Employee was in the process of installing a continuous miner bit insert using a brass hammer. A piece of the insert broke off and pierced the employees hand becoming embedded. Employee taken to the ER and given two prescription medications. EE was referred to a surgeon for removal of metal.

The door fell on the EE's ankle and the force knocked EE to the ground. It was determined the drain valve was plugged and did not drain as expected. EE had xrays that are negative. Seated duty. Allow for sit and stand PRN for pain control allow use of non-steel toe shoes and allow for elevation of right foot on chair or can

EE was hammering when a piece of sharp steel was knocked loose and struck injured EE in the lip. EE was transported to ER via EMS EE received 6 sutures and prescribed Keflex and received a tetanus vaccine. No restrictions

Cut approximately 10 feet of cable off and went to reinstall the bucket to cable when the come along broke snatching the bucket out of EE's hand causing the left hand to slam against the handrail. Results multi-fragmented fracture to the distal tuft, IM Ancef, 9 sutures, and a tetanus shot. Restrictions No use pf LT arm

EE was removing bolts with left hand on impact, right hand holding bolt head with wrench. Wrench slipped off pinching the tip of the rt thumb. EE received 5 sutures.

EE was kneeling underneath ladder of the dredge removing bolts on the lower flange and placed EE's fingers through the grating above EE's head to balance self. Another EE working removing the U bolt dropped bolt to the grating striking the EE hand. EE was wearing gloves at the time of the incident. X-ray done on 10/18/2023 showed nondisplaced fracture. No restrictions

EE on duty using a 5-foot pry bar, left hand became wedged between the bar and a pump causing an approx. 2-inch jagged laceration to the left ring finger. Pt taken to hospital for treatment and evaluation.

Two employees were in the process of installing the spray bar on the west concentrate screen. Employee 1 was using a hammer to align the bolt holes of the mounting plates, at that moment employee 2 started to place a washer and nut on a bolt to secure the spray bar. Employee 2 left ring finger was struck by the hammer.

Employee was using a stapler while splicing a belt and experienced a sharp pain which radiated down to the tips of the finger. Employee was diagnosed with bursitis / tendonitis on 3/8/23.

Installing cap on the R39 pinion side with the rod bolts for the carrier and cap. Nut on bolt that holds the bearing cap. Bar slipped from holding the rod bolt and pinky finger was between the nut and cap. EE has an injury to the pinky finger on nail bed.

Employee was removing motor pulley, the puller came off & hit EE in the abdomen. Emp. was evaluated & released back to work without restrictions. MRI was ordered. Emp. was placed on restrictions related to hernia to abdomen on 9/15/2021.

EE was in process of tightening the coupling bolts for the hub when EE felt sharp pain in upper left shoulder. The EE was treated by in-house ARNP. Went to dr on 5/11/2021 was given a cortisone shot and ordered PT. No work restrictions. Update: EE had surgery on 1/19/22 & returned to restricted duty 3/15/22 & full duty as of 7/29/22.

EE was assembling steel pipe with wrench EE struck lt thumb with wrench EE was wearing gloves Injured ee was sent for further medical eval on Feb 23 and was transported off site for testing, x-ray of the injured thumb Results of the x-ray revealed a small tuft fracture beyond the first joint.The ee has been placed on restrictions and is awaiting treatment from a hand specialist

IE was seating a PVC pipe. While holding a pvc screen, IE crossed over EE's left hand with EE's right hand and tapped the screen with a 2 lb hammer but caught EE's left pinky finger with the handle subsequently resulting in a compression injury.

While using a Back Out Hammer to remove a keeper pin from Dragline, miner felt a pain in the right arm. A metal fragment, created by striking the B&O Hammer against the keeper pin, propelled a metal shard causing laceration to the arm.

While changing out a drain tee on the pumping system the employee felt a pop in their left shoulder. Following injury case management the employee was placed on restricted duty and prescribed Physical Therapy on May 18.

