide it in place. The chute swung left and pinched EE's right middle finger.

Employee was removing large stones from the hazemag impact crusher (a blockage occurred in the discharge area). The employee was standing above the rotor and off to the side to get a good angle with a 6ft pry bar to dislodge the material, when a stone approximately 4'x 2' fell out of the chute and hit the rotor causing the stone to bounce up and strike the employee's left shin.

Employee was on a catwalk next to the screen making a 4"x7" notch in a 10' strip of belting with a utility knife, when the knife slipped and struck his left wrist.

While adjusting shims on crusher pry bar slipped off and struck employee in the cheek causing a cut. Laceration to right cheek, needed eight sutures to close wound.

Employee found an issue with a power cable splice on SC65 screw, he was using a retractable safety knife to cut the outer electrical jacket off the splice when the knife came down cutting him below the left thumb. It took seven stitches to close the wound.

Employee was repairing screen, as he lifted the balance wheel sheave with a 1 1/2 ton coffin hoist the nut on 1/2" eyebolt backed off, this allowed the balance wheel sheave to fall hitting employee on both legs below the knee, landing on the deck, and falling over onto right ankle. Contusion to both legs and right ankle.

EE was using a utility knife to cut rubber belting. The blade went through easier than expected and the blade struck him in his right thigh. 3 stitches were required to suture the wound.

Michigan State Police came to the Holly Sand & Gravel site because a missing person and possible suicide had been reported. At 11:20 AM they found a body on Holly Sand & Gravel site. This person was not an employee of Holly Sand & Gravel. The deceased person had died for a self inflicted gun shot wound to the head.

Employee was changing screens on the plant and pounding on bolts with a hammer creating dust. The next day he woke up with his left eye sore/burning and went to the hospital. The injury was reported to the supervisor on the following Monday

While changing flashing on a conveyor using a hammer and punch for bolt holes. EE hit his left hand with the hammer.

Employee alledges while working on the north side of #3 tower, loosening bolts on the screen deck and pulling a wrench toward his body, he felt a pop in his right shoulder. The pain continued for the remainder of the day, increasing through the night. He went to the clinic the next day where he was given medication to take every 4 - 6 hours as well as restricted work duties.

While changing bucket teeth on loader, tooth fragment broke loose and went into left thigh.

Cutting belt holes in belting, knife slipped and cut left thigh.

EE and co-worker were removing teeth from the drag bucket. Injured had a large pin hammer and co-worker had a sledge hammer, hitting the pin hammer. Pieces of steel from the tooth on the bucket chipped off and hit ee in the calf of his left leg. He ended up with a piece of steel lodged in his left calf.

Employee was repairing a splice on the field conveyor. While cutting out a patch to fill the hole, the utility knife passed through the belt (patch material) and struck right index finger. Finger was lacerated requiring 5 sutures and a tetanus shot.

While installing side bits on a loader bucket, employee hit the bit with a hammer and a piece of steel came off hammer and hit left index finger. Employee went to clinic on 4/25/05. He was dispensed 800 mg of Motrin, 500 mg. of Ciprofloxacin and given a splint to wear on finger. Has follow up visit scheduled for 4/29/05.

Employee was lying on the ground underneath loader trying to tighten bolts on center pin using a ratchet and breaker bar. The bolt snapped off and the employee jerked himself out of the way from the falling cheater bar. At this time he felt some pain in his shoulder and on up into his neck. Pain increased as the day progressed.

EE WAS CLEANING GRIZZLY ON JAW FEEDER WITH A 8 LB HAMMER. THE HAMMER CAME IN CONTACT WITH THE UPPER BEAM SUPPORT AND STRUCK EE ON LEFT SIDE OF HIS HEAD.

EE CUTTING CONVEYOR BELT WITH UTILITY KNIFE. KNIFE SLIPPED & CUT INTO LEFT PALM OF HAND. 7 STITCHES REQUIRED.

WHILE HELPING ANOTHER EMPLOYEE LIFT UP THE BUTT SECTION OF A BOOM USING A PRY BAR TO PULL OUT A CO-WORKER FROM UNDERNEATH THE BOOM, THE EE INJURED HIS RIGHT ELBOW. EE DID NOT VISIT MEDICAL FAC

EE WAS USING A 5' PRY BAR TO FREE AN OLD LINER FROM A 4 1/2' SHORT HEAD MANTLE. THE PRY BAR SLIPPED & THE EE LOST HIS BALANCE. WHILE STEPPING BACKWARDS TO CATCH HIS BALANCE, HIS LEG TURNED & A S THIS HAPPENED HE FELT A SNAP IN HIS LOWER LEFTLEG. AT THAT TIME, HE FELL TO THE GROUND. SURGERY WAS REQUIRED BUT NO MEDICAL REPORTS HAVE BEEN RECEIVED AT THIS TIME.

WHILE BREAKING UP FROZEN MATERIAL ON A CONVEYOR A LARGE CHUNK FELL ON L. HAND. CAUSING BRUISING AND SWELLING OF INDEX AND MIDDLE FINGER-L. HAND.

SPLICING C-11 CONVEYOR BELP-RAZOR KNIFE SLIPPED L. LEG.

EMPLOYEE WAS SPLITTING A ROCK WHEN A PIECE OF THE ROCK FLEW BACK AT HIM AND STRUCK HIM ON THE LIP.

THE BRACE HE WAS HOLDING SLIPPED AND PINCHED HISFINGER.

EE IS A STONE CUTTER. IN SPLITTING A ROCK, 1/2 OF PIECE FELL ON EMPLOYEE'S LEG, INJURING THE KNEE.

WHILE TIGHTENING DRILL BIT WITH SCREW DRIVERS RIGHT HAND SLIPPED AND CUT LITTLE FINGER ON DRILL BIT. REQUIRED C9 STITCHES TO CHOSE WOUND.

HAMMERING ON CHUTE W / SLEDGE HAMMER. SMASHED R THUMB.

THE EE WAS STRUCK IN THE CHEST BY A BAR WHILE WORKING ON AN OVERSIZED CONVEYOR.

A STACKER BELT HEAD PULLEY HAD JAMMED. WHILE FREEING UP THE PULLEY, THE EE STRAINED SOME STOMACH MUSCLES.

WHILE REMOVING A RIPPER TOOTH FROM A DOZER EE HIT TOP OF TOOTH WITH A HAMMER CAUSING A PIECE OF METAL TO CHIP & ENTER THE THIGHT OF THE RIGHT LEG

WHILE SHOVELING DIRT OUT FROM BENEATH CONVEYOR MATERIAL OVERFLOWED FROM DROP BOX & FELL ONTO FACE OF EE EE WAS WEARING SAFETY GLASSES AT THE TIME

PRIMARY CRUSHER CHUTE PLUGGED. EE CRAWLED UNDE W/10# SLEDGE TO BREAK ROCK OUT.

EMPLOYEE LACERATED THE TIP OF HIS RIGHT MIDDLE FINGER 1" WHILE CUTTING THE CONVEYOR BELT TO SQUARE THE ENDS UP. THE BELT KNIFE SLIPPED. IT WAS RAINING AND DARK, ALTHOUGH THERE WAS GOOD LIGHT ING ON THE JOB SITE.

EE STRUCK ON RT KNEE BY HAMMER USED BY CO-WORKER. Y

THE EE WAS ATTEMPTING TO LOOSED A BOLT WHILE ADJUSTING A JAW CRUSHER AND STRAINED HIS LOWER BACK WHILE DOING SO

WHILE CHANGING ROLLERS ON CONVEYOR, EE SMASHED HIS FINGER BETWEEN THE CONVEYOR FRAME AND THE ROLLERS.

CUTTING STRAP OFF BLOCKING. KNIFE SLIPPED - CUT R. LEG.

WHILE EE WAS PULLING ON A COME-A-LONG, THE LOAD SHIFTED. THIS RELEASED LOAD ON THE CHAIN AND EE'S ARM MOVED INTO BOLT.

TURNING CRANK ON MASON PLANT, CRANK SLIPPED OUT OF HAD. HIT BACK OF RIGHT HAND.

WHILE LOWERING CONVEYOR WITH CHAIN, CHAIN MOVED,STRIKING EE ON WRIST BREAKING RIGHT RADIUS.