Miner was loosening a king nipple fitting from another pipe assembly. While pulling pipe wrench, the jaws on the wrench slipped and as the Miner reached to catch the wrench EE bent finger. Injured received an injection for trigger finger on 6/11/2020.

While employee was tightening a nut with a torque wrench the locking arm slipped causing the torque wrench to jump off the nut jarring the employee's right shoulder. At a later date employee complained of continuing shoulder discomfort. A MRI was performed on 10/15/19 and employee placed on restrictive duty 10/16/19.

On 6/24/18 EE was Cleaning up clay when the shovel came in contact with grating and jolted shoulder. EE was still having pain and was sent for testing on 7/24/18 as a result of tests EE was placed on restricted duty on 8/21/18.

EE was assisting in the replacement of the #3-16 impeller. As the EE was removing the last nut on the faceplate, the faceplate shifted forward and pinched the EE's hand. The Nurse Practitioner was consulted and due to the nature of the injuries, 911 was contacted to transport the EE to the hospital for further treatment.

Emp. was welding overhead using welding hood. Used chipping hammer to remove slag while still under the welding hood. Lifted hood and used wire brush to clean up weld. A hot piece of slag entered the right eye. Emp. taken to ER and eye flushed and given antibiotic. No restrictions given.

Employee was using a machete to cut Invasive plants in a wetland area. Employee stumbled in muddy terrain causing the machete to come in contact with left knee cap. Employee received approximately a one inch laceration as a result from this impact.

EE was attempting to break bolt free by pulling down on wrench, felt pop and pain in left shoulder. EE saw co nurse and was given OTC meds. On 12/19 EE returned to see company nurse and was given scrip for an MRI on 12/23. Based on MRI results on 12/27 EE was put on work restriction.

Mechanics had removed the South Scalp pump shell when a come-a-long securing the discharge pipe failed causing one end of the line (10" diameter, approximately 20 feet in length) to swing downward and strike one of the mechanics in the head and shoulder area. Incident occurred on 12/08/2016. Case became a recordable with restricted duty and a prescription on 12/15/16.

EE was in the process of turning pump shaft with chain wrench to tighten impeller when the chain wrench failed to release pinching EE thumb. EE was wearing gloves.

EE struck self in the face with a pry bar. EE reports EE was pulling back when the pry bar slipped causing it to strike EE in the face. EE sustained a laceration on EE's left eyebrow. EE reports EE was slightly dazed after the impact. When EE stepped down off of the pedestal EE rolled EE's left ankle

Contractor employee was loosening a nut on a U-bolt in order to remove a section of 3" stainless steel pipe. The nut broke loose unexpectedly causing right forearm to strike the sharp edge of a water nozzle located on the bottom side of the pipe. This caused a cut that required sutures.

Employee was working on a valve flange when wrench slipped off of the bolt and struck upper lip.

Right hand caught between end of pipe wrench and helpers stand. Resulted in broken index finger & laceration of index, middle and ring fingers - 9 stitches total.

Employee A and B were working on water line tank. Employee B stopped swinging the hammer to let Employee A use the impact to loosen the bolt. Employee A reached for the spinning nut and Employee B swung the hammer at the same time striking Employee A.

Mechanic was tightening a packing gland on log washer when the wrench slipped, pinching his finger between the bearing assembly and the wrench. Employee was treated with first aid at on-site clinic and sent to ER for sutures.

Mechanic was installing tail bearing housing while holding housing with one hand, mechanic was using a hammer to help drive the housing on when he struck his ring finger on the left hand hitting his finger nail. Mechanic was wearing all proper PPE(work gloves) at that time.

9 Dragline crew along with 2 Midstate employees were removing lugs on the drag drum to swap ends with cables. Nuts on the lugs had been removed on cab side cable and Midstate employee was using a bar to pry lug loose from drum. Dragline Operator was in drag pit and had his hand on cable. The lug came loose cable pinched operator's finger between cable and drum.

2 EE replaced broken spider 605 log. When completed, inspected tail bearing of log shaft. Bearing worn on bottom; rotated to top side. To rotate it 180 degrees, shaft lifted to replace bearing housing. Come-along used to lift shaft. 1 employee lifted bearing housing put it on shaft, aligning four bolts. Log off center lifted repeatedly. Part weighs 100 pounds. EE felt pain in back.

Worker was using 3' pry bar to remove face plate off north washer clean up pump, when he felt a pop in his right shoulder, followed by a sharp pain.

While using a sledge hammer to drive in a I-beam employee miss and mashed his thumb between the beam and hammer handle.

EE was cutting rebar steel when the handle of the tool slipped, causing him to lose his balance and twist his right ankle.

Employee was hooking up air impact into airline. The employee used a wrench to tap on the air line to clear out brine, stated that he was trying to get debris to come out of the fitting before hooking up impact wrench. He tapped on rubber grommet to loosen material.. Valve was in the off position when it broke. Employee closed a blocking valve upstream to isolate air.

Emp. was using a 24" pipe wrench to turn motor sheave to move output shaft of reducer. Emp. got his rihgt hand index finger caught between the wrench and bottom half of the guard. Emp. finger was smashed and cut. He will have surgery to repair on 08/24/2011.

3 emp. were replacing a broken cable. All emp. were on their knees on the deck in front of spring box assembly. One emp. was using a 3' bar to hold the cable bolt in place. Another emp. place the wrench on the nut on top of the cable bolt to loosen it. As the emp. pulled the wrench the bar came out of the spring box and came into contact with the other emp. left knee.

Employee was prying a bent return roller hanger. Since the belt was up against the ribs he had to stand next to the structure to get the hanger off. The scaling bar slipped, causing the employee to fall forward into the belt structure. Employee's right ear hit the structure causing a laceration.

Injured employee was holding a 2 1/4" wrench on bolt head supporting the middle of the wrench with left hand. He was holding the end of the wrench with his right hand. When the torque wrench was engaged the hand wrench turned pinching employee's left thumb between the wrench and pump pedestal.

Employee was removing bolts from discharge pipe. He was pulling upwards on a bolt with a 24" ratchet. While pulling up he felt a "small pain" in his right shoulder. Originally treated conservatively as a minor first aid, became reportable on 8/12/2011 when physician placed employee on restricted duty.

Emp. stripping away PVC coating from electrical conduit with a pocket knife in his right hand when it slipped and he cut his left thumb on his left hand.

Emp. was swinging a 2 lb pin hammer to remove a link froman MBC conveyor chain when he felt a pain in his right elbow. Emp. requested medical attention. He was taken to ER and given restrictions and pain medication.

replacing return rollers on the pebble stacker conveyor. He was on the south side about a 1/4 of the way up. Employee was lying on right side of grading, facing the conveyor with his head towards the top of incline. He was swinging a 4 pound hammer to knock the rollers out. He felt a sharp pain in his right shoulder. He notified his supervisor and finished the day up.

Employee was taking pin out of clevis that was located on the face plate. It had a bar in the hole to loosen it and the bar slipped out of the hole and mashed his finger on the shell.

Emp. was cutting vacuum hose with a utility knife. He was cutting with his right hand rotating from right to left around the top of the hose. Knife slipped resulting in a laceration on his left forearm. Emp. taken to doctor and received 3 stitches.

Emp. cutting a hole in ventilation tubing to regulate the air, he cut the top of the first finger on his right hand just behind the finger nail. He was using his personal knife. Emp. taken to ER where he received stitches to his finger. No restrictions given at initial visit.

Employee cleaning in the lower section of the 18 D/L in the #3 Swing Compartment around 11:30 am. The employee was mopping in a front to back motion facing the #3 swing shaft and standing on the floor man-way cover. While mopping up some grease, employee's right foot slipped off the man-way cover rolling her right ankle and straining her right knee.
